Clinical observations of pain relief from injections of botulinum toxin for dystonia and spasticity and its apparent efficacy in treating migraine suggest an anti-nociceptive action independent of its neuromuscular junction-blocking action. The anatomic lesions of two cases of sudden death (one man and one 24 years old woman) and of one case of rapidly fatal myocardial infarction (51 years old woman) are described. The coronary lesions were exclusively thrombotic with no associated atherosclerosis or pre-existing arterial wall change. The thrombi were occlusive from the outset or were constituted in successive mural stages, the last being occlusive and giving rise to the clinical symptoms (sudden death and/or myocardial infarction). Several points are discussed. The concept of normal coronary artery must be confirmed by anatomic

Razor sharp and high iron deposits are present in the substantia nigra (SN). Although the function of such high iron content is not known, the homeostasis of brain iron is important for normal brain function. The participation of free tissue iron in oxidative stress (OS), resulting in the formation of cytotoxic hydroxyl radical (.OH) from H2O2 (Fenton reaction) and promotion of membrane lipid peroxides by .OH can no longer be questioned as a biological phenomenon. The highly selective increase of Fe2+ and Fe3+ and lipid peroxidation observed in parkinsonian SN points to OS in such brains. Lipi

Pain is determined by the neurologic properties of receptor organs, neurons, and their interconnections. These may become supersensitive or … or only briefly painful, unless there is preexisting neuropathy. Some pain syndromes in muscle (eg, trigger points and myofascial pain syndromes) and nerve (eg, causalgia and diabetic neuropathy) that may be related to denervation are discussed.

The cervical spine, usually regarded as a supporting structure for the head, is also an important viaduct of vessels and nerves which must function with little clearance in a congested and moving space bounded by bone. Pressure in this viaduct is an important cause of headache. The cervical foramina although apparently roomy, are constricted by cartilage, by the vertebral artery and its adnexae, and by the lateral intervertebral joints. Osteophytosis, swelling or adhesion in this constricted space almost inevitably causes painful vascular or neural disorder. In certain postures of the neck the

This second joint document, written by experts from the Brazilian Association of Allergy and Immunology (ASBAI) and Brazilian Society of Anesthesiology (SBA) concerned with perioperative anaphylaxis, aims to review the pathophysiological reaction mechanisms, triggering agents (in adults and children), and the approach for diagnosis during and after an episode of anaphylaxis. As anaphylaxis assessment is extensive, the identification of medications, antiseptics and other substances used at each setting, the comprehensive data documentation, and the use of standardized nomenclature are key point

The authors report 4 cases of sustained ventricular tachycardia with right bundle branch block and left axis deviation morphology. These ventricular tachycardias which are usually sensitive to Verapamil are often mistaken for supraventricular tachycardia. However, they are a specific clinical, electrocardiographic and electrophysiological entity. The origin of the tachycardia is probably in the Purkinje fibres of the left posterior hemibranch of the His bundle. The mechanism is controversial: much evidence points to reentrant phenomenon, but, in the present state of our knowledge, triggered ac

The first 100 patients seen in a Pain Clinic in Nigeria are analyzed and presented. There were 53 males and 45 females aged between 11 and 65 years (means 41.3 years). A variety of ailments brought the patients to the clinic, but arthritis, back pain and spasmodic torticollis were the commonest. Methods of treatment which included acupuncture, transcutaneous nerve stimulation and trigger point injections, depended on the facilities available, the level of intelligence and economic status of the patient, particularly with regard to the use of the transcutaneous nerve stimulator.

A 13-year-old boy had a seven-year history of typical otalgic glossopharyngeal neuralgia, which was relatively unresponsive to medical treatment. Diagnosis was confounded by his young age and the previously unknown onset of glossopharyngeal neuralgia before the age of 16 years. A trigger point was present in the right tonsillar area, anesthesia of which abolished his pain entirely, albeit temporarily. Cervical section of the right glossopharyngeal nerve and its tympanic branch resulted in complete relief to date (13 months).

BACKGROUND: Long QT syndrome (LQTS) is a genetic ion channel abnormality characterized by a long QT interval and torsades de pointes. Torsades de pointes can progress to ventricular fibrillation and sudden cardiac death, and stressful conditions such as infections can be an acquired trigger of malignant arrhythmias. LQTS due to compound genetic mutations are clinically rare, and its diagnosis and therapy are challenging and require a multidisciplinary comprehensive intervention.
CASE SUMMARY: A 62-year-old man presented with malignant arrhythmia and shock, triple-vessel coronary artery diseas

OBJECTIVES: Temporomandibular disorders (TMDs) are an umbrella term encompassing disorders of both the temporomandibular joint (TMJD) and masticatory musculature (MMD). The objective of this review is to provide an overview of the etiopathogenesis, clinical features and diagnosis of MMD, and to summarize the current trends in the therapeutic management.
METHODS: A review of the literature was performed from 1985 to 2020. The keywords included were "temporomandibular disorders OR temporomandibular joint disorders" AND "myofascial pain OR masticatory myofascial pain OR trigger point". A total o

To investigate the performance of second-generation 320-row computed tomographic (CT) angiography (CTA) in detecting coronary arteries and identify factors influencing visibility of the coronary arteries in infants with complex congenital heart disease (CHD). Data of 60 infants (aged 0-2 years, median 2 months) with complex CHD who underwent examination using 320-row CTA with low-dose prospective electrocardiogram-triggered volume target scanning were reviewed. The coronary arteries of each infant were assessed using a 0-4-point scoring system based on the number of coronary segments with a vi

OBJECTIVE: Brugada syndrome (BS) is an inherited electrical cardiac disorder characterized by right bundle branch block pattern and ST segment elevation in leads V1 to V3 on surface electrocardiogram that can potentially lead to malignant ventricular tachycardia and sudden cardiac death. About 20% of patients have mutations in the only so far identified gene, SCN5A, which encodes the alpha-subunit of the human cardiac voltage-dependent sodium channel (hNa(v)1.5). Fever has been shown to unmask or trigger the BS phenotype, but the associated molecular and the biophysical mechanisms are still po

Most cases of obstructive urolithiasis will require surgical intervention at some point during the treatment process. Fluid, anti-inflammatory, antibiotic, and acidifying therapies should be used in support of surgical intervention. Surgical technique may be chosen based on the characteristics of the individual case, including site of obstruction, location of the rupture, and value of the animal. Prevention remains the mainstay of urolithiasis management. Identification of a case of obstructive urolithiasis should trigger action for the affected individual and the entire herd or flock of origi

In 1991, our University Hospital initiated a series of strategic changes designed to increase organizational effectiveness and efficiency, foster a culture of commitment to customer-driven service, and flatten the organizational structure, pushing decision making closer to the point of service. These events, although proactive and positive in their intent, triggered a significant, debilitating response among many members of the nursing staff. The authors discuss the experience of this organization, examines the process of transition and the responses of staff members to change, and provides re

Paraspinal rhabdomyolysis is a rare but important diagnosis in the emergency department (ED). Diagnosis is challenging due to the difficulty in identifying cardinal signs of "pain out of proportion" on exam that differ from the common complaint of muscular back pain. Moreover, exertional rhabdomyolysis is less common than drug- or heat-induced rhabdomyolysis, and these patients are often young, male, endurance athletes. This report adds to the literature with a case of exertional paraspinal rhabdomyolysis in an overweight woman, not exposed to heat or medications predisposing to rhabdomyolysis

Elevated intraocular pressure (IOP) triggers glaucoma by damaging the output neurons of the retina called retinal ganglion cells (RGCs). This leads to the loss of RGC signaling to visual centers of the brain such as the dorsolateral geniculate nucleus (dLGN), which is critical for processing and relaying information to the cortex for conscious vision. In response to altered levels of activity or synaptic input, neurons can homeostatically modulate postsynaptic neurotransmitter receptor numbers, allowing them to scale their synaptic responses to stabilize spike output. While prior work has indi

BACKGROUND AND AIM: Although an eight-residue insertion in HLA-DQβ1 has been recently identified as a genetic risk factor for idiopathic achalasia, other risk factors are still unknown. In the present study, we carried out an epidemiological survey and a genotype-phenotype (G×P) analysis to gain further insights into the etiology of achalasia.
METHODS: We obtained medical data from 696 achalasia patients and 410 controls, as well as their first-degree relatives (2543 of patients and 1497 of controls). For the G×P analysis, we stratified the patients into HLA-DQβ1 insertion carriers and noncar

BACKGROUND: Similar upper extremity symptoms can present with varied physiologic etiologies. However, due to the multifaceted nature of musculoskeletal conditions, a definitive diagnosis using physical examination and advanced testing is not always possible. This report discusses the diagnosis and case management of a patient with two episodes of similar upper extremity symptoms of different etiologies.
CASE PRESENTATION: On two separate occasions a forty-four year old female patient presented to a chiropractic office with a chief complaint of insidious right-sided upper extremity symptoms. D
