Emergency Alcohol Septal Ablation for Shock After TAVR: One More Option in the Toolbox.

We hereby report a case of severe shock from left ventricular outflow tract obstruction following transcatheter aortic valve replacement that did not respond to medical therapy and had to be treated with emergent alcohol septal ablation (ASA). Emergent ASA should be considered for bail-out treatment for these refractory cases. (Level of Difficulty: Advanced.).

Aortic arch interruption in infancy: radio- and angiographic features.

The radio- and angiocardiographic characteristics of 19 infants with an interrupted aortic arch were reviewed. There were three with type A (distal to the left subclavian artery), 15 with type B (between the left carotid and subclavian arteries), and one with type C (between the innominate and left carotid arteries). Associated cardiovascular anomalies were numerous, a ventricular septal defect (19/19) and a patent ductus arterisous (18/19) being the most common. Plain film signs of aortic interruption were nonspecific. Left ventriculography using angled oblique views was diagnostic of the int

Transesophageal echocardiographic (TEE) evaluation of the aortic valve, left ventricular outflow tract, and pulmonic valve.

The most important role of TEE in aortic valve disease is in the diagnosis of endocarditis and its complications. Examination of the annulus and subvalvular region is essential in any patient with possible aortic valve endocarditis. Assessment of the severity of aortic stenosis is a useful application of TEE when other data are either inconsistent or unavailable. TEE can provide a diagnosis of the origin of acute severe aortic insufficiency; this information may play a critical role in surgical planning. The diagnosis of a variety of aortic valve diseases can be made when TEE is performed to f

Balloon aortic valvuloplasty for critical aortic stenosis in neonates and infants: early outcomes from a single-center cohort.

BACKGROUND: Critical aortic valve stenosis in neonates and infants is a life-threatening congenital heart disease characterized by severe left ventricular outflow tract obstruction and compromised systemic perfusion. Balloon aortic valvuloplasty (BAV) has become an established initial treatment option; however, data on early outcomes in this vulnerable population remain relatively limited. METHODS: This retrospective single-center study included 30 neonates and infants (≤ 12 months) with critical aortic valve stenosis who underwent balloon aortic valvuloplasty between 2016 and 2024. Clinical,

Incidental Murmurs and the Elective Noncardiac Surgical Patient: A Case Report.

We present a case of left ventricular outflow tract (LVOT) obstruction detected by limited bedside transthoracic echocardiography (TTE). This involved a young and otherwise healthy patient presenting for elective hand surgery with a previously undetected cardiac murmur. It highlights the utility of bedside TTE as an assessment tool and shows the importance of anesthesiologists as perioperative physicians.

[Right ventricular outflow obstruction due to a giant pseudoaneurysm of the anterior descending coronary artery in a patient with Behçet's disease].

Giant pseudoaneurysms of coronary arteries in patients with Behçet's disease is a uncommon finding. It has been described exceptionally in the literature. We present a case of giant pseudoaneurysm of the left anterior descending coronary artery with obstruction of the right ventricular outflow in a patient with Behçet's disease. He improved after surgical resection and steroid treatment.

Maternal hyperoxygenation and foetal cardiac MRI in the assessment of the borderline left ventricle.

Using phase-contrast MRI in a foetus with borderline left ventricular hypoplasia at 37 weeks' gestation we showed an increase in pulmonary blood flow during maternal hyperoxygenation. The associated increase in venous return to the left atrium, however, resulted in reversal of the atrial shunt, with no improvement in left ventricular output. The child initially underwent single ventricle palliation with a neonatal hybrid procedure, but following postnatal growth of the left ventricle tolerated conversion to a biventricular circulation at 5 months of age. We conclude that when there is signific

Intracardiac yolk sac tumor and dysrhythmia as an etiology of pediatric syncope.

This report describes a new etiology of pediatric syncope. Epilepsy, brain anomalies, infection, electrolyte abnormalities, and trauma are commonly identified etiologies of seizures in the pediatric population. We report here a child with third-degree heart block and right ventricular outflow tract obstruction related to an intracardiac tumor presenting with syncope and seizure-like activity. Echocardiography revealed a large (3 x 8-cm) intracardiac mass filling the right atrium, extending across the tricuspid valve into the right ventricle and crossing the atrial septum into the left atrium,

Percutaneous septal ablation for left mid-ventricular obstructive hypertrophic cardiomyopathy: a case report.

BACKGROUND: Mid-ventricular obstructive hypertrophic cardiomyopathy (MVOHC) is a rare type of cardiomyopathy. The diagnosis is based on the hourglass appearance on the left ventriculogram and the presence of pressure gradient between apical and basal chamber of the ventriculum on the hemodynamic assessment. CASE PRESENTATION: The present case represents successful percutaneous treatment with septal ablation to patient with MVOHC associated with systolic anterior motion of the mitral valve and obstruction at both the mid-ventricular and outflow levels. CONCLUSION: Alcohol septal ablation has

Adverse effects of nifedipine therapy on hypertrophic obstructive cardiomyopathy.

A 62-year-old man with obstructive hypertrophic cardiomyopathy was given sublingual nifedipine, 10 mg, during invasive hemodynamic monitoring. After 15 minutes, his left ventricular outflow gradient increased from 22 to 80 mm Hg while arterial pressure fell from 152/70 to 122/64 mm Hg. Left ventricular end-diastolic pressure increased from 15 to 22 mm Hg. These adverse hemodynamic responses may have been a result of vasodilation of the peripheral circulation induced by nifedipine. Thus, some patients with hypertrophic obstructive cardiomyopathy may develop serious hemodynamic compromise when t

Allograft root on closed pulmonary valve for subaortic obstruction in double-inlet left ventricle with transposition of the great arteries.

BACKGROUND: Until recently closure of the pulmonary valve during staged Fontan-type palliation in the setting of double-inlet left ventricle with an unrestrictive or adequately enlarged ventricular septal defect and transposition of the great arteries with the aorta on a left-sided outflow chamber was regarded as an appropriate part of surgical treatment. Lately, however, an increased incidence of subsequent subaortic obstruction has been described in this regard. METHODS: Allograft root placement on the previously closed pulmonary orifice in combination with a modified Damus-Kaye-Stansel pro

Long-term outcomes after surgical repair of complete atrioventricular septal defect.

OBJECTIVE: Survival after surgical repair for complete atrioventricular septal defect (CAVSD) has improved, but patients are at risk for reoperation to address left atrioventricular valve regurgitation and left ventricular outflow tract obstruction. We examined the long-term survival, need for reoperation, and associated risk factors after CAVSD repair at our institution. METHODS: Between 1974 and 2000, a total of 198 patients underwent surgical repair for CAVSD. Of these, 178 patients survived to hospital discharge, of whom 153 (86%) had available follow-up data at a median postoperative tim

Repair of Double-Chamber Right Ventricle and Subvalvular Pulmonic Stenosis in a 78-Year-Old With Repaired Tetralogy of Fallot: Anesthetic Considerations and Review of the Literature.

BACKGROUND: Double-chambered right ventricle (DCRV) is a rare congenital anomaly characterized by hypertrophied muscular bundles dividing the right ventricle into high- and low-pressure chambers. It is frequently associated with congenital heart disease, particularly tetralogy of Fallot (TOF), but rarely presents in late adulthood. CASE PRESENTATION: We report the case of a 78-year-old man with a history of prior pulmonary valvotomy who presented with progressive dyspnea and was found to have severe subvalvular right ventricular outflow tract obstruction. Multimodal imaging and cardiac cathet

Transient Ventricular Capture Loss Following Alcohol Septal Ablation in Left Bundle Branch Area Pacing: A Case Report.

BACKGROUND: Alcohol septal ablation (ASA) is an established therapy for symptomatic hypertrophic obstructive cardiomyopathy (HOCM) but may cause conduction disturbances. Data on interactions between ASA and left bundle branch area pacing (LBBAP) leads are limited. CASE SUMMARY: A 70-year-old woman with complete AV block and dual-chamber pacemaker with LBBAP developed severe LVOT obstruction (Valsalva 93.3 mmHg) with syncope and dyspnea. ASA of the first septal branch (4 mL 95% ethanol) caused immediate hypotension and transient loss of ventricular capture due to abrupt LBBAP threshold rise, r

Delayed systolic anterior motion after mitral valve repair.

A rare case of systolic anterior motion (SAM) after mitral valve repair is described. A temporary postoperative left ventricular (LV) dysfunction protected against this complication during the early postoperative period. Only on day 9 postoperatively did the left ventricle recover and SAM develop, with significant obstruction of the LV outflow tract. Subsequently, mitral valve replacement was required. This case demonstrates the importance of the LV geometrical relationships that predispose to this complication.

Survival and prognostic factors in hypertrophic cardiomyopathy: a meta-analysis.

Hypertrophic cardiomyopathy (HCM) is a clinically and genetically heterogeneous disorder but data on survival rates are still conflicting and have not so far been quantitatively reviewed. The aim of this study is to conduct a meta-analysis of cohort studies to assess pooled survival rates and prognostic factors for survival in patients with HCM. Nineteen studies were included representing 12,146 HCM patients. The pooled 1-, 3-, 5- and 10-year survival rates were 98.0%, 94.3%, 82.2% and 75.0%, respectively. Among patients with HCM, age, NYHA functional class, family history of sudden death (FHS

Clinical Course and Predictors of Heart Failure in Asymptomatic Obstructive Hypertrophic Cardiomyopathy.

BACKGROUND: Left ventricular outflow tract obstruction is a major contributor to morbidity in hypertrophic cardiomyopathy and an established therapeutic target. However, the risk factors driving progression to symptomatic heart failure in asymptomatic patients with obstructive hypertrophic cardiomyopathy remain incompletely understood. METHODS: We retrospectively identified 341 consecutive asymptomatic patients with obstructive hypertrophic cardiomyopathy (New York Heart Association class I, left ventricular ejection fraction >50%, left ventricular outflow tract gradient ≥30 mm Hg) across 4 c

Left ventricle-aortic conduits in pediatric patients.

From August, 1974, to January, 1982, left ventricle-aortic porcine valved conduits were inserted in three patients less than 2 years old (Group 1) and in 10 patients between 2 and 14 years of age (Group 2) for relief of severe left ventricular outflow tract obstruction. The distal anastomosis was made to the ascending aorta in seven patients and to the supraceliac abdominal aorta in six patients. In six patients, the conduit was sutured directly to the left ventricle, and in seven a stented right-angle connector was employed. The left ventricle-aortic gradients were relieved in all cases (mean

[Resection of a cardiac rhabdomyoma of the right and left ventricular outflow tract].

Rhabdomyomas are benign cardiac tumors often occurring as multiple lesions. We describe an infant with multiple cardiac rhabdomyomas causing severe right-ventricular and left-ventricular outflow tract obstructions. The intracavitary parts of the rhabdomyomas were successfully resected when the patient was 5 weeks old. Echocardiography showed regression in size of the remaining intramural tumors. Since there is only a low growth potential in cardiac rhabdomyomas, surgical management is indicated only in patients with hemodynamic obstruction or significant arrhythmias. Surgery should be primaril

Echocardiographic Brockenbrough-Braunwald-Morrow sign.

The Brockenbrough-Braunwald-Morrow sign is the paradoxical decrease in pulse pressure during the post-extrasystole beat seen in patients with hypertrophic obstructive cardiomyopathy. We present a case of intermittent left ventricular outflow tract obstruction and secondary mitral regurgitation resulting from post-extrasystolic potentiation following a premature atrial beat, demonstrating using echocardiography the mechanism behind this sign.
