[Electron microscopic investigation of connective tissue in vestibular ganglion and nerve in morbus Menière].

Vestibular ganglia of 17 patients with Menière's disease, obtained by transtemporal or translabyrinthine neurectomy, were studied by electron microscopy. Three ganglia removed because of other disease and four ganglia of normal ears taken post mortem served as controls. The neuronal fibrous tissue of Menière cases showed without exception pathologic changes of various extent. The amount of collagen was increased, whereby different fiber diameters were observed as well as changes in the periodicity of cross banding. Signs of continuing collagen formation were found: active fibroblasts and an in

Pontocerebellar atrophy combined with vestibular-reticular degeneration.

A 41-year-old woman was hospitalized in 1939 with neurologic signs from which there was complete recovery over a two-year period. In 1965 she was readmitted because of increasing forgetfulness and inability to care for herself; there were no neurologic deficits. In June 1970 torticollis developed and persisted until her death 15 months later. No cerebellar signs were elicited, but increased muscle tone was prominent. The brain showed remarkable flattening of the inferior surface of the cerebellum; the pons was small. The medullary substance of the cerebellum and each branchium pontis appeared

[Reeducation of patients with vertigo].

The use of physical therapy in the treatment of vertigo is based on the plasticity and the restoration and adaptive qualities of the vestibular apparatus. Indications for physical therapy and the choice of techniques are based on the location of the lesions: exercises aimed at habituation in positional vertigo, with use of the rocking manoeuvre in the case of cupulolithiasis, and stimulation of spinal, labyrinth, visual or extraocular muscle proprioceptors in the case of unilateral lesions of the first neuron or in ototoxic destruction. These techniques give lasting effects and avoid prolonged

Investigations into the cause of canal paresis in Menière's disease.

Since the literature has a paucity of documented lesions of the vestibular hair cells and neurons in Menière's disease, the cause of canal paresis remains unexplained. A clinicopathological correlation was sought and demonstrated between ampullary distortion of lateral canal and reduced caloric response. The findings in this investigation make a strong case for ampullary distortion disrupting the tenuous but vital cupulary attachment resulting in canal paresis. This concept is attractive because it explains an important clinical event in Menière's disease on the basis of the only consistent hi

Comparison between caloric and video-head impulse tests in Ménière's disease and vestibular neuritis.

OBJECTIVE: To compare the diagnostic accuracies of air caloric testing with electronystagmography and the vHIT (video-head impulse test). DESIGN: Prospective, controlled study. STUDY SAMPLE: MD (Ménière's disease), 26; vestibular neuritis, 27; control, 56. RESULTS: In MD, CP (canal paresis) was pathological in 88.5%, the GA (gain asymmetry) on vHIT was pathological in 65.3%, and the gain was abnormal in only one patient. The GA and CP, were significantly higher in the MD group than in the control group, indicating hypofunction of the horizontal canals in MD, whereas a hyperfunction may also

Acute vestibular neuritis may provoke atrial fibrillation.

BACKGROUND: Exclusion of stroke is the focus of guidelines in the emergency department assessment of acute vertigo, especially with new-onset atrial fibrillation (AF). Early diagnosis of vestibular neuritis (VN) is also important but may be deferred awaiting brain magnetic resonance imaging (MRI) for exclusion of stroke. This may delay potentially beneficial corticosteroid therapy. AIMS: To highlight that VN can provoke acute AF. METHODS: In the course of a prospective study of acute vertigo in patients assessable within 24 h of admission, we encountered three patients with acute onset trans

A potential serological biomarker for inner ear pathologies: OTOLIN-1.

OBJECTIVE: The goal of the study was to determine whether the level of OTOLIN-1, a protein whose expression is highly restricted to the inner ear,is increased in the body fluids of patients with inner ear disorders in comparison to healthy subjects. METHODS: In the preliminary part of the study, OTOLIN-1 levels were measured in the serum, urine, and saliva of patients with an acute onset of Ménière´s disease and in healthy individuals. Subsequently, only serum OTOLIN-1 levels were taken into account and were compared between patients with acute onset of Ménière´s disease, sudden hearing loss,

Horizontal nystagmus is gravity-dependent in patients with vestibular neuritis.

OBJECTIVE: Horizontal nystagmus can be observed in the acute stage of vestibular neuritis, Although the direction of the nystagmus is gravity independent, its intensity can be influenced by gravity. In this study, we compared the slow phase velocity (SPV) of horizontal nystagmus in different head positions in patients with vestibular neuritis to analyze the static effects of gravity on horizontal nystagmus. METHODS: The study enrolled 22 vestibular neuritis patients with spontaneous horizontal nystagmus (9 men, 13 women; median age 40 years). The deficits were right-sided in 9 patients and le

[Acute vestibular syndrome in emergency departments : Clinical differentiation of peripheral and central vestibulopathy].

INTRODUCTION: The differentiation between central and peripheral vestibular disorders is difficult in some cases, especially during the clinical routine of an emergency department (ED) without otoneurological diagnostic equipment. This study evaluated the frequency of vestibular pseudoneuritis as distinguished from acute peripheral vestibular disorders in patients who were admitted to hospital with the suspicion of vestibular neuropathy (VN). METHODS: This retrospective study analyzed the results of anamnestic and clinical examinations of 315 patients admitted to the emergency department and

Acute evaluation of the acute vestibular syndrome: differentiating posterior circulation stroke from acute peripheral vestibulopathies.

This review article aims to provide an evidence-based approach to evaluating the patient who presents with acute prolonged, spontaneous vertigo in the context of the acute vestibular syndrome (AVS). Differentiation of posterior circulation stroke (PCS) presenting as an AVS has been regarded as an important diagnostic challenge for physicians involved in acute care. Current evidence suggests that a targeted approach to history taking and physical examination with emphasis on the oculomotor examination, more specifically the HINTS (Head Impulse/Nystagmus/Test-of-skew) examination battery, yields

Alexander's law during high-acceleration head rotations in humans.

Alexander's law states that the amplitude of the spontaneous nystagmus grows with increasing gaze in the direction of the fast phase. Using the search-coil method we employed head impulses at various eye-in-orbit azimuth angles to test (i) whether the normal vestibulo-ocular reflex (VOR) in the behaviorally relevant high-frequency range has intrinsic properties that could account for Alexander's law and (ii) whether such properties can also be shown in patients with unilateral vestibulopathy. We showed that the gain of the VOR remained unaffected by eye-in-orbit position in contols and in pati

[Contribution of the vestibular evoked myogenic potentials to the study of the vestibular neuritis].

INTRODUCTION: Until recently, the only tests available to provide information about vestibular function were caloric and kinetic tests, which only give us information about the external semicircular canal and the superior vestibular nerve. In recent years the development of vestibular evoked myogenic potentials has allowed us to assess the saccule and the inferior vestibular nerve. Our aim is, by studying the caloric test results as well as the vestibular evoked myogenic potentials in patients with Vestibular Neuritis, to determine whether they have involvement of the superior, inferior or bot

Characterization of neuronal populations in the human trigeminal ganglion and their association with latent herpes simplex virus-1 infection.

Following primary infection Herpes simplex virus-1 (HSV-1) establishes lifelong latency in the neurons of human sensory ganglia. Upon reactivation HSV-1 can cause neurological diseases such as facial palsy, vestibular neuritis or encephalitis. Certain populations of sensory neurons have been shown to be more susceptible to latent infection in the animal model, but this has not been addressed in human tissue. In the present study, trigeminal ganglion (TG) neurons expressing six neuronal marker proteins were characterized, based on staining with antibodies against the GDNF family ligand receptor

Pattern analysis of peripheral-vestibular deficits with machine learning using hierarchical clustering.

Background Disorders affecting the vestibular organs (semicircular canals, utriculus, sacculus), may result in distinct patterns of peripheral-vestibular loss that may facilitate the diagnostic assessment. When neuropathological tests of these sensors are available, it is possible to classify responses as being due to different deficit types. Objective To provide a topical review and to summarize recent advances in pattern-recognition of unilateral and bilateral vestibular disease by use of hierarchical cluster analysis (HCA) as published by the authors. Hypothesis We propose that certain patt
