High-Impact Laryngotracheal Trauma: A Combined Narrative and Systematic Review Evaluating Gaps in the Current Laryngeal Injury Reporting System.

UNLABELLED: <p>Introduction: External neck trauma represents a potentially life-threatening clinical scenario. Blunt and penetrating injuries are the primary etiologies and may result in laryngeal injuries with concurrent tracheal involvement. High-impact or high-energy laryngotracheal trauma arises from diverse mechanisms; therefore, injury reporting must account for varied patterns and incorporate any synchronous tracheal injury, thereby establishing a more effective framework for managing complex external airway trauma. METHODS: Data from 7 patients experiencing high-intensity extreme lary

Pathology of acute sub-lethal or near-lethal irradiation of nonhuman primates prophylaxed with the nutraceutical, gamma tocotrienol.

Exposure to high, marginally lethal doses or higher of ionizing radiation, either intentional or accidental, results in injury to various organs. Currently, there is only a limited number of safe and effective radiation countermeasures approved by US Food and Drug Administration for such injuries. These approved agents are effective for only the hematopoietic component of the acute radiation syndrome and must be administered only after the exposure event: currently, there is no FDA-approved agent that can be used prophylactically. The nutraceutical, gamma-tocotrienol (GT3) has been found to be

The use of rigid eye shields (Fox shields) at the point of injury for ocular trauma in Afghanistan.

BACKGROUND: Unlike hemorrhagic injuries in which direct pressure is indicated, any pressure placed on the eye after penetrating trauma can significantly worsen the injury by expulsing intraocular contents. The accepted first response measure for obvious or suspected penetrating ocular injury is placement of a rigid shield that vaults the eye so as to prevent accidental iatrogenic aggravation during transport to the ophthalmologist. Patching and placing intervening gauze between the shield and the eye are both contraindicated. Anecdotally, compliance with these recommendations is poor in the mi

Postmortem Otorrhagia in Positional Asphyxia.

Otorrhagia is generally associated with basilar skull fractures or diving accidents. In routine forensic medical practice, an accurate knowledge of the etiology of otorrhagia may have a key role to distinguish between traumatic and atraumatic causes and accidental, homicidal, or suicidal manners. The authors present the case of an unusual cause of atraumatic otorrhagia occurred in an elderly farmer found dead in a barn. He remained entrapped in a narrow tunnel created by some hay bales. The autopsy findings revealed only an intense polyvisceral congestion and subpleural petechiae, with no sign

Predictors of Surgical Versus Medical Management of Eyes in the Setting of Orbital Fracture at an Academic Level I Trauma Center.

We present clinical and imaging predictors of ocular injuries that required medical management versus surgical intervention in cases of orbital fractures. From 2014 to 2020, a retrospective review of patients with orbital fractures who received ophthalmologic consultation and computed scan (CT) analysis at a level I trauma center was performed. Inclusion criteria were patients with confirmed orbital fracture on CT and ophthalmology consultation. Patient demographics, associated injuries, comorbidities, management, and outcomes were collected. Two hundred and one patients and 224 eyes (11.4% bi

Concurrent flexor carpi radialis tendon rupture and closed distal radius fracture.

Tendon rupture as a complication of distal radius fractures has been documented; however, flexor tendon rupture associated with closed distal radius fractures is rare. We report a case of a 43-year-old man who suffered a closed distal radius fracture. Intraoperatively, it was discovered that the flexor carpi radialis tendon had ruptured. From the frayed ends of the tendon and review of the radiographs, it was determined that the sharp ends of the fractured radius had lacerated the tendon at the time of injury. After fixation of the fracture with locking plate, the severed tendon was repaired a

Forgotten Kirschner wire passing across the sigmoid colon.

The Kirschner wire (K-wire) is used in the treatment of hip fractures, and migration of a K-wire into the pelvis with resultant colon injury is a very rare complication. We report a case in which a forgotten K-wire passing across the sigmoid colon was accidentally found by screening colonoscopy in a patient with no abdominal symptoms. Surgery was performed to remove the K-wire. Although the migration of a K-wire is extremely rare, it is important for it to be considered in patients with a history of orthopedic surgery.

Accidental puncture of the right lymphatic duct during pulmonary artery catheterization. A case report.

A case of an accidental placement of the introducer sheath of a Swan Ganz catheter into the right lymphatic duct during right internal jugular vein catheterization is presented. This rare complication has to be reported, because the choice of the right internal jugular vein for central venous catheterization has been strongly recommended, with special concern being paid to the avoidance of any thoracic duct injury. No deleterious complications were observed either in the immediate or in the later postanaesthetic period.

[Laparotomy indications in pelvic fractures].

Infra-umbilical abdominal section concomitant with peritoneal lavage was applied to 74 patients with pelvic fracture and has proved to be helpful in early diagnosis of intra-abdominal organ injuries. However, the percentage of falsely positive results was found to be as high as 17.3 per cent, in the context of pelvic fractures. This has been attributable to the fact that the pelvic floor is likely to be elevated by the haematoma caused by a pelvic fracture, and the syringe may thus get accidentally into the haematoma.

An Unusual Transorbital Penetrating Injury by House-key (lock): A Case Report with a Small Review of Literature.

Penetrating injuries of the brain are quite uncommon, comprising approximately 0.4% of all head injuries. In our case, a four-year-old boy who fell forward on a house-key (lock) accidentally while playing with some other children sustained a left sided penetrating transorbital brain injury. After hospital admission, the patient had a Glasgow Coma Scale (GCS) score of 15/15, no visual loss but restriction of upward gaze (left eye) and profuse bleeding from the wound site. Firstly, the metallic key was removed in emergency operation theatre and haemostasis secured. Next day we did a combined sur
