A Case of Refractory Vernal Keratoconjunctivitis Showing Improvement after the Administration of Upadacitinib for the Treatment of Atopic Dermatitis.

Vernal keratoconjunctivitis is a persistent allergic ocular disease predominantly mediated by the T-helper 2 lymphocyte-associated immune response. The standard therapeutic approaches for vernal keratoconjunctivitis include topical corticosteroids and immunosuppressive eye drops. However, managing vernal keratoconjunctivitis with only topical treatments becomes challenging during seasonally exacerbated periods. Systemic treatments such as oral corticosteroids or cyclosporine may be alternative options. Recently, dupilumab's efficacy in refractory vernal keratoconjunctivitis treatment has been

Electroretinography, Ocular Ultrasonography, and Phacoemulsification of Bilateral Cataracts in Two Juvenile Loggerhead Sea Turtles (Caretta caretta) of the Mediterranean Region.

Bilateral cataracts were diagnosed in two rescued juvenile, immature loggerhead sea turtles (Caretta caretta), weighing 1.65 and 1.7 kg. Both animals showed vision impairment and difficulty in feeding without assistance. In fact, they did not notice the presence of the food in the tank unless it was brought close to touching the mouth. Ocular ultrasonography and electroretinography showed no lesions of the vitreal body and retinal layer, therefore, both animals were candidates for bilateral cataract surgery. Topical administration of tropicamide + phenylephrine alternating with rocuronium resu

Postoperative Hematoma-induced Vasospasm after Sarcoma Reconstruction Using a Pedicled Anterolateral Thigh Flap.

Hematoma-induced vasospasm is a significant factor that can compromise the success of flap reconstructive surgery. Despite advances in microsurgical techniques and knowledge, vasospasm remains a direct cause of flap loss. Hematoma-induced vasospasm occurs due to the presence of blood breakdown products, which can lead to arterial constriction and reduced blood flow to the transplanted tissue. A 77-year-old man with a history of coronary angina developed soft tissue sarcoma on the right groin. Postoperative hematoma-induced vasospasm occurred subsequent to the reconstruction using a pedicled an

Mucoadhesive brinzolamide-loaded nanofibers for alternative glaucoma treatment.

Despite the advances in the field of pharmaceutical materials and technology, topical administration remains a method of choice for the treatment of eye diseases such as glaucoma, with eye drops being a leading dosage form. Their main disadvantage is a very short drug residence time and thus poor drug bioavailability, leading to the necessity of continuous repeated dosing. Mucoadhesive electrospun nanofibers are promising candidates for overcoming these challenges, while still benefiting from topical ocular administration. As an alternative for eye drops, a nanofibrous drug delivery system (DD

Colloidal nanosystems with mucoadhesive properties designed for ocular topical delivery.

Over the last years, the scientific interest about topical ocular delivery targeting the posterior segment of the eye has been increasing. This is probably due to the fact that this is a non-invasive administration route, well tolerated by patients and with fewer local and systemic side effects. However, it is a challenging task due to the external ocular barriers, tear film clearance, blood flow in the conjunctiva and choriocapillaris and due to the blood-retinal barriers, amongst other features. An enhanced intraocular bioavailability of drugs can be achieved by either improving corneal perm

Biopharmaceutical Assessment of Dexamethasone Acetate-Based Hydrogels Combining Hydroxypropyl Cyclodextrins and Polysaccharides for Ocular Delivery.

We previously developed two optimized formulations of dexamethasone acetate (DXMa) hydrogels by means of special cubic mixture designs for topical ocular administration. These gels were elaborated with hydroxypropyl-β-CD (HPβCD) and hydroxypropyl-γ-CD (HPγCD) and commercial hydrogels in order to enhance DXMa water solubility and finally DXMa's ocular bioavailability and transcorneal penetration. The main objective of this study was to characterize them and to evaluate in vitro, ex vivo, and in vivo their safety, biopermanence, and transcorneal permeation. Gels A and B are Newtonian fluids and

Design and development of artemisinin and dexamethasone loaded topical nanodispersion for the effective treatment of age-related macular degeneration.

Age-related macular degeneration (AMD) is a disease affecting the macula by the new blood vessels formation. AMD is widely treated with a combination of anti-angiogenic and anti-vascular endothelial growth factor (VEGF) agents. The topical administration of nanodispersions showed enhanced ocular residence time with controlled and prolonged drug delivery to the disease site at the back of the eye. In the present study we developed and characterized nanodispersion containing anti-angiogenic (artemisinin) and anti-VEGF agent (dexamethasone) for the topical ocular administration in order to obtain

Systemic Platelet-Activating Factor-Receptor Agonism Enhances Non-Melanoma Skin Cancer Growth.

Platelet-activating factor-receptor (PAF-R) agonists are pleiotropic lipid factors that influence multiple biological processes, including the induction and resolution of inflammation as well as immunosuppression. PAF-R agonists have been shown to modulate tumorigenesis and/or tumor growth in various skin cancer models by suppressing either cutaneous inflammation and/or anti-tumoral adaptive immunity. We have previously shown that a chronic systemic PAF-R agonist administration of mice enhances the growth of subcutaneously implanted melanoma tumors. Conversely, chronic topical applications of

Ocular hypotensive effect of topical ketanserin in timolol users.

BACKGROUND: Ketanserin is a specific antagonist of 5-HT2 and 5-HT1c receptors. These receptors are linked to the stimulation of phosphoinositide metabolism and are involved in IOP controls. Orally and topically administered ketanserin reduces IOP in normotensive and glaucomatous eyes. METHODS: Ketanserin 0.5% eye drops were administered to 20 patients with primary open-angle glaucoma in a randomised, crossover, double-masked fashion to evaluate the effect of ketanserin in glaucomatous patients already receiving timolol who did not have controlled IOP (> 21 mmHg). Mean and range of IOP curve (

Modulation of ultraviolet light-induced epidermal damage: beneficial effects of tocopherol.

Oxygen free radicals have been shown to result from and mediate deleterious effects of ultraviolet radiation on the skin. The purpose of this study was to determine if topical DL-alpha-tocopherol (vitamin E) could reduce ultraviolet-induced damage to the epidermis. Twenty mice were treated with either ethanol or a 1:1 mixture of tocopherol and ethanol. Treatments consisted of once-daily 0.1-ml topical applications for 1 week, followed by irradiation with 0.30 mW/cm2 of ultraviolet B irradiation. A statistically significant decrease in Schiff base formation was noted between tocopherol-treated

A magnetic resonance imaging evaluation of histamine-mediated allergic response in the guinea pig nasopharynx.

BACKGROUND: Magnetic resonance imaging (MRI) is a powerful technique for visualizing tissues with a high water content. In this study we used MRI to evaluate the effect of antigen and mediators of allergic responses on the nasopharyngeal airway of guinea pigs. METHODS AND RESULTS: Longitudinal relaxation time-weighted transverse images of the nasopharyngeal airway revealed a clearly defined airway lumen and mucosa. Topical administration of ovalbumin (0.00006% to 0.06%) to the nasopharyngeal airway of sensitized guinea pigs caused a concentration-dependent reduction (34% +/- 1%, maximum) in a

Sedation and topical anesthetics in audiology and speech-language pathology. Ad Hoc Committee on Advances in Clinical Practice. American Speech-Language-Hearing Association.

Audiologists and speech-language pathologists who participate in or perform procedures on patients who have been medicated for sedation or topical anesthesia should appreciate the complex factors which may expose their patients to risk or harm. Administration of medications to achieve a desired patient state is a medical procedure requiring physician or dentist prescription, physician or dentist approval on the conditions of administration and monitoring, and physician or dentist availability for provision of emergency care that may be required. For these reasons, audiologists and speech-langu

Corticosteroid pharmacokinetics in the inner ear fluids: an animal study followed by clinical application.

OBJECTIVE: Autoimmune disease (e.g., Cogan syndrome) and other inflammatory inner ear diseases may ravage the labyrinth if not treated aggressively with antiinflammatory medication. Corticosteroids are the mainstay of treatment, yet, partly because of the existence of the blood-labyrinthine barrier, the ideal drug, dose, and route of administration are currently unknown. STUDY DESIGN: In the present study, we established cochlear fluid pharmacokinetic profiles of hydrocortisone, methylprednisolone, and dexamethasone in the guinea pig following oral, intravenous, and topical (intratympanic) ad

AI-guided multi-objective optimization identifies a Jelleine I-derived gram-negative antimicrobial peptide lead.

Antimicrobial peptides are promising alternatives to conventional antibiotics, but their translational development is often limited by the difficulty of simultaneously optimizing antibacterial potency, mammalian-cell compatibility, and formulation-relevant properties. Here, using Jelleine-I (J1) as a representative scaffold, we developed a multi-task learning and reinforcement learning-based framework for the multi-objective optimization of antimicrobial peptides across antibacterial activity, hemolytic toxicity, and self-assembly propensity. Experimental validation of AI-designed analogues id

Virus-biomimetic melanin nanoparticles with spiky structures and neutral zwitterionic shell for non-invasive prevention of dry AMD.

Clinically, no effective preventive strategies currently exist to inhibit the progression of early dry age-related macular degeneration (dAMD). Moreover, current retinal therapies rely on intravitreal injections, which carry inherent procedural risks. Inspired by natural melanin and viral structures, we developed virus-biomimetic melanin nanoparticles (VMNPs) surface-modified with ergothioneine (ET), featuring virus-like spike structures and a neutral zwitterionic shell. This study aimed to evaluate whether virus-inspired engineered nanoparticles could enhance retinal delivery and improve the

Baricitinib-loaded separable microneedle array patch (S-MAP) based on hyaluronic acid for alopecia areata therapy.

Separable and dissolvable microneedle array patch based primarily on hyaluronic acid is designed for the topical delivery of baricitinib in alopecia areata (AA) therapy. Locoregional application of a separable microneedle array patch (S-MAP) may avoid the systemic side effects of oral administration and can be administered without pain compared with typical intradermal injections. Baricitinib (which has poor aqueous solubility) is evenly suspended with sodium carboxymethylcellulose. Subsequently, the mixture is blended with other pharmaceutical excipients, such as hyaluronic acid, sucrose, and
