Myelolipoma within an adrenal cortical adenoma is a very rare cause of adrenal incidentaloma, and only nine cases have been reported in the English and Japanese literature. We report a 66-year-old Chinese man, with a history of hypertension and hyperlipidaemia, who presented with lower limb oedema and had a computed tomography (CT ) of the abdomen done to exclude intra-abdominal mass. His lower limb symptoms resolved after switching his antihypertensive medication. Oestrogen producing adrenocortical tumours are extremely rare. We report a 65-year-old woman who presented with abnormal vaginal bleeding, with no significant abnormalities in her uterus or ovaries, who was found to have a right adrenal mass by radiological examination. We report a rare case of primary hyperaldosteronism in an 11-year-old female patient due to a unilateral adrenal cortical adenoma. The patient presented with hypertension, hypokalaemia and metabolic alkalosis associated with inappropriate kaliuresis. Thecal metaplasia of the adrenal was observed as an incidental autopsy finding in a 77-year-old man with acquired bilateral testicular atrophy, probably secondary to previous bilateral inguinal herniorrhaphy. Primary aldosteronism, when caused by an aldosterone-producing adenoma, is a surgically treatable condition. An operation was performed on a 44-year-old patient with this condition. The pre-operative preparation and anaesthetic management are presented.
