Role of cerebrospinal fluid [H+] in ventilatory deacclimatization from chronic hypoxia.

Once ventilatory acclimatization begins in sea level residents sojourning at high altitude, abrupt restoration of normal oxygen tensions will not restore ventilation to normal. We have investigated the role of cerebrospinal fluid (CSF) [H(+)] in this sustained hyperventilation by measuring CSF acid-base status in seven men (lumbar) and five ponies (cisternal) in normoxia, first at sea level and then periodically over 13-24 h of "deacclimatization" after 3-5 d in hypoxia (P(B) = 440 mm Hg). After 1 h deacclimatization, hyperventilation continued at a level only slightly less than that obtained

Effect of hypoxaemia on water and sodium homeostatic hormones and renal function.

Changes in body fluid homeostasis during acute hypoxaemia suggest a crucial role of renal function in acclimatization processes. Hypoxaemia stimulates sympathetic nervous activity, and also the cardiovascular system is affected with increases in heart rate and cardiac output. In most subjects, a hypoxic ventilatory response produces hypocapnia and respiratory alkalosis. Acute hypoxaemia depresses aldosterone secretion secondary to a direct effect on adrenal cells. Also plasma renin is decreased in resting hypoxaemic conditions, but the mechanism remains unknown. These hormonal changes may have

Enhanced bioavailability of anemoside B4 by dry powder inhalation mitigates high-altitude acute lung injury.

BACKGROUND: Anemoside B4 (AB4), a pentacyclic triterpenoid saponin, exhibits potent anti-inflammatory and antioxidant activities; however, it has poor oral bioavailability. Hypobaric hypoxia (HH) can induce high-altitude acute lung injury (ALI) through inflammatory response and oxidative stress pathways, highlighting the need for effective targeted therapies. PURPOSE: This study aimed to develop an AB4 dry powder inhaler (DPI) for improving lung deposition and bioavailability, and to evaluate its prophylactic efficacy against high-altitude ALI. METHODS: AB4 inhalable microparticles were prep

Cistanche tubulosa (Schrenk) Wight Extract Ameliorates Learning and Spatial Memory Abilities in High-Altitude Hypobaric Hypoxia Rats.

Background: High-altitude hypobaric hypoxia (HH) is a major environmental stressor that impairs cognitive function, yet effective and widely available therapeutics remain limited. Although Rhodiola rosea has shown neuroprotective effects, its resource scarcity restricts large-scale application. Cistanche tubulosa (Schrenk) Wight, a traditional Chinese functional food, has been increasingly used in health supplements due to its anti-fatigue, anti-dementia, and memory-enhancing properties, suggesting potential benefits against hypoxia-induced cognitive impairment. Objective: This study aimed to

Modulation of HIF1α/CXCR4 by CDDP, acetazolamide, and salidroside in early lung injury caused by acute high-altitude hypoxia.

BACKGROUND: The inflammatory response of pulmonary vascular endothelial cells is critical in the pathogenesis of hypobaric hypoxia-induced acute lung injury. Elucidation of the underlying mechanisms and potential therapeutic interventions is crucial for both medical professionals and individuals exposed to high altitudes. METHODS: A murine model of acute lung injury was established by subjecting mice to hypobaric hypoxia at an altitude equivalent to 6000 m for 8 and 72 h. Histopathological and immunohistochemical analyses were performed using hematoxylin-eosin staining and immunohistochemistr

Physiological and genomic evidence that selection on the transcription factor Epas1 has altered cardiovascular function in high-altitude deer mice.

Evolutionary adaptation to extreme environments often requires coordinated changes in multiple intersecting physiological pathways, but how such multi-trait adaptation occurs remains unresolved. Transcription factors, which regulate the expression of many genes and can simultaneously alter multiple phenotypes, may be common targets of selection if the benefits of induced changes outweigh the costs of negative pleiotropic effects. We combined complimentary population genetic analyses and physiological experiments in North American deer mice (Peromyscus maniculatus) to examine links between gene

Parental smoking. Its effects on fetus and child health.

Smoking is a risk factor in pregnancy. Maternal smoking reduces the birthweight by 150-250 g (dose-response relationship), shifting the birthweight (and length) distribution to the left. Reduction of birthweight without shortening of gestation creates a group of small-for-date babies. Intrauterine hypoxia seems responsible for the growth retardation in smokers' babies as in babies of mothers living at high altitudes. Cigarette smoking of the mother affects fetal oxygenation, due to high levels of COHb in the blood of both mother and child. This observation is strongly supported by animal exper

Restructuring Riley's Historic 3-Compartment Lung Model for Evaluation of Pulmonary Gas Exchange.

In 1951, Riley's classic 3-compartment model of gas exchange estimated pulmonary shunt, alveolar dead space, and an effective compartment representing the functioning lung. But trial-and-error steps and conversion charts made its application impractical. We implemented estimates of alveolar and effective PCO2 to simplify computations, making it useful when more advanced technologies are unavailable. Using stepwise computations, we studied 10 healthy individuals and 43 outpatients with mild to severe chronic obstructive pulmonary disease and, in another study, 32 healthy subjects during 12 h of

Painless acute myocardial infarction on Mount Kilimanjaro.

An individual experiencing dyspnoea or syncope at high altitude is commonly diagnosed to have high-altitude pulmonary edema or cerebral edema. Acute myocardial infarction (AMI) is generally not considered in the differential diagnosis. There have been very rare cases of AMI reported only from Mount Everest. We report a case of painless ST segment elevation myocardial infarction (STEMI) that occurred while climbing Mount Kilimanjaro. A 51-year-old man suffered dyspnoea and loss of consciousness near the mountain peak, at about 5600 m. At a nearby hospital, he was treated as a case of high-altit

High-altitude cerebral oedema mimicking stroke.

High-altitude cerebral oedema (HACO) is the most fatal high-altitude illness seen by rural physicians practising in high-altitude areas. HACO presents clinically with cerebellar ataxia, features of raised intracranial pressure (ICP) and coma. Early identification is important as delay in diagnosis can be fatal. We present two cases of HACO presenting with focal deficits mimicking stroke. The first patient presented with left-sided hemiplegia associated with the rapid deterioration in the sensorium. Neuroimaging revealed features suggestive of vasogenic oedema. The second patient presented with

Oxidative stress in rat testis and epididymis under intermittent hypobaric hypoxia: protective role of ascorbate supplementation.

Hypobaric hypoxia (HH), an environmental condition of high altitude encountered by mountaineers, miners, and observatory, rural health, border patrol, and rural education workers, jeopardizes normal physiologic functions in humans. The present study was conducted to evaluate the effects of intermittent HH (IHH; equivalent to 4600 m above mean sea level) on oxidative stress and the protective role of dietary ascorbic acid on rat testis and epididymis. Ten-week-old male Wistar rats were assigned to 1 of 6 groups: 1) normobaric (Nx), 2) Nx + physiologic solution (Nx + PS), 3) Nx + ascorbic acid (

Decompression sickness in women: a possible relationship with the menstrual cycle.

BACKGROUND: Women are increasingly participating in recreational scuba diving and the professional roles of women are expanding in the fields of aviation, space, and diving. Evidence exists that there may be a relationship between altitude decompression sickness (DCS) and the menstrual cycle, although diving studies to support such findings are limited. The aim of the present study was to investigate the presence of any relationship between the development of DCS in female sports divers, the phase of the menstrual cycle, and the use of the oral contraceptive pill (OCP). METHOD: Personal, dive

High-altitude hypoxia drives dentate gyrus neuronal vulnerability through an IL1α-astrocyte-SLC1A2 pathway.

BACKGROUND: High-altitude hypoxia is known to impair cognition, yet the underlying cellular and molecular mechanisms remain insufficiently understood. The hippocampus, especially the dentate gyrus (DG), is highly sensitive to hypoxic stress, but the pathways driving neuronal vulnerability and glial state transitions remain unclear. METHODS: We combined human cohort cognitive assessments, hypobaric chamber mouse models, and multi-omics profiling, including single-cell and spatial transcriptomics, spatial metabolomics, and cell–cell communication analyses, to construct an integrated map of hippo

Th1/Th2 Immune Imbalance in the Spleen of Mice Induced by Hypobaric Hypoxia Stimulation and Therapeutic Intervention of Astragaloside IV.

This study aims to establish a hypobaric hypoxia-induced immune injury model and investigate the intervention and therapeutic effects of Astragaloside IV (AS-IV). This study simulated hypobaric hypoxia stimulation in mice at an altitude of 7000 m on a plateau for 1, 3, 5, and 7 days. HE staining and transcriptomic analysis were performed on mouse spleens. In addition, AS-IV was selected for intervention in prevention and treatment, and validated by flow cytometry, ELISA, and Q-PCR. The results showed that under simulated hypoxic conditions at an altitude of 7000 m for 5 days, the peripheral bl

Exposure to 16 h of normobaric hypoxia induces ionic edema in the healthy brain.

Following prolonged exposure to hypoxic conditions, for example, due to ascent to high altitude, stroke, or traumatic brain injury, cerebral edema can develop. The exact nature and genesis of hypoxia-induced edema in healthy individuals remain unresolved. We examined the effects of prolonged, normobaric hypoxia, induced by 16 h of exposure to simulated high altitude, on healthy brains using proton, dynamic contrast enhanced, and sodium MRI. This dual approach allowed us to directly measure key factors in the development of hypoxia-induced brain edema: (1) Sodium signals as a surrogate of the d

Ibuprofen versus acetazolamide for prevention of acute mountain sickness.

INTRODUCTION: Acute mountain sickness is a common condition occurring in healthy subjects that undergo rapid ascent without prior acclimatization, as low as 2500 meters above sea level. The classic preventive agent has been acetazolamide, although in the last decade there has been evidence favoring ibuprofen. However, it is unclear which method is more efficient. METHODS: We searched in Epistemonikos, the largest database of systematic reviews in health, which is maintained by screening multiple information sources, including MEDLINE, EMBASE, Cochrane, among others. We extracted data from the
