Prolonged duration local anesthesia with minimal toxicity.

Injectable local anesthetics that would last for many days could have a marked impact on periprocedural care and pain management. Formulations have often been limited in duration of action, or by systemic toxicity, local tissue toxicity from local anesthetics, and inflammation. To address those issues, we developed liposomal formulations of saxitoxin (STX), a compound with ultrapotent local anesthetic properties but little or no cytotoxicity. In vitro, the release of bupivacaine and STX from liposomes depended on the lipid composition and on whether dexamethasone was incorporated. In cell cult

Inhibition of chondrocyte and synovial cell death after exposure to commonly used anesthetics: chondrocyte apoptosis after anesthetics.

BACKGROUND: An intra-articular injection of local anesthetics is a common procedure for diagnostic and therapeutic purposes. It has been shown that these agents are toxic to articular cartilage and synovial tissue in a dose- and time-dependent fashion, and in some cases, they may lead to postarthroscopic glenohumeral chondrolysis (PAGCL). However, the role of apoptosis in cell death is still unclear, and the potential role of apoptosis inhibition in minimizing chondrocyte and synovial cell death has not been reported. PURPOSE: (1) To quantify the degree of apoptotic cell death in chondrocytes

Ropivacaine inhibits the migration of esophageal cancer cells via sodium-channel-independent but prenylation-dependent inhibition of Rac1/JNK/paxillin/FAK.

The direct anti-proliferative and pro-apoptotic effects of local anesthetics have been well documented in various cancers. However, whether local anesthetics affect cancer metastasis and their underlying molecular mechanisms are not well understood. In this work, we show that ropivacaine at the clinically relevant concentration significantly inhibits esophageal cancer cell migration. Interestingly, ropivacaine at the same concentration does not display inhibitory effects on esophageal cancer cell growth and survival. We further demonstrate that ropivacaine significantly decreases activities of

A retrospective study of paresthesia of the dental alveolar nerves.

Paresthesia is a rare clinical finding subsequent to surgery accompanied by the administration of local anesthetics. A small patient population was identified whose clinical problem may be explained by neurotoxicity due to a local anesthetic metabolite. Reasonable questions arise from these clinical observations that would benefit from prospective studies to explain sensory loss on a biochemical basis.

Intravenous Regional Anesthesia: A Historical Overview and Clinical Review.

Intravenous regional anesthesia (IVRA) is an established, safe and simple technique, being applicable for various surgeries on the upper and lower limbs. In 1908, IVRA was first described by the Berlin surgeon August Bier, hence the name "Bier's Block". Although his technique was effective, it was cumbersome and fell into disuse when neuroaxial and percutaneous plexus blockades gained widespread popularity in the early 20th century. In the 1960s, it became widespread, when the New Zealand anesthesiologist Charles McKinnon Holmes praised its use by means of new available local anesthetics. Toda

Analgesic effect of perineural magnesium sulphate for sciatic nerve block for diabetic toe amputation: A randomized trial.

BACKGROUND AND OBJECTIVES: High concentrations of local anesthetics may be neurotoxic for diabetic patients. Additive perineural administration of magnesium was reported to decrease the consumption of local anesthetics for nerve block. It was hypothesized that MgSO4 added to dilute ropivacaine was equianalgesic to more concentrated ropivacaine for toe amputations in diabetic patients. METHODS: Seventy diabetic patients were allocated into 3 groups: 1) perineural 200 mg MgSO4 added to 0.25% ropivacaine, 2) 0.25% ropivacaine alone, and 3) 0.375% ropivacaine alone. All patients underwent poplite

The Role of Topical Anesthetics in the Treatment of Pediatric Facial Lacerations.

PURPOSE: Pediatric facial lacerations (PLACS) are a common emergency, often requiring a general anesthetic (GA) or sedation if infiltration of local anesthetic (LA) cannot be tolerated. Topical anesthetics are a well-established and accepted alternative to infiltrations for facial lacerations which can reduce the need for general anesthesia. We report our experience following the introduction of topical 5% cocaine w/v and 1:2000 adrenaline gel (TCA) in a single center. METHODS: A secondary data analysis of 11 months attendances with PLACS following the introduction of TCA as part of a service

Pruritus ani. What to do, what not to do to control this infernal itch.

The essentials of the treatment of pruritus ani consist of keeping the perianal skin scrupulously clean and dry. The most important damaging factors are fecal residue, moisture, scratching, steroids, and local anesthetics.

Antinociceptive effects of topical mepivacaine in a rat model of HIV-associated peripheral neuropathic pain.

BACKGROUND: A consequence of HIV infection is sensory neuropathy, a debilitating condition that degrades the quality of life of HIV patients. Furthermore, life-extending antiretroviral treatment may exacerbate HIV sensory neuropathy. Analgesics that relieve other neuropathic pains show little or no efficacy in ameliorating HIV sensory neuropathy. Thus, there is a need for analgesics for people with this particular pain. While lidocaine is used in the management of painful peripheral neuropathies, another local anesthetic mepivacaine, with a potentially improved bioavailability, could be utiliz

Hypocapnia prolongs bradycardia induced by bupivacaine or levobupivacaine in isolated rat hearts.

PURPOSE: Systemic alkalinization is recommended for resuscitation from local anesthetic-induced cardiotoxicity. It has been suggested that inducing hypocapnic alkalosis, prior to exposure to toxic concentrations of local anesthetics, may minimize cardiotoxicity. However, it remains unclear whether inducing severe hypocapnic alkalosis after administration of local anesthetics will minimize the duration of bradycardia. We used isolated rat hearts to investigate the effects of hypocapnic alkalosis on heart rate (HR) recovery from bupivacaine or levobupivacaine-induced bradycardia. METHODS: We me

Non-Adherent Bone Marrow-Derived Mesenchymal Stem Cells Ameliorate Clinical Manifestations and Inflammation in an Experimental Model of Ulcerative Colitis in Rats.

BACKGROUND: The use of amide-based local anesthetics is generally considered to be safe. However, the literature on their safety in patients with hepatic injury is scarce. For the first time, the present study aimed to evaluate the effect and safety of five commonly used amide-based local anesthetics in the setting of hepatic failure. METHODS: A total of 96 Sprague-Dawley rats were studied from September 2015 to September 2016 in the Animal Laboratory Center, Shiraz University of Medical Sciences, Shiraz, Iran. They divided into three groups, namely a control, induced liver failure (LF), and

Sympathicotomy under local anesthesia: a simple way to treat primary hyperhidrosis.

General anesthesia has been the standard approach for sympathicotomy to treat primary hyperhidrosis. Sympathicotomy through thoracoscopic guidance is a straightforward procedure with a short operation time. Therefore we propose using local anesthesia for the procedure. The advantages of local anesthesia include avoiding endotracheal intubation and systemic anesthetics, and allowing immediate assessment by means of verbal communication with the patient. We effectively treated primary hyperhidrosis in 3 patients by thoracoscopic sympathicotomy under local anesthesia.

A rare case of facet joint synovial cyst connected to the intrathecal space: a case report.

Unexpected injection of local anesthetics into the intrathecal space can cause adverse effects, such as hypotension, shortness of breath, bradycardia, numbness or weakness, nausea and vomiting and serious complications, such as respiratory arrest, loss of consciousness and cardiac arrest. In this study, we report a case of a 68-year-old woman with a facet joint synovial cyst connected to the intrathecal space. The patient's clinical symptoms and radiological findings were consistent with those of a facet joint synovial cyst; however, during facet joint synovial cyst aspiration, it was found th

Bronchospasm and anaphylactic shock following lidocaine aerosol inhalation in a patient with butane inhalation lung injury.

Allergic reactions to local anesthetics are very rare and represent <1% of all adverse local anesthetics reactions. A 54-year-old man was admitted to the hospital in the winter because of shortness of breath. The patient reportedly had an inhalation lung injury due to butane gas fuel. On the fifth day, he developed an asthmatic attack and anaphylactic shock immediately after lidocaine aerosol administration to prepare for bronchoscopy to confirm an acute inhalational lung injury diagnosis. Cardiopulmonary resuscitation was performed immediately after respiratory arrest, and the patient was adm
