Hereditary angioedema (HAE) is characterized by nonpitting, nonpruritic subcutaneous, or submucosal edema that may be accompanied by a nonpruritic serpentine erythematous rash. The swelling in this autosomal dominantly inherited disorder is mediated by uncontrolled localized bradykinin production. The extremities, face, gastrointestinal tract, genitalia, larynx, and trunk are commonly involved. Nearly one-third of patients who are not treated may suffer asphyxiation from a laryngeal attack. HAE symptoms overlap with other conditions that cause swelling. An accurate diagnosis is therefore cruci…

Hereditary angioedema (HAE) results from a congenital deficiency of C1 inhibitor and is characterized by submucosal and subcutaneous edema of skin, larynx and abdomen. Occasional reports have appeared linking HAE with autoimmune diseases. We report a case of HAE presenting recurrent nondeforming polyarthritis, erythema marginatum-like rash and chest pain. There were no significant radiographic joint changes. Serologic tests for rheumatologic and autoimmune diseases were negative. After danazol treatment, physical examination and laboratory findings were normal over five years. We suggest that …

The first component of the classical pathway of the complement system, C1 is regulated by a serum protein, the C1-esterase inhibitor (C1-INH). Deficiency of this protein leads to the release of vasoactive mediators (C2 kinin and bradykinin) that increase vascular permeability and can induce edema formation in subcutaneous and submucosal tissues. The genetic variant of C1-INH deficiency is inherited as an autosomal dominant trait and causes hereditary angioneurotic edema. The acquired form of C1-INH deficiency is characterized by similar manifestations and can occur in association with lymphopr

We report the case of a 54-year-old woman with no history of atopic diseases or drug allergies who developed a generalized anaphylactic reaction with urticaria and angioedema a few hours after tooth extraction. The skin scratch and RAST tests demonstrated IgE-mediated hypersensitivity to the product Hémofibrine, a hemostatic sponge made of bovine fibrin. Of all RAST controls, only one serum with known positivity to bovine serum albumin (BSA) showed a low degree of positivity to Hémofibrine. Bovine fibrin should be added to the list of medical products that can elicit allergic reactions followi…

Attenuated androgens (AAs) historically have been used as long-term prophylaxis (LTP) for hereditary angioedema (HAE), and they continue to be used, especially in regions where access to newer therapies remains restricted. This systematic literature review assessed the adverse health outcomes associated with AAs, including adverse events (AEs), comorbidities, cardiovascular (CV) risk, as well as patient and physician perspectives. In total, 99 unique studies were identified through MEDLINE and EMBASE (searches up to March 2025), of which the majority were conducted in Europe (n = 57) and the U…

BACKGROUND: Hereditary angioedema (HAE) is a rare, potentially life-threatening disease caused in most cases by C1 inhibitor deficiency. Lanadelumab, a monoclonal antibody targeting plasma kallikrein, is an effective long-term prophylactic (LTP) treatment for HAE. However, consensus on best practices remains lacking.
OBJECTIVES: This study aimed to report consensus statements on key principles on long-term lanadelumab therapy for HAE in Germany developed at an HAE LTP Expert Meeting in the year 2024 in Frankfurt, Germany.
MATERIALS AND METHODS: A multidisciplinary panel of seven German HAE e…

We present a Mexican multigenerational family series of type I hereditary angioedema (HAE) with predominant abdominal involvement, including three affected adult sisters and a child identified through family screening. The index case was a 44-year-old woman with recurrent abdominal attacks, chronic diarrhea, multiple hospitalizations, and previous abdominopelvic interventions with no conclusive findings. During her clinical course, she developed non-urticarial lip edema, and abdominal CT showed segmental thickening of jejunal loops and mesenteric edema, findings consistent with bowel wall edem…

INTRODUCTION: Hereditary angioedema due to C1-inhibitor deficiency (HAE-C1INH) features clinical heterogeneity and stress-triggered attacks. Behavioral tolerance to acute stress may reveal vulnerability profiles beyond standard clinical descriptors. This study aimed to characterize stress response patterns and compare groups based on behavioral tolerance.
METHODS: HAE-C1INH patients underwent the Socially Evaluated Cold Pressor Test (SECPT) and were stratified as Completers or Non-completers (early withdrawal). Stress appraisal, cardiovascular parameters (heart rate, HR; systolic/diastolic ar…

BACKGROUND: Episodic angioedema with eosinophilia (Gleich syndrome) is a rare disease marked by periodic angioedema, fever, and severe eosinophilia, with limited understanding of its pathogenesis.
OBJECTIVE: We sought to identify pathogenic factors contributing to severe Gleich syndrome through a comprehensive multiomics approach, using whole-genome sequencing (WGS) and RNA sequencing (RNA-seq).
METHODS: A multiomics analysis was conducted on a 16- to 20-year-old female patient with severe Gleich syndrome, presenting with periodic high fever, extensive urticaria/eczema, and marked eosinophil…

Hereditary Angioedema (HAE) due to C1-esterase inhibitor (C1-INH) deficiency or dysfunction (C1-INH-HAE), is an autosomal dominant genetic disease characterized by recurrent cutaneous and submucosal swelling episodes most often caused by heterozygous pathogenic Single Nucleotide Variants (SNVs), small insertions or deletions (indels) or Copy Number Variants (CNVs) in the SERPING1 gene, coding for C1-INH. Rare subtypes of HAE not associated with SERPING1 pathogenic variants (nC1-INH-HAE) have been described. This case report aimed to characterize a 55-year-old proband, without a family history …

Angioedema (AE) is a potentially life-threatening condition involving swelling under the skin. The most common pharmacologic cause of this reaction is angiotensin-converting enzyme (ACE) inhibitor (ACEi) therapy, but other medications may contribute to the development of AE. This case describes a 68-year-old male who presented to the emergency department with tongue and lip swelling and was diagnosed with AE, initially attributed to lisinopril use. However, outpatient follow-up revealed that the only recent medication change was an inadvertent supratherapeutic 60 mg daily dose of simvastatin. …

BACKGROUND: Hereditary angioedema (HAE) is a rare, potentially life-threatening genetic disorder. No US prevalence for all types of HAE has been estimated. Approval of 8 effective HAE therapies in the United States significantly expanded treated patient numbers and data collected in claims databases.
OBJECTIVE: To provide the first US prevalence estimate of HAE (all types) using claims-based insurance data.
METHODS: This retrospective study estimated the US prevalence of HAE using IQVIA's PharMetrics Plus MedTech claims database. Patients with 24 months of continuous database enrollment were…

INTRODUCTION: Angiotensin converting enzyme (ACE) inhibitor-associated angioedema is the most common cause of angioedema seen in the emergency department (ED) and can be associated with a high morbidity. Most cases occur within months of initiation of an ACE inhibitor and are associated with facial and/or oropharyngeal swelling. We present a case of isolated laryngeal edema requiring intubation following 10 years of ACE inhibitor therapy.
CASE REPORT: An 82-year-old female, who was on lisinopril therapy for 10 years, presented to the ED with shortness of breath and a sensation that her throat…

BACKGROUND: Angioedema is the most common acute allergic presentation to emergency centres (EC), with hospitalisation rates increasing in high-income countries. Angioedema can complicate with life-threatening laryngeal obstruction. There are no local data; therefore, we aimed to characterise acute angioedema cases presenting to ECs and develop a simple management algorithm.
OBJECTIVE: To characterise the clinical presentation, management and outcomes of acute angioedema cases presenting to ECs. Based on these findings, we developed a management algorithm for acute angioedema to improve the ca…

BACKGROUND: The pathogenesis of hereditary angioedema (HAE) type I and type II is linked to defective C1 esterase inhibitor (C1-INH) encoded by the SERPING1 gene. There are substantial variabilities in the clinical presentations of patients with HAE that are not directly correlated to the serum levels of C1-INH. The impact of SERPING1 variants on C1-INH expression, structure, and function is incompletely understood.
OBJECTIVE: To investigate the influence of SERPING1 variants on the C1-INH expression, structure, and function of 20 patients with HAE from 14 families with no prior genetic diagn…

Hereditary angioedema (HAE) is a rare, autosomal disorder that manifests with unpredictable episodes of severe swelling of the skin and mucous membranes. These attacks can be highly disfiguring and range in severity from mild to-in cases of airway swelling-life-threatening. Fluctuations in female sex hormones-such as the changes that occur during puberty, menses, contraceptive use, pregnancy, and menopause-can all affect the frequency and severity of HAE attacks. Disease management decisions for women of childbearing age may be more complex and require additional considerations since they coul…

Angioedema is a form of allergic mediated by histamine and non-allergic mediated by bradykinin and can be lethal if not recognized and treated promptly. This case demonstrates the proper diagnosis of and intervention in rapid onset severe angioedema. A 68-year-old male came to the emergency department with a complaint of dyspnea that started two hours before. He had type II diabetes, chronic kidney disease and several different antihypertensive medications, including an ACE inhibitor for hypertension. During physical examination, the patient was hypertensive, tachycardic, tachypnoic, and edema…
