Decision-Making and Management of Acute Isolated Syndesmosis Injuries in Athletes.

BACKGROUND: The ankle joint is one of the most frequently injured joints in the athletic population. Some articles estimate that it accounts for approximately 40% of all sports injuries, with lateral ankle sprains accounting for approximately 85% of injuries whereas medial ankle and syndesmosis injuries account for approximately 4-5% and 1-11%, respectively.1-4 Incidence of ankle sprains has been reported to be as high as 7 per 1,000 exposures for court sports and 1 per 1,000 exposures for field sports with lateral ankle sprains being more common than medial or syndesmotic ankle inju-ries.5 Wh

Influence of Landing in Neuromuscular Control and Ground Reaction Force with Ankle Instability: A Narrative Review.

Ankle sprains are generally the most common injuries that are frequently experienced by competitive athletes. Ankle sprains, which are the main cause of ankle instability, can impair long-term sports performance and cause chronic ankle instability (CAI). Thus, a comprehensive understanding of the key factors involved in repeated ankle strains is necessary. During jumping and landing, adaptation to the landing force and control of neuromuscular activation is crucial in maintaining ankle stability. Ankle mobility provides a buffer during landing, and peroneus longus activation inhibits ankle inv

Republication of "Ankle Fracture-Dislocations: A Review".

Ankle fractures are common musculoskeletal injuries that may result in tibiotalar joint dislocations. Ankle fracture-dislocations occur via similar mechanisms as ankle fractures, although the persistence or magnitude of the deforming force is sufficient to disrupt any remaining bony or soft-tissue stability. Ankle fracture-dislocations likely represent distinct clinical entities, as the pathology, management, and patient outcomes following these injuries differ from those seen in more common ankle fractures without dislocation. Ankle fracture-dislocations have higher rates of concomitant injur

Prevention of ankle sprains.

Two different methods for the prevention of ankle joint injuries in soccer were tested. Coordination training on an ankle disk improves functional stability and postural control, whereas an orthosis provides mechanical support. Both techniques reduce the frequency of ankle sprains in soccer players with previous ankle problems. The orthosis is an alternative to taping, and can be used during the rehabilitation period after injury or when playing on uneven ground. Coordination training on an ankle disk ought to be included in the rehabilitation of ankle injuries to prevent functional instabilit…

Acute injuries in orienteerers.

The aim of this study was to characterize the type and severeity of acute injuries occurring in Finnish orienteerers in 1987 to 1991. The study is based on the orienteering license insurance records accounting for 2189 orienteering injuries during 69268 person-years of exposure in active orienteerers. Of these orienteerers, 73.0% were male; 73.5% (N = 1608) of all injuries occurred in males, so the injury rate was similar in males and females. The rate was highest in orienteerers 20 to 24 years of age and lowest in children. Injuries occurred most commonly during May to September (78.9% or all

Injuries in Swedish elite basketball.

All Swedish male and female elite basketball players were interviewed concerning their injuries during 1981/1982. Fifty-eight percent of the male and 62% of the female players reported injuries. The injury frequency was 2.5 injuries/1,000 activity-hours in male and 2.85 injuries/1,000 hours of activity in female players. This corresponds to 8.6 injuries/male team/season and 7.5 injuries/female team/season. An ankle twist was the most common injury (52%) while a knee injury was involved in 18%. Prophylactic orthoses for knee and ankle are suggested.

Can the Y balance test identify those at risk of contact or non-contact lower extremity injury in adolescent and collegiate Gaelic games?

OBJECTIVES: Lower extremity (LE) injuries are common in Gaelic games and lead to a significant economic and injury burden. Balance is considered a predictor of injury in other sports, however no research has examined its effect on LE injury in Gaelic games. This study aims to present normative data for the Y Balance Test (YBT), determine whether the YBT can identify those at risk of contact and non-contact LE and ankle injuries and generate population specific cut-off points in adolescent and collegiate Gaelic games. DESIGN: Prospective cohort study. METHODS: A convenience sample of 636 male…

Chronic lateral ankle instability and associated conditions: a rationale for treatment.

BACKGROUND: Ankle sprains have a high incidence of associated injuries and conditions that may be unrecognized at the initial time of injury. Failure to treat these conditions at the index surgery may compromise outcomes and delay recovery. The purpose of this study was to determine the type and frequency of associated injuries and conditions in military patients with chronic lateral ankle instability. METHODS: Between 1996 and 2002, 160 patients had 180 modified Broström-Gould lateral ankle ligament reconstructions for chronic ankle instability. A retrospective review of the clinical history

Prevention and treatment of infected foot and ankle wounds sustained in the combat environment.

Combat injuries to the foot and ankle are challenging to treat due to frequent high-energy mechanisms, environmental contamination, and soft tissue and bony damage. Prevention and treatment of infections in injuries to the foot and ankle are critical to achieving the goals of tissue healing and restoration of function. The guidelines for treatment of these foot and ankle injuries are similar to those in place for civilians; however, allowances must be made for the realities of combat including an often austere environment, the need for evacuation, and limitations on resources available for tre…

Common and Uncommon Injuries in Ultra-endurance Sports.

Ultra-endurance sports are associated with prolonged physical exercise both during training and competition. Musculoskeletal injuries are common as a result of the repetitive physical stresses. Stress fractures in the weight-bearing bones should always be suspected when ultra-endurance athletes present with pain over bony structures. Most stress fractures can be treated with activity modifications but some such as femoral neck and tibial shaft stress fractures may require operative fixation. The knee seems to be the most frequent source of injuries in ultra-endurance athletes. Patellofemoral s

Management of acute injuries of the tibiofibular syndesmosis.

The syndesmosis is important for ankle stability and load transmission and is commonly injured in association with ankle sprains and fractures. Syndesmotic disruption is associated with between 5 and 10% of ankle sprains and 11-20% of operative ankle fractures. Failure to recognize and appropriately treat syndesmotic disruption can portend poor functional outcomes for patients; therefore, early recognition and appropriate treatment are critical. Syndesmotic injuries are difficult to diagnose, and even when identified and treated, a slightly malreduced syndesmosis can lead to joint destruction

Sprain of the Medial Ankle Ligament Complex.

Injuries of the medial ankle ligament complex (MALC; deltoid and spring ligament) are more common following ankle sprains than expected, especially in eversion-external rotation mechanisms. Often these injuries are associated with concomitant osteochondral lesions, syndesmotic lesions, or fractures of the ankle joint. The clinical assessment of the medial ankle instability together with a conventional radiological and MR imaging is the basis for the definition of the diagnosis and therefore the optimal treatment. This review aims to provide an overview as well as a basis to successfully manage
