Thyrotoxicosis may present in many ways; severe vomiting as a prominent symptom of thyrotoxicosis is uncommon. In this paper, we report a 24-year-old Chinese male with hyperthyroidism who presented with recurrent severe vomiting. The patient had had intermittent vomiting for seven years and had lost approximately 15 kg of weight. Gastroscopic examinations revealed chronic gastritis and one occasion peptic ulcer. He was treated with antacid and proton pump inhibitors, but his symptoms had no relief. His presenting symptoms suggested hyperthyroidism and were confirmed by laboratory data. After a

Pseudomembranous colitis is mainly caused by antibiotics and Clostridium difficile infection. But conditions such as gastrointestinal surgery, antacid medication, anti-neoplastic agent or immunosuppressive agent which influences the normal flora of colon can induce colitis without the administration of any antibiotics. We experienced a 13 year-old male who was taking low-dose methotrexate for juvenile rheumatoid arthritis complained diarrhea and abdominal pain for 3 weeks. Sigmoidoscopic findings revealed diffuse patch yellowish pseudomembranes on the rectum. Histologic finding was compatible

Epidemiological studies have suggested an association between antacid therapy and development of listeriosis in humans. In this study we used a neutropenic mouse model to demonstrate that oral administration of sodium bicarbonate shortly before intragastric (i.g.) inoculation with Listeria monocytogenes EGD (serotype 1/2a) significantly increased the severity of the resulting systemic infection. An explanation for this observation is provided by evidence that L. monocytogenes EGD is rapidly inactivated in synthetic gastric fluid at pH below 5. A second strain of L. monocytogenes (CM [serotype

Following a multicenter, double-blind trial comparing cimetidine, antacid, and placebo for the treatment of gastric ulcer, patients whose ulcers had healed were followed prospectively to assess the frequency of ulcer relapse. Fifty-eight patients entered the follow-up study. Patients were encouraged to discontinue smoking and excessive ethanol intake, but were not maintained on antiulcer medications. Clinical evaluation was performed at monthly intervals; repeat endoscopy was performed at the time of symptom recurrence or at 6 to 9 months after ulcer healing. Gastric ulcer recurred in 20 of 58

134 outpatients with acute benign gastric ulcer confirmed by endoscopic biopsy received either 1 g sucralfate suspension four times daily or one 150 mg ranitidine tablet twice daily for six to 12 weeks in a multicentre therapy study (double-blind study according to the double-dummy technique). After six to 12 weeks, respectively, 56% and 82% of the ulcers had healed in the sucralfate group. The rates of healing in the ranitidine group were 72% and 88%, respectively. The differences in the rates of healing between sucralfate and ranitidine were not statistically significant. Ranitidine was supe

Patients who have rheumatic disease and who are undergoing long-term aspirin therapy have a high incidence of peptic ulcer disease. Whether it is possible to heal long-term aspirin-related peptic ulcer disease if aspirin intake is continued is unknown. Nine patients with rheumatic disease who were receiving long-term aspirin therapy and who had 15 endoscopically verified gastric and/or duodenal ulcers were studied. Patients were treated daily with 1,200 mg of cimetidine plus at least 120 mL of antacid (Mylanta II), while continuing aspirin therapy at the same dose and type. By eight weeks, 14

BACKGROUND: Laparoscopic sleeve gastrectomy (LSG) is one of the most popular bariatric procedures because of its simplicity and good weight loss effects. However, transhiatal sleeve migration (THSM) after LSG is currently considered an aggravating factor for postoperative weight regain and gastroesophageal reflux disease (GERD). Thus, our aims in this study were to clarify the incidence of THSM after LSG and to evaluate the relationships between THSM parameters and clinical outcomes. In this paper, we introduce a novel definition of THSM after LSG.
METHODS: We enrolled 107 patients with sever

INTRODUCTION: Raltegravir is a potent HIV-integrase strand transfer inhibitor (INSTI). Despite its strong activity against HIV-1 strains resistant to other antiretroviral drug classes, it is usually used in combination with other antiretroviral drugs due to the empirical requirement for anti-HIV drug combinations to ensure effective anti-retroviral therapy (ART). As an early-arriving INSTI, raltegravir is clinically familiar for its safety, tolerability and treatment effectiveness. High-dose calcium carbonate formulated as an antacid (as opposed to a supplement formulation) taken orally togeth

BACKGROUND/AIMS: The short-term outcomes of laparoscopic antireflux surgery (LARS) in treatment of gastroesophageal reflux disease (GERD) are quite good, but some long-term studies have shown that the early good results are worsening with time. The aim of this study was to evaluate retrospectively, the long-term outcome of LARS with special attention to determinant factors.
METHODOLOGY: From August 1994 (start of Mansoura program) to June 2004, 250 patients underwent LARS at the Mansoura gastroenterology surgical center, Mansoura University, Egypt. Preoperative symptoms and the results of esop

In otorhinolaryngology, the relationship between gastroesophageal reflux (GER) and pharyngolaryngeal disorders is well-known. In fact, many patients with GER debut with head and neck symptoms or are first seen by an otolaryngologist. We proposed to identify the ENT symptoms most frequently associated with GER, to differentiate between physiological and pathological GER, and to confirm the effectiveness of antacid treatment. Our study included 20 ambulatory patients who presented pharygolaryngeal symptoms and clinical manifestations of GER (heartburn and regurgitation). The patients underwent a
