Cataract surgery is the most frequently performed surgical procedure worldwide. We aim to determine the prevalence of having implanted an artificial lens (pseudophakia) and of no lens (aphakia) and to compare visual function. As part of the Gutenberg Health study, a population-based cross-sectional study was conducted in Germany. An ophthalmological examination including slit-lamp examination was conducted. Prevalence including 95% confidential intervals were calculated and analyses were conducted for systemic and ocular associated factors with pseudophakia using multivariable logistic regressi

UNLABELLED: We describe using the glued intraocular lens (IOL) scaffolding technique to remove Soemmerring ring during secondary IOL implantation in aphakic eyes with associated posterior capsule defect following previous pediatric cataract surgery. Vitrectomy is performed, and the remnants of the posterior capsule are removed from the visual axis. A 3-piece foldable IOL is injected below the Soemmerring ring, and glued transscleral haptic fixation of the IOL is performed. The Soemmerring ring is dislodged from the periphery with a Sinskey hook and brought into the center of the pupil over the

UNLABELLED: We describe a technique for sutureless intrascleral fixation of a 3-piece intraocular lens (IOL) and custom silicone iris prosthesis complex to address the optical and functional issues of aphakia and aniridia, as well as to restore excellent cosmesis. The 3-piece IOL is anchored to the iris prosthesis using the belt-loop technique. We believe the intrascleral haptic fixation is overall a more robust fixation method in patients with aphakia and aniridia, who are often younger and require a long-lasting anterior segment reconstruction.

We describe a technique for performing cataract surgery with a femtosecond laser in infants. Anterior capsulotomy and lens division are performed by the laser. A fluid-filled interface makes it possible to re-dock the laser to the eye for posterior capsulotomy after the eye has been opened for lens aspiration without complications. The integrated 3-dimensional optical coherence tomography also visualizes the posterior capsule, allowing a centered central posterior capsulotomy followed by mechanical anterior vitrectomy. The anterior and the posterior capsule disks are removed without capsule te

Aniridia or iris deficiencies can severely affect visual function in mostly younger patients and should therefore be corrected. Currently various technical options for the treatment of iris deficiencies are available. The present review considers the main causes of aniridia and the incidence and summarizes the negative effects on visual function. A brief historical overview of the development of prosthetic iris devices is given. Currently available prosthetic iris devices, mainly iris diaphragm lenses and segmental prosthetic iris devices, are presented along with the specific advantages and d

A 68-year-old man with Graves orbitopathy underwent a bilateral swinging eyelid orbital decompression for disfiguring proptosis. A penetrating keratoplasty for keratoconus of the left eye had been performed 13 years earlier. Prior to orbital decompression, biomicroscopy of the left eye showed a partially decompensated sutureless corneal graft with good wound apposition. The corneal stroma appeared edematous, and some epithelial bullae were seen. During orbital wall removal, the corneal wound dehisced for 270 degrees. Swinging eyelid orbital decompression is the technique of choice in the Nethe

PURPOSE: Intraocular lenses (IOLs) are now being implanted in infants and children with unilateral cataracts. However, there are no prospective data on the development of visual acuity after implantation. The aim of the present study was to prospectively assess the development of acuity in infants and preschool children who received IOLs or aphakic contacts lenses (CLs) after the extraction of a unilateral cataract.
METHODS: Visual acuity was assessed using Teller Acuity Cards and/or crowded HOTV tests at target ages of 6 months, 1, 2, 3, and 4 years.
RESULTS: Infants who received a primary

PURPOSE: Open-angle glaucoma may develop after surgery for congenital or developmental cataract with an incidence ranging from 3% to 41%. The pathogenesis of "aphakic" (open-angle) glaucoma remains unknown. Despite numerous reported clinical series (>1000 eyes), we are unaware of any reported case of open-angle glaucoma after primary intraocular lens (IOL) implantation for congenital or developmental cataract. We decided to test the hypothesis that primary posterior chamber IOL implantation might decrease the incidence of open-angle glaucoma in children.
METHODS: Pseudophakic eyes were collec
