Severe obstructive sleep apnea in a child with osteopetrosis.

The clinical features of a 5-year-old patient with osteopetrosis and severe obstructive sleep apnea are presented. The patient responded to tonsillectomy, adenoidectomy, uvulectomy, and tracheostomy. The tracheostomy was later removed. Osteopetrosis, while rare, may be added to the list of causes of obstructive sleep apnea.

[Concomitant narcolepsy and sleep apnea: report of a case].

A case of concomitant narcolepsy and obstructive sleep apnea evaluated in a sleep disorder center is reported. Tracheostomy decreased the frequency of the apneas but the clinical and polysomnographic features of narcolepsy remained on 3 and 6 months post-surgical follow-ups. These findings support the need for systematic use of all-night polysomnography and multiple sleep latency test in patients with excessive daytime sleepiness.

The electrocardiogram in preterm infants with intraventricular hemorrhage and apnea.

To determine the relationship of electrocardiographic changes to both intraventricular hemorrhage (IVH) and neonatal apnea, continuous electrocardiographic monitoring, serial electrocardiograms, and cranial echoencephalograms were obtained on 41 consecutive preterm neonates with birth weight below 1,250 g. Nineteen patients (46%) were diagnosed as having an IVH within the first postnatal week. Six patients died while still requiring mechanical ventilation. Of the 35 surviving infants, 21 experienced apnea. Though the QRS axis was significantly different in the two groups, in both groups it was…

Phenothiazine-induced sleep apneas in normal infants.

A relationship between sudden infant death syndrome and the use of phenothiazine-containing medications is suspected. In order to investigat … in body movements. During the second night, each infant showed an increase in the number of central apneas, and each infant also had several episodes of obstructive apneas (median duration four seconds). It is concluded that promethazine depresses the arousal and respiratory mechanisms in normal infants during sleep. This observation reinforces the opin …

A surgical treatment for snoring and obstructive sleep apnea.

Snoring caused by oropharyngeal obstruction and some cases of obstructive sleep apnea syndrome can be cured or considerably lessened by resecting redundant tissue from patients' oropharynx and soft palate. Preoperative, and in some instances postoperative, sleep monitoring is a necessary part of evaluating these conditions.

Esophageal spasm associated with apnea and bradycardia in an infant.

A newborn infant was found to have diffuse esophageal spasm in association with apnea and bradycardia. This association has not been physiologically documented before. The infant's condition improved after the administration of glycopyrrolate, an anticholinergic agent. It is speculated that the association of esophageal spasm and bradycardia may be mediated by the vagus.

Apnea and sudden unexpected death in infants with achondroplasia.

Thirteen infants with achondroplasia and sudden unexpected death or unexplained apnea were discovered through nonsystematic retrospective case collection. Most were initially thought to have died from sudden infant death syndrome. However, historical and pathologic findings suggest that many of these infants had apnea and sudden unexpected death secondary to acute or chronic compression of the lower brainstem or cervical spinal cord. Infants with achondroplasia evidently are at considerably increased risk for such deaths between 1 month and 1 year of age. Appropriate intervention, given these …

A critical dissection of obstructive apnea in the human infant.

It is universally accepted that with the cessation of airflow, the infant is technically apneic, but the significance of such respiratory "pauses" remains a matter of discussion. Central apnea is easily recognized, but the absence of airflow with continued "respiratory" movements is commonly interpreted as obstructive apnea. The respiratory pattern of individual normal and abnormal infants was studied in depth. Airflow was monitored using a recently devised facial plate thermistor system; chest movement was recorded by measuring changes in thoracic impedance. On several occasions, esophageal p…

Sleep and aminophylline treatment of apnea in preterm infants.

The influence of short-term aminophylline treatment on sleep behaviour was studied in six preterms infants with recurrent apnea. The incidence of apnea, respiratory pauses, and bradycardias which were closely related to the phases of active sleep, decreased during aminophylline treatment. However, the amount of active sleep remained unaffected. The mode of action of aminophylline is discussed in view of the previously proposed neurophysiological concept of apnea of prematurity.

Association Between Sleep Apnea Symptoms Subtypes and Obesity.

Background and Objectives: Obstructive sleep apnea (OSA) is a heterogeneous disease with diverse clinical presentations and high global prevalence. Obesity is a common comorbidity in OSA, but its relationship with symptom subtypes remains unclear. This study aimed to evaluate the association between OSA symptom subtypes and obesity in a clinical cohort. Methods: This observational study analyzed data from the Santiago Obstructive Sleep Apnea (SantOSA) prospective clinical cohort, including adults with OSA confirmed by home sleep apnea testing. Symptom subtypes were identified using latent clas…

Hypoxic blackout in dynamic apnea: A case report.

Blackout (BO) in breath-hold diving is attributed to cerebral hypoxia, yet direct observations are rare. We continuously recorded arterial oxygen saturation (SpO₂) and heart rate (HR) in 11 trained freedivers (5 females) performing two dynamic apneas (75 m, 100 m) using a waterproof forehead oximeter. One diver experienced BO at the end of a 100 m dive (SpO₂ 51 %), recovering within 5 s. Group SpO₂ fell from 98 ± 1 % to 77 ± 9 % (75 m) and 68 ± 9 % (100 m; range 51-83 %), while mean HR declined from 83 ± 12 to 43 ± 8 and 40 ± 4bpm, respectively. No arrhythmias were detected. Within-diver SpO₂ …

Sleep apnea and substance use: a meta-analysis.

INTRODUCTION: Sleep apnea and substance use disorders appear to interact bidirectionally, negatively influencing each other. Obstructive sleep apnea syndrome (OSA) seems particularly prevalent among substance users. METHODS: This meta-analysis aims to evaluate the impact of the most commonly used substances (alcohol, tobacco, cannabis, opioids, and cocaine) on the apnea-hypopnea index (AHI) by comparing the mean AHI scores between users and non-users. RESULTS: 3784 articles were identified from the PubMed search, and 28 studies were included in the meta-analysis. We found a higher AHI among …

Delayed Arousal Response to Sleep Apnea Encodes Mortality.

RATIONALE: Conventional measures of obstructive sleep apnea severity, particularly the apnea-hypopnea index, do not adequately capture event-level neurophysiologic responses to respiratory events. Whether post-apnea/hypopnea arousal dynamics provide prognostic information beyond established metrics remains unknown. OBJECTIVES: To determine whether post-apnea/hypopnea arousal dynamics are associated with all-cause and cardiovascular mortality. METHODS: We conducted a retrospective analysis of in-home polysomnography data from 8,053 adults across four community-based cohorts. Peak time (PT; la…

Association of nasopharynx cancer with obstructive sleep apnea.

INTRODUCTION: Despite the extensive research conducted on the association between OSA and HNCs, the specific relationship between obstructive sleep apnea (OSA) and nasopharyngeal carcinoma (NPC) has received comparatively less attention. The present study aimed to explore the association between OSA and NPC. METHODS: The data for this study was drawn from the Longitudinal Health Insurance Database 2010. The study incorporated a total of 3,235 NPC patients and 12,940 propensity score-matched controls. The NPC and controls' baseline clinical and demographic parameters were compared using standa…

NLRP3 Inflammasome Triggers Inflammation of Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is widespread in the population and affects as many as one billion people worldwide. OSA is associated with dysfunction of the brain system that controls breathing, which leads to intermittent hypoxia (IH), hypercapnia, and oxidative stress (OS). The number of NOD-like receptor family pyrin domain-containing (NLRP3) inflammasome was increased after IH, hypercapnia, and OS. NLRP3 inflammasome is closely related to inflammation. NLRP3 inflammasome causes a series of inflammatory diseases by activating IL-1β and IL-18. Subsequently, NLRP3 inflammasome plays an import…

[Advances in rehabilitation therapy for obstructive sleep apnea].

Obstructive sleep apnea (OSA) is a systemic disease with multi-organ impairment. Rehabilitation therapy is an important component of the comprehensive treatment strategy for OSA. Exercise training can reduce the severity of sleep apnea and daytime sleepiness in OSA patients, while improving sleep efficiency and cardiopulmonary function. Upper airway muscle training can enhance muscle tone during sleep and increase airway anti-collapse capacity, although its long-term efficacy requires further confirmation through high-quality studies. Respiratory muscle training may reduce upper airway collaps…
