Hand-arm vibration syndrome: a common occupational hazard in industrialized countries.

Regular exposure to hand-transmitted vibration can result in symptoms and signs of peripheral vascular, neurological and other disorders collectively known as the hand-arm vibration syndrome (HAVS). A significant proportion of workers can suffer from HAVS after using vibrating power tools. HAVS is a chronic and progressive disorder. Early recognition and prevention is the key to managing vibrating tool exposures and health effects. This article gives a broad overview of the condition with a detailed account of its pathogenesis, identification and management.

Tissue expansion and latissimus dorsi transfer for arm-thorax synechia reconstruction.

Inadequate treatment of severe burn injury often results in scar contractures with subsequent functional impairment and cosmetic deformities depending on the body area involved. Arm-thorax synechia is a rare but devastating complication of burns. The authors report a case of a 17-year-old boy with subtotal arm-thorax synechia, secondary to burn contracture. Reconstruction was accomplished, in two stages, with the use of tissue expanders and a latissimus dorsi muscle flap. The postoperative course was uneventful in both stages, with complete survival of both the expanded skin and the muscle fla…

Bilateral simultaneous traumatic upper arm compartment syndromes associated with anabolic steroids.

BACKGROUND: Acute compartment syndrome, a surgical emergency, is defined as increased pressure in an osseofascial space. The resulting reduc … of the accident, the patient developed clinical features that were consistent with bilateral upper arm compartment syndrome. Bilateral fasciotomies of both anterior and posterior compartments were performed, confirming clinical suspicion. CONCLUSION: We suggest consideration of a history of anabolic steroid use when evaluating patients with extremit …

Contributions of altered stretch reflex coordination to arm impairments following stroke.

Patterns of stereotyped muscle coactivation, clinically referred to as synergies, emerge following stroke and impair arm function. Although researchers have focused on cortical contributions, there is growing evidence that altered stretch reflex pathways may also contribute to impairment. However, most previous reflex studies have focused on passive, single-joint movements without regard to their coordination during volitional actions. The purpose of this study was to examine the effects of stroke on coordinated activity of stretch reflexes elicited in multiple arm muscles following multijoint…

[Compartment syndrome in upper arm in anticoagulant therapy after minor trauma].

Compartment syndrome can be a complication of warfarin treatment after a minor trauma. We report a case of an elderly woman who had an uncontrolled, high Internationalised Normalized Ratio (INR) level and had incurred a large haematoma on the left upper arm after a fall. After two days, the patient developed a massive oedema and clinical compartment syndrome. It is essential to be aware of symptoms and signs of compartment ischaemia, as early fasciotomy can prevent late complications such as muscle necrosis and contracture or at worst amputation. The patient should be hospitalized for observat…

Achieving super-resolution X-ray imaging with mobile C-arm devices.

BACKGROUND: The term super-resolution refers to the process of combining a set of low-resolution images into a high-resolution image using image processing methods. This work is concerned with the reconstruction of high-resolution X-ray images. Specifically, we address the problem of acquiring X-ray images from multiple, very close view points. METHODS: We propose to use a novel experimental robotic C-arm device to create high-resolution X-ray images. For this purpose, we suggest different strategies for acquiring multiple low-resolution images, and we provide the steps to achieve acquisition…

Palmoplantar pustulosis treated with itraconazole: a single, active-arm pilot study.

Pustulosis palmoplantaris (PPP; synonyms: pustulosis palmaris et plantaris, palmoplantar amicrobic pustulosis) is a common chronic, relapsing, pustular eruption affecting the palms and soles. The authors report the successful treatment of six therapy-experienced patients with histologically confirmed PPP with oral itraconazole (100 mg/day for 1 month, followed by a month of 100 mg/day every other day). Three of six patients showed complete clearance of pustules, significant reduction of erythema, and unnoticeable desquamation, whereas the other three patients had no new pustules appearing and

[Reconstruction of the tongue with lateral arm free flap after hemiglossectomy].

OBJECTIVE: To investigate the value of microsurgical reconstruction of the tongue after hemiglossectomy with lateral arm free flaps in the treatment of tongue cancer. METHODS: From October 2006 to April 2007, ten patients underwent simultaneous reconstruction of the tongue and oral floor defects with lateral arm free flaps after resection of squamous cell carcinoma of tongue in our hospital. The flaps ranged from 8 cm x 5 cm to 9 cm x 6 cm in size, and were adjusted to the defect of the tongue. The vascular pedicle included the posterior radial collateral artery and the accompanying veins. Th…

Altered dorsal premotor-motor interhemispheric pathway activity in focal arm dystonia.

Given the possible role of dorsal premotor cortex (PMd) in the pathophysiology of dystonia, we used transcranial magnetic stimulation (TMS) methods to study PMd and PMd-primary motor cortex (M1) interactions in patients with focal arm dystonia. Here, we tested the connectivity between left PMd and right M1 as well as the intracortical excitability of PMd in 11 right-handed patients with focal arm/hand dystonia and nine age-matched healthy controls. The results showed that excitability of the inhibitory connection between PMd and M1 was reduced in patients, but there was no significant differen…

Minimally invasive vertebroplasty managed by a two C-arm fluoroscopic technique.

One C-arm fluoroscope is widely considered to be sufficient for effectively monitoring minimally invasive vertebroplasty. However, rotating the C-arm fluoroscope for different radiographic views of the spine can be time-consuming, causing delay in detecting cement leakage. We retrospectively reviewed the results of minimally invasive vertebroplasty utilizing two C-arm fluoroscopes in the treatment of osteoporotic vertebral fractures. Forty-two patients (50 vertebrae) were treated with minimally invasive vertebroplasty under the guidance of two C-arm fluoroscopes. The mean operation time, patie…

Hand-arm vibration syndrome with concomitant arterial thrombosis in the hands.

BACKGROUND: Hand-arm vibration syndrome (HAVS) refers to the vascular, neurological and musculoskeletal effects that may occur in workers with prolonged exposure to vibrating tools. Hypothenar hammer syndrome (HHS) is a lesion of the ulnar artery at the level of the hamate bone secondary to single or repeated episodes of trauma to the hypothenar eminence. The literature suggests that digital arterial thrombosis and HHS may be associated with the use of vibrating tools. AIM: This study will familiarize investigators with the range of vascular abnormalities seen in workers using vibrating tools…

When mirrors lie: "visual capture" of arm position impairs reaching performance.

If we stand at a mirror's edge, we can see one half of our body reflected in the mirror, as if it were the other half of our body, seen "through" the mirror. We used this mirror illusion to examine the effect of conflicts between visually and proprioceptively specified arm positions on subsequent reaching movements made with the unseen right arm. When participants viewed their static left arm in the mirror (i.e., as if it were their right arm), subsequent right-arm reaching movements were affected significantly more when there was conflict between the apparent visual and the proprioceptively s…
