AV node reentry tachycardia in infants.

The purpose of this study was to determine the frequency of atrioventricular (AV) node reentry tachycardia in infants undergoing transesophageal electrophysiological study for paroxysmal tachycardia. The records of all 52 infants < 1-year-old with structurally normal hearts who underwent transesophageal study for paroxysmal tachycardia over a 3-year period were reviewed. Those with a diagnosis of AV node reentry tachycardia underwent complete data review, and follow-up of > 12 months was obtained. Six of 52 infants had a diagnosis of the common type of AV node reentry tachycardia. Tachycardia

Differential sequential septal pacing: a simple maneuver to differentiate nodal versus extranodal ventriculoatrial conduction.

BACKGROUND: Distinguishing retrograde nodal conduction from extranodal conduction using an accessory pathway (AP) can sometimes be challenging. OBJECTIVE: To distinguish nodal from extranodal ventriculoatrial (VA) conduction regardless of AP location by proposing a simple method. This method is based on the principle that moving the pacing site progressively from the basal region toward the entrance of the His-Purkinje system should shorten VA time for nodal but not for AP conduction. METHODS: Sixty-seven patients with supraventricular tachycardia were prospectively recruited. Quadripolar ca

Histopathological changes in the electrical conduction of cardiac nodes after acute myocardial infarction in dogs and horses, compared with findings in humans: A histological, morphometrical, and immunohistochemical study.

BACKGROUND AND AIM: The heart conduction system is responsible for the occurrence of various types of cardiac arrhythmia. This study aimed t … iac conditions. RESULTS: In both species, the sinoatrial node (SAN) was ovoid in shape whereas the atrioventricular node (AVN) was pyramidal in shape. The percentage of collagen fibers inside the SAN of dogs (47%) and horses (50%) was found to be higher than that of cells. In contrast, the percentage of cells in the AVN of dogs (24%) and horses (16%) was higher than t …

Pacemaker treatment in familial amyloidosis with polyneuropathy.

Involvement of the heart is common in familial amyloidosis with polyneuropathy (FAP) and is manifested by disturbances of atrioventricular and intraventricular conduction and dysfunction of the sinus node. Pacemaker implantation was performed in 20 patients with FAP in northern Sweden between 1968 and 1983 for the following indications: complete heart block (11), second degree heart block (1), sinus node dysfunction (5), and atrial fibrillation with a slow ventricular rate (3). There was a prompt improvement of symptoms attributable to a slow heart rate. Dislodgement of the electrode occurred

[Carotid massage for diagnosis and treatment of supraventricular tachycardia].

Supraventricular tachycardias (SVTs) are a common reason for A and E visits, presenting with palpitations or dizziness. Differentiating subtypes is crucial but can be challenging. Adenosine treatment is standard for atrioventricular node-dependent SVTs, but vagal manoeuvres like Valsalva or carotid sinus massage (CSM) offer effective, non-invasive options. This case report details a 66-year-old man with SVT, where CSM slowed the heart, revealing atrial flutter on ECG. This highlights CSM's diagnostic utility, safety, and practicality, emphasizing simple, cost-effective methods in emergency car…

Surgical cryoablation for life-threatening postoperative junctional tachycardia.

Junctional ectopic tachycardia is usually a limited but potentially life-threatening postoperative arrhythmia. We describe a case of malignant postoperative junctional ectopic tachycardia in a 13-month-old child who had undergone transatrial and transpulmonary repair of tetralogy of Fallot resistant to all conventional therapeutic measures and complicated by recurrent ventricular tachycardia. The arrhythmia was finally treated by open surgical cryoablation of the atrioventricular node and the implantation of a permanent pacemaker. The patient made an uneventful recovery followed by good long-t…

Focal right atrial dysplasia and atrial flutter in a patient with myotonic dystrophy.

Focal dysplasia of the right atrium was identified postmortem in a 22-year-old man with myotonic dystrophy and sudden death. Antemortem cardiac abnormalities included a single syncopal episode associated with atrial flutter with exercise-induced 1:1 atrioventricular conduction, sinus node dysfunction, and mild mitral valve prolapse. Pathologically there was only mild conduction system disease and the ventricular myocardium was normal. Right atrial dysplasia, previously unreported in myotonic dystrophy, appears to have been an arrhythmogenic lesion in this patient, serving as a morphological su

Sinuatrial disease in young people.

A survey at three cardiac centres disclosed nine patients under the age of 25 years with sinus node dysfunction in the absence of other forms of heart disease. All were male and seven were above the 90th centile for height. Ambulatory monitoring was performed on all the first-degree relatives of six of the patients and three families showed conducting system disturbances in the form of sinuatrial disorders or delayed atrioventricular conduction. A genetic factor may be involved in the aetiology of sinuatrial disease in young people.

Coexistence of functional Kent and Mahaim-type tracts in the pre-excitation syndrome. Demonstration by catheter techniques and epicardial mapping.

An unusual patient with Ebstein's anomaly of the tricuspid valve and the Wolff-Parkinson-White syndrome is presented. Ventricular pre-excitation related to conduction over both a right posterior Kent bundle and Mahaim fibers coursing from the atrioventricular node to the right ventricle. Two types of supraventricular tachycardia were demonstrable. These were due to re-entry involving antegrade conduction over either the normal or Mahaim paths, and retrograde retrograde conduction via the Kent bundle. Surgical division of the Kent bundle has abolished the clinically debilitating arrhythmias.

A left atrial versus a biatrial lesion set for persistent atrial fibrillation ablation during open heart surgery.

OBJECTIVES: To compare the outcomes of left atrial ablation and biatrial (BA) ablation in patients with persistent and long-standing persistent atrial fibrillation undergoing open heart surgery. METHODS: Between January 2007 and December 2016, 588 consecutive patients with either persistent (20.9%) or long-standing persistent atrial fibrillation (79.1%) were enrolled in this study. To reduce between-group differences, propensity score-matched groups (156 patients/group) were obtained, with similar preoperative and perioperative characteristics. RESULTS: The propensity score-matched left atri

Prospective assessment after pediatric cardiac ablation: fate of intracardiac structure and function, as assessed by serial echocardiography.

BACKGROUND: Catheter ablation puts cardiac valves at risk of damage, and children are of particular concern. METHODS: A multicenter prospective study was performed to assess the results and risks associated with radiofrequency (RF) ablation in children. Patients were aged 0 to 16 years with supraventricular tachycardia due to accessory pathway or atrioventricular node reentry, excluding patients with more than trivial congenital heart disease. A total of 481 patients were recruited into the prospective cohort and were followed up at 2, 6 and, 12 months after ablation. Complete echocardiograms…

Non-pharmacological treatments for atrial fibrillation. A critical perspective on the status quo.

In a proportion of patients with atrial fibrillation, antiarrhythmic drugs are either ineffective, unsafe, or poorly tolerated. Accordingly, a variety of non-pharmacological treatments have been developed. This article critically reviews these modalities. (i) For ventricular rate control, catheter ablation of the atrioventricular node with pacemaker implantation is commonplace. An alternative is atrioventricular node modulation using a procedure similar to "slow pathway" ablation. (ii) For restoration of sinus rhythm, internal cardioversion using low energy shocks is highly effective; this has…

Antiarrhythmic and proarrhythmic properties of diazepam demonstrated by electrophysiological study in humans.

We evaluated the electrophysiological parameters before and after the intravenous infusion of diazepam (0.2 mg/kg) in 20 cardiac patients to investigate the drug's antiarrhythmic effect. Diazepam did not significantly change the arterial pressure. After the intravenous infusion of diazepam, the sinus cycle length significantly shortened from 847 +/- 132 to 747 +/- 155 ms (p less than 0.01). No significant change in the maximal sinus node recovery time was noted. The AH interval at the atrial pacing length of 600 ms shortened significantly from 140 +/- 40 to 127 +/- 39 ms (p less than 0.05). Ho

Effects of the antianginal drug trapidil on atrioventricular conduction disturbances during acute myocardial ischemia.

Adenosine and related compounds have been shown to produce atrioventricular (AV) conduction block during acute myocardial ischemia. We investigated the effects of the antianginal drug trapidil, which has been shown to inhibit phosphodiesterase, on AV conduction disturbances in a canine model of acute myocardial ischemia. In 35 anesthetized dogs, the AV node artery was cannulated and perfused with arterial blood. Adenosine (300 µg, 650 µg, or 1000 µg) was injected into the AV node artery. With administration of adenosine at 300 µg, 650 µg, or 1000 µg, the atrio-His (AH) interval was increased b

Morphological classification of atrial muscle in the atrioventricular junctional area--3-dimensional reconstruction of serial sections of the human heart.

To trace anatomical structures that might be associated with the dual atrioventricular (AV) nodal pathway and to investigate the morphologic characteristics of the cells that form these pathways, we examined serial sections of the AV junctional area with a light microscope and reconstructed them 3-dimensionally with a computer. Twelve hearts were obtained at autopsy from patients who had not shown AV conduction disturbances or supraventricular tachycardia before death. The method of Lev et al was used to prepare serial sections. Fascicles of atrial muscle contiguous with the AV node were exami
