[Clinical application of micro-vibration to autonomic function test].

Microvibration (MV) is the minute vibration observed on surface of the human body. MV on the thenar eminence has been used as an autonomic function test, and is one of the important psychophysiological methods in psychosomatic medicine because the measurement of MV is not invasive. Although the mechanism generating MV has not been definitely clarified, there seems to be two major components, a ballistocardiographic one and a muscular one. The analysis of MV focuses on frequency characteristics. To broaden the clinical application of MV measurement, it is necessary to improve the analyzing meth

Electrophysiological assessment of peripheral and central autonomic disorders.

The autonomic nervous system (ANS) coordinates multiple reflex actions which are essential for life. The tests employed to evaluate the ANS provide valuable information of the functional state of these reflex arcs. The ideal test should be simple to perform, noninvasive, reproducible, sensitive, specific, safe, and appropriate for longitudinal studies. The availability of computer-based techniques has facilitated the electrophysiological assessment of ANS-mediated reflexes. The information provided by autonomic testing must be analyzed in combination with the clinical history and physical exam…

[Regulation of Systemic Metabolism by Autonomic Nerve-Mediated Inter-Organ Networks].

Autonomic nerves, including sympathetic nerves and parasympathetic nerves, innervate organs and modulate those function, thereby regulating systemic metabolism. Autonomic nerve bundles contain both afferent and efferent nerve fibers, and both fibers play important roles in regulating systemic metabolism for maintenance of homeostasis at the whole-body level. Furthermore, recently-obtained evidence unraveled the metabolic regulatory systems through autonomic nerve-mediated inter-organ networks. In these systems, afferent innervations of autonomic nerves transmit metabolic information from perip

[Autonomic function tests for analysis of pathophysiology and prognosis of autonomic neuropathy].

It is recommended to assess each autonomic system with system-specific function tests. Pupillary and cardiocirculatory systems have a benefit to perform the tests quantitatively, but most of the other systems are measured qualitatively. The aim of the tests exist in detecting the focus and the severity of a disease. Some tests are favored to be used repeatedly for judging the recovery. Head-up tilt and Valsalva tests are not adequate to observe the short-term improvement, but serum noradrenalin level has a good correlation to the rapid recovery. Autonomic symptoms may occur due to the dysfunct

[Acute Sensory Neuropathies and Acute Autonomic Neuropathies].

From the perspective of neuropathies with an acute onset mimicking that of Guillain-Barré syndrome (GBS), cases with profound sensory and/or autonomic impairment without any significant weakness have been reported. Although the possibility of infectious or toxic etiologies should be carefully excluded, immune mechanisms similar to those in GBS are suggested to be involved in these so-called acute sensory neuropathies and acute autonomic neuropathies. The types of neuropathy include those with predominant sensory manifestations, predominant autonomic manifestations such as autoimmune autonomic

[Cognitive function and blood pressure regulation in Lewy body diseases].

It is well known that autonomic failure is severer in patients with dementia with Lewy bodies (DLB) compared with patients with Parkinson's disease (PD). According to the Braak's hypothesis, Lewy bodies first appear in the olfactory bulb or peripheral autonomic nervous system. Lewy bodies in the peripheral autonomic nervous system ascend to dorsal motor nuclei of vagus nerve, while those in the olfactory bulb expand to the limbic system. Lewy bodies later attain the substantia nigra. However, it seems that Braak staging can not explain difference in severity of autonomic failure between DLB an…

Autonomic control of bone formation: its clinical relevance.

Bone remodeling is thought to be regulated by many factors including nutritional status, humoral factors, and biomechanical stress. However, the involvement of the autonomic nervous system, mainly the sympathetic nervous system (SNS), in the modulation of bone remodeling is beginning to receive more attention. Neural innervation of bone has been demonstrated. Both experimental and clinical evidence has indicated the involvement of autonomic nervous system regulation in bone metabolism. The sympathetic neural pathway is so far the only identified link between the potent leptin-dependent central…

Paraneoplastic autonomic neuropathies and GI dysmotility.

A number of the well-recognized autoimmune and paraneoplastic neurologic syndromes commonly involve the autonomic nervous system. In some cases, the autonomic nerves or ganglia are primary targets of neurologic autoimmunity, as in immune-mediated autonomic ganglionopathies. In other disorders such as encephalitis, autonomic centers in the brain may be affected. The presence of autonomic dysfunction (especially gastrointestinal dysmotility) is sometimes overlooked even though this may contribute significantly to the symptom burden in these paraneoplastic disorders. Additionally, recognition of …

Making the Invisible Visible: Understanding Autonomic Dysfunctions Following Spinal Cord Injury.

Autonomic dysfunctions are a major challenge to individuals following spinal cord injury. Despite this, these consequences receive far less attention compared with motor recovery. This review will highlight the major autonomic dysfunctions following SCI predominantly based on our present understanding of the anatomy and physiology of autonomic control and available clinical data.

Autoimmune Autonomic Disorders.

PURPOSE OF REVIEW: Autonomic disorders sometimes occur in the context of systemic autoimmune disease or as a direct consequence of autoimmunity against the nervous system. This article provides an overview of autonomic disorders with potential autoimmune etiology. RECENT FINDINGS: Recent evidence highlights a close association between the autonomic nervous system and inflammation. The autonomic nervous system regulates immune function, and autonomic manifestations may occur in a number of systemic autoimmune diseases. In a few instances, autoimmunity directly influences autonomic function. Au…

Sensory-autonomic interactions in health and disease.

Although sensory and autonomic nerve fibres generally do not interact directly, both may exert influences on blood flow during inflammation. For example, the sympathetic neurotransmitter noradrenaline/norepinephrine evokes axon reflexes, a response that involves release of vasoactive neuropeptides from the peripheral terminals of primary nociceptive afferent fibres. As well as boosting inflammation, this mechanism could play a role in normal renal function and heat dispersal from the skin. In certain disease states, aberrant communication between sensory and autonomic nerves might not only agg

Autonomic dysfunction in multiple sclerosis.

Autonomic dysfunction is a prevalent and significant cause of disability among patients with multiple sclerosis. Autonomic dysfunction in multiple sclerosis is usually explained by lesions within central nervous system regions responsible for autonomic regulation, but novel evidence suggests that other factors may be involved as well. Additionally, the interactions between the autonomic nervous system and the immune system have generated increased interest about the role of autonomic dysfunction in the pathogenesis of multiple sclerosis. In this paper we analyze systematically the most relevan…
