Suspected bile acid gastritis and dumping syndrome after pyloric drainage procedures: experience from a tertiary care center.

BACKGROUND: Pyloric drainage procedures, such as gastric peroral endoscopic myotomy (G-POEM) and pyloroplasty, have emerged as effective treatment modalities for refractory gastroparesis. Bile acid gastritis (BAG) and dumping syndrome (DS) are known complications of pyloric procedures. However, the incidence of these complications is underreported. METHODS: A retrospective review of patients with refractory gastroparesis who underwent G-POEM or pyloroplasty (July 2019 to June 2024) was conducted. Data included demographics, gastroparesis etiology, preoperative reflux, procedure type, and post

Comparison of Sleeve Gastrectomy with Loop Duodenojejunal Bypass Versus One Anastomosis Gastric Bypass for Type 2 Diabetes: The Role of Pylorus Preservation.

BACKGROUND: One anastomosis gastric bypass (OAGB) is recognized as a standard procedure in metabolic surgery. However, concerns about postoperative bile reflux and nutritional risks are prevalent. Comparatively, sleeve gastrectomy with loop duodenojejunal bypass (SG + LoopDJB) bypasses an equivalent length of the foregut as OAGB while maintaining pyloric function. The role of pylorus function remains to be further elucidated regarding these metabolic procedures' therapeutic outcomes and side effects. METHOD: A retrospective study was conducted in our center to compare the surgical safety and …

BACKGROUND: Billroth-I (BI) is a simple, physiological method of reconstruction following distal gastrectomy. In actuality, postoperative QO … at 12 months or later. CONCLUSIONS: BJI is as safe as BI, but is better in terms of improvement in bile reflux and food intake without stasis. This procedure, therefore, appears to be a useful method for reconstruction after distal gastrectomy.

Usefulness of pyloric reconstruction without compromising curative resection in gastric cancer treatment.

INTRODUCTION: We herein report the short-term results of the newly developed modified technique of Billroth I (modified B-I; pylorus reconstruction) that prevents duodenogastric reflux (DGR) and remnant gastritis after distal gastrectomy. PATIENTS AND METHODS: Distal gastrectomy with this technique was performed in 20 patients (age, 41 to 86 years [mean, 68.5 ± 11.8 years], male/female = 12:8) with gastric cancer from June 2006 through December 2009. These patients were compared with another 20 patients who underwent conventional B-I after distal gastrectomy (age, 41 to 85 years [mean, 69.3 ±

When the "Y" Becomes an "O": A Narrative Review of Roux-en-O Complication After Roux-en-Y Gastric Bypass-Incidence, Diagnosis, Management, and Prevention.

Roux-en-O misconfiguration is a rare but serious technical error in Roux-en-Y gastric bypass (RYGB), resulting from the misidentification of small bowel limbs in which the biliopancreatic limb (BPL) is incorrectly anastomosed to the gastric pouch and the jejunojejunostomy (JJ) is misconfigured, leading to functional obstruction, bile reflux, and substantial morbidity. This narrative review synthesizes the available literature on its incidence, risk factors, clinical presentation, diagnostic challenges, differential diagnosis, treatment strategies, associated morbidity, and prevention methods. …

Esophageal visceral sensitivity to bile salts in patients with functional heartburn and in healthy control subjects.

Patients with nonerosive gastroesophageal reflux disease often have relatively low esophageal acid exposure and respond suboptimally to gastric acid suppression. In these patients, other constituents of gastric contents may induce esophageal symptoms. We have demonstrated that gastric contents can cause heartburn when the gastric pH >4. (Aliment Pharm Ther 14:129-134, 2000). The aim of this study was to determine relative sensitivities to chenodeoxycholic and ursodeoxycholic acids, and 0.1 N HCl, administered as provocative perfusion tests. Patients with functional heartburn and healthy contro

Cellular lineage origins of spasmolytic polypeptide-expressing metaplasia (SPEM): persistent and intensifying debates.

Spasmolytic Polypeptide-Expressing Metaplasia (SPEM) is a gastric fundic gland metaplasia resembling deep antral glands, associated with drug injury, Helicobacter pylori (H. pylori), or bile reflux. Early-stage SPEM acts as a reparative response, but if the damaging stimuli persist, the metaplastic changes may become irreversible, raising the risk of gastric cancer development. Traditionally, SPEM arises via passive transdifferentiation of chief cells following parietal cell loss. However, recent lineage tracing and genetic models challenge this, suggesting active depletion of chief cells and …

Review of the abdominal manifestations of cystic fibrosis in the adult patient.

Cystic fibrosis is a common inherited fatal disease. As the life expectancy of affected individuals continues to increase with advances in disease management, this disease is no longer limited to the pediatric population. Currently, 40% of patients with cystic fibrosis are adults. In addition, patients may not present until adulthood and frequently have extrapulmonary symptoms. Abdominal manifestations are common and affect multiple organ systems. Hepatobiliary manifestations include fatty infiltration of the liver, gallbladder abnormalities, bile duct abnormalities, focal biliary fibrosis, an

[Treatment of obstruction of extrahepatic biliary pathways by antireflux, bilio-cholecystic (cystico) enteroanastomosis methods].

The main target of our investigations was the development of antireflux, bilio-cholecysto (cystico) enteral anastomosis method for the surgical treatment of extrahepatic biliary duct obstruction. We were using the cholecystic duct spiral valve (Lutkens valve, Haister valve) as the antireflux structure. In this case the bile drains into the biliary cyst not through the cholecystic duct but through the extrahepatic biliary duct - the bilio-cholecystic anastomosis. Such an interposition of cholecystic and spiral valves promotes inhibition of chemical reflux between extrahepatic ducts and gastroin

Endoscopic Diagnosis of Remnant Gastric Cancer After Gastrectomy.

Remnant gastric cancer (RGC) can develop in patients who have undergone curative resection for gastric cancer and in those who have undergone gastrectomies for benign disorders such as peptic ulcer disease. Routine endoscopic surveillance is recommended for early detection of cancer at the anastomosis site or within the remnant stomach. However, endoscopy in postgastrectomy patients may be challenging because of the limited space of the remnant stomach, deformity at the anastomosis site, interference from retained food material, and inflammation associated with recurrent bile reflux. The inter…

Drug- and Drug Abuse-Associated Hyperbilirubinemia: Experience With Atazanavir.

Hyperbilirubinemia is a common finding in individuals with a history of substance abuse. Although this may indicate a serious disorder of liver function, this is not always the case. An understanding of bilirubin formation, metabolism, and transport can provide a helpful approach to dealing with these patients. This is typified by studies of patients treated with the antiretroviral drug atazanavir. Atazanavir has been associated with hyperbilirubinemia in as many as one-third of individuals for whom it has been prescribed, evoking concerns of hepatotoxicity. The studies in this report were des

The effect of ethanol on the pancreatic duct sphincter of Oddi.

Alcohol ingestion causes pancreatitis to develop in some people, but the mechanism(s) by which alcohol causes pancreatitis are unclear. The effect of alcohol on the pancreatic duct sphincter of Oddi (PD-SO) may play a significant role. To better understand the effects of alcohol on the PD-SO, we studied 24 alcoholic subject's PD-SO before and after 80 g of alcohol was instilled into their stomachs. Seventeen of the subjects had a prior history of pancreatitis. The mean maximal alcohol level achieved by 30 min after the instillation was 98.1 +/- 47.9 mg/dl. The only prealcohol differences betwe
