Multimodal physical therapy versus topical lidocaine for provoked vestibulodynia: a multicenter, randomized trial.

BACKGROUND: Provoked vestibulodynia is the most common subtype of chronic vulvar pain. This highly prevalent and debilitating condition is characterized by acute recurrent pain located at the entry of the vagina in response to pressure application or attempted vaginal penetration. Although physical therapy is advocated as a first-line treatment for provoked vestibulodynia, evidence supporting its efficacy is scarce. OBJECTIVE: The purpose of this study was to establish the efficacy of multimodal physical therapy compared with topical lidocaine, a frequently used first-line treatment. STUDY D

Effects of biofeedback on muscular tension in selected personality states.

The effects of electromyographic biofeedback on three selected personality states from the MMPI and one comparison group were investigated. Forty in-patients (28 males and 12 females) ranging in age from 15 to 52 in a psychiatric hospital were classified as manic, agitated, depressed, or designated as a comparison group, according to their MMPI scores and Gilberstadt and Duker's assessment of personality. Fourteen sessions of electromyographic biofeedback were given each individual. Significant differences in EMG readings were found between the manic group and the the comparison group, the agi

Review article: advances in the diagnosis and management of anorectal motility disorders.

BACKGROUND: Anorectal motility disorders such as dyssynergic defecation (DD), faecal incontinence (FI) and anorectal pain affect 40% of the population and are a frequent reason for gastroenterology consultation. They significantly affect the quality of life and lead to psychological distress. Lack of understanding of these problems compounded by a lack of availability and knowledge of diagnostic tools in most medical centres and/or trained physicians has significantly hampered this field. AIMS: To discuss the latest advances in pathophysiology, diagnostic tests and therapeutic options for the

[The occurrence, precaution and treatment strategies of postoperative fecal incontinence in rectal and anal diseases].

In the surgical treatment of hemorrhoids, rectal prolapse, rectal cancer, anal fissures, or anal fistulas, inadvertent damage to the nerves or muscles responsible for bowel control may potentially lead to varying degrees of fecal incontinence (FI). Surgeons need to conduct preoperative assessments based on the patient's individual condition to select an appropriate surgical plan, aiming to minimize the incidence of postoperative FI and improve the patient's postoperative quality of life as much as possible while effectively treating the disease. Additionally, the proficiency of the surgeon's s

Functional anorectal disorders.

This report defines criteria for diagnosing functional anorectal disorders (ie, fecal incontinence, anorectal pain, and disorders of defecation). Functional fecal incontinence is defined as the uncontrolled passage of fecal material recurring for > or =3 months in an individual with a developmental age of > or =4 years that is associated with: (1) abnormal functioning of normally innervated and structurally intact muscles, and/or (2) no or minor abnormalities of sphincter structure and/or innervation insufficient to explain fecal incontinence, and/or (3) normal or disordered bowel habits (ie,

A Digital Therapeutic Pelvic Health System Improves Fecal Incontinence Symptoms in a Real-World Patient Cohort.

BACKGROUND: Fecal incontinence is a common yet underdiagnosed condition, disproportionately affecting women and significantly impacting health and quality of life. Pelvic floor muscle training and biofeedback are recommended first-line treatments, yet access, adherence, and stigma remain substantial barriers to care. Digital therapies may offer an effective alternative to address barriers. OBJECTIVE: This study aimed to evaluate real-world outcomes with the use of a prescription digital therapeutic device for the treatment of fecal incontinence in women. DESIGN: This observational cohort stu

Physiotherapist-led ultrasound-guided visual biofeedback prelabor training: a randomised controlled trial.

BACKGROUND: Prenatal education improves maternal and neonatal outcomes. Ultrasound-guided visual biofeedback provides real-time visualization of fetal head movement, enabling expectant mothers to refine their pushing techniques. Pelvic floor muscle training may improve the ability to contract and relax the pelvic floor during labor. When used during the second stage of labor, US-BF has been shown to confer obstetrical benefits. Implementing prelabor ultrasound-guided visual biofeedback can further improve motor learning and optimize pushing effectiveness during the second stage of labor. This

Gamified electromyographic biofeedback for people with tetraplegia from SCI is enjoyable, results in high-dosage therapy, increases muscle electrical activity, and may improve arm function, a pilot study.

BACKGROUND: Individuals with tetraplegia due to spinal cord injury (SCI) place a high priority on improving arm function for their independence and quality of life. Animal studies have shown a therapeutic benefit of much higher therapeutic dosages than obtained in clinical practice. Electromyographic (EMG) biofeedback is validated in people with SCI and can produce high dosage therapy. The goal of this pilot study is to test if gamified electromyographic (EMG) biofeedback can address this need by quantifying (1) participant enjoyment, (2) dosage of muscle activations, (3) increases in muscle a

Modulating cortical excitability and cortical arousal by pupil self-regulation.

The brain's arousal state (i.e., central arousal) is regulated by multiple neuromodulatory nuclei in the brainstem and significantly influences high-level cognitive processes. By exploiting the mechanistic connection between the locus coeruleus, a key regulator of central arousal, and pupil dynamics, we recently demonstrated that participants could gain volitional control over arousal-regulating centers including the locus coeruleus using a pupil-based biofeedback approach. Here, we test whether pupil-based biofeedback modulates electrophysiological markers of cortical excitability, cortical a

Effect of Systematic Effortful Swallowing Exercise on the Activation Level of the Submental Muscles and Tongue Strength in Older Adults.

INTRODUCTION: Effortful swallowing (ES) is known to significantly increase the activity of the submental muscles, including the suprahyoid muscles, which play an important role in elevating the hyoid bone and larynx during swallowing and contributing to the opening of the upper esophageal sphincter. However, inconsistent results have been reported regarding the long-term effects of ES exercise in increasing the submental muscle activity. Therefore, this study aimed to investigate the effect of a 6-week systematic ES exercise program on the activation level of the submental muscle and tongue st

Evaluating the Benefits of Pelvic Floor Muscles Exercises Combined With Biofeedback Therapy for Improving Functional Urinary Incontinence in Children.

BACKGROUND: Pelvic floor muscles training (PFMT), with or without biofeedback (BF), is widely utilized as an alternative treatment for various refractory lower urinary tract dysfunctions in adults and children. This study aimed to compare the efficacy of PFMT alone versus in combination with BF on functional urinary incontinence (UI) in children. PATIENTS AND METHODS: Medical records of children who had undergone pelvic floor rehabilitation for non-neuropathic intermittent UI from 2018 to 2022 were retrieved. Patients were categorized based on their treatment regimen. Group I had undergone st

Nonpharmacological treatments for Tourette syndrome and tic disorders: A protocol for systematic review and network meta-analysis.

BACKGROUND: A tic is a sudden, rapid, recurrent, nonrhythmic motor movement, or vocalization. Tic disorders are diagnosed based on the presence of motor or vocal tics, duration of tic symptoms, and age at onset. Current clinical practice guidelines strongly recommend behavioral therapies because they are more effective and safer than medications. To determine the most effective nonpharmacological intervention for tic disorders and Tourette syndrome, we will conduct a systematic review and network meta-analysis. METHODS: We will search the Cochrane Central Register of Controlled Trials, MEDLIN

A Meta-analysis of Brief Personalized Feedback Interventions for Problematic Gambling.

Personal Feedback Interventions (PFIs) have been widely used to reduce the amount of time and money individuals spend on gambling. A central component of these interventions is personalized information about an individual's gambling behavior, often in comparison to others' gambling. The purpose of the present review and meta-analysis was to evaluate these interventions in terms of content, mode of delivery, target sample, and efficacy. Sixteen interventions from 11 studies were reviewed. We found a small, statistically significant effect in favor of PFIs versus control (d = 0.20, 95% CI 0.12,
