Transfusion-associated circulatory overload in a pediatric patient with neuroblastoma.

There have been few reports on pediatric transfusion-associated circulatory overload (TACO). A 5-year-old boy with neuroblastoma underwent resection of the residual tumor. Because anemia progressed at the end of the operation, transfusion of red cell component was initiated. Ten minutes later, he suddenly developed hypoxemia, tachypnea, and tachycardia. Although elevated blood pressure and bilateral infiltrative shadows on chest X-rays were not observed, TACO was diagnosed based on positive balance during operation and N-terminal pro-brain natriuretic peptide elevation. He had no cardiac or re

Association of Donor Age and Sex With Survival of Patients Receiving Transfusions.

IMPORTANCE: Following animal model data indicating the possible rejuvenating effects of blood from young donors, there have been at least 2 observational studies conducted with humans that have investigated whether donor age affects patient outcomes. Results, however, have been conflicting. OBJECTIVE: To study the association of donor age and sex with survival of patients receiving transfusions. DESIGN, SETTING, AND PARTICIPANTS: A retrospective cohort study based on the Scandinavian Donations and Transfusions database, with nationwide data, was conducted for all patients from Sweden and Den

Next generation spray-dried on-demand plasma: A multicenter, randomized, dose-escalation, first-in-human safety study.

BACKGROUND: Prehospital plasma transfusion for patients at risk of hemorrhagic shock reduces mortality, but cold-chain and logistical constraints limit product availability. This study reports the first-in-human clinical evaluation of a new spray-dried plasma product, Frontline On-Demand Plasma (ODP). METHODS: Twenty-four healthy volunteers were sequentially enrolled into three dose-escalation cohorts. Autologous apheresis plasma was processed into plasma frozen within 24 h (PF24), with half of each collection manufactured into ODP. Cohort 1 (n = 6) received 200 mL ODP; Cohort 2 (n = 6) recei

Prehospital Plasma Is Not Associated With an Increase in Orthopedic Trauma Patients With Hemorrhagic Shock That Meet Early Appropriate Care Criteria.

INTRODUCTION: The Prehospital Air Medical Plasma trial demonstrated that prehospital helicopter administration of thawed plasma versus standard of care (SOC) resuscitation reduced 30-d mortality in patients with hemorrhagic shock. There is no current analysis of patients who underwent orthopedic interventions. We aimed to determine if prehospital administration of plasma is associated with an increased proportion of patients that meet early appropriate care (EAC) criteria. MATERIALS AND METHODS: This was a secondary analysis of the subgroup of patients from the Prehospital Air Medical Plasma

Use of plasma late on cardiopulmonary bypass in patients undergoing left ventricular assist device implantation.

Coagulopathy is common during left ventricular assist device (LVAD) implantation, treatment of which can be challenging given the often-limited ability for the right ventricle to accommodate volume transfusion after device initiation with 20% to 40% of patients developing right ventricular failure (RVF). Transfusion of plasma late on cardiopulmonary bypass (CPB) combined with ultrafiltration may replace clotting factors while reducing volume administration. We compared outcomes in patients undergoing LVAD implantation receiving plasma on CPB and ultrafiltration with traditional transfusion pra

Risk Factors for Invasive Candida Infection in Critically Ill Patients: A Systematic Review and Meta-analysis.

BACKGROUND: Current guidelines recommend empirical antifungal therapy in patients with sepsis with high risk of invasive Candida infection. However, many different risk factors have been derived from multiple studies. These risk factors lack specificity, and broad application would render most ICU patients eligible for empirical antifungal therapy. RESEARCH QUESTION: What risk factors for invasive Candida infection can be identified by a systematic review and meta-analysis? STUDY DESIGN AND METHODS: We searched PubMed, Web of Science, ScienceDirect, Biomed Central, and Cochrane and extracted

Treatment of combined traumatic brain injury and hemorrhagic shock with fractionated blood products versus fresh whole blood in a rat model.

INTRODUCTION: Treatment of combined traumatic brain injury and hemorrhagic shock, poses a particular challenge due to the possible conflicting consequences. While restoring diminished volume is the treatment goal for hypovolemia, maintaining adequate cerebral perfusion pressure and avoidance of secondary damage remains a treatment goal for the injured brain. Various treatment modalities have been proposed, but the optimal resuscitation fluid and goals have not yet been clearly defined. A growing body of evidence suggests that in hypovolemic shock, resuscitation with fresh whole blood (FWB) may

Treatment of plasminogen deficiency patients with fresh frozen plasma.

Congenital plasminogen (Plg) deficiency leads to the development of ligneous membranes on mucosal surfaces. Here, we report our experience with local and intravenous fresh frozen plasma (FFP). We retrospectively reviewed medical files of 17 patients and their eight first-degree relatives. Conjunctivitis was the main complaint. Thirteen patients were treated both with intravenous and conjunctival FFP. Venous thrombosis did not develop in any. Genetic evaluation revealed heterogeneous mutations as well as polymorphisms. Diagnosis and treatment of Plg deficiency is challenging; topical and intrav

Addition of oral iron to plasma transfusion in human congenital hypotransferrinemia: A 10-year observational follow-up with the effects on hematological parameters and growth.

Congenital hypotransferrinemia (OMIM 209300) is an extremely rare disorder of inherited iron metabolism. Since its description in 1961, only 16 cases have been reported. The defective gene and molecular defect causing this disorder and clinicolaboratory findings seen in the homozygous and heterozygous states have been documented in both humans and mice. However, due to the lack of follow-up studies of the described cases, the long-term prognosis remains unknown. We present a 10-year observational follow-up of a patient previously diagnosed on a molecular basis who was treated with a unique the

Successful treatment of avian-origin influenza A (H7N9) infection using convalescent plasma.

In January 2015, there was an outbreak of avian-origin influenza A (H7N9) virus in Zhejiang Province, China. A 45-year-old man was admitted to the First Affiliated Hospital of Zhejiang University with a high fever that had lasted 7 days, chills, and a cough with yellow sputum. Laboratory testing confirmed infection with the H7N9 virus, likely obtained from contact with poultry at a local live poultry market. A large dense shadow was apparent in the patient's left lung at the time of admission. Treatment with oseltamivir (75mg twice daily) did not improve the patient's condition. The decision w

[Conservative treatment of chronic tendinopathies].

The majority of insertional and noninsertional tendinopathy cases are associated with repetitive or overuse injuries. Certain tendons are particularly vulnerable to degenerative pathology; these include the Achilles and patella tendon, the rotator cuff, and forearm extensors/flexors. Disorders of these tendons are often chronic and can be difficult to manage successfully in the long term. Eccentric exercise has the strongest evidence of therapeutic efficacy. Extracorporeal shock wave treatment, sclerosing agents as well as nitric oxide patches show promising early results but require long-term

Fatal complication from central venous cannulation in a paediatric liver transplant patient.

We report a fatal complication from central venous cannulation in a child undergoing heterotropic liver transplantation. Following the attempted placement of a cannula in the left internal jugular vein, extravasation of blood products via the cannula resulted in haemothorax, hypotension and eventual brain death. Possible causes and strategies for prevention of this complication are discussed.

Clinical and molecular remission after allogeneic blood cell transplantation in a patient with mantle-cell lymphoma.

Mantle-cell lymphoma (MCL) is a B-cell tumour with a usually poor prognosis, characterized by the proliferation of small cleaved lymphocytes with a diffuse growth pattern. We report a polymerase chain reaction-based analysis of minimal residual disease in a patient who achieved complete remission after allogeneic blood cell transplantation (BCT). Rearrangement of the immunoglobulin heavy-chain genes was used to generate a lymphoma-specific molecular marker. Lymphoma cells were not detectable in a bone marrow sample collected 12 months after BCT. Our findings suggest that allogeneic BCT may off

Transfusion-transmitted cytomegalovirus infection in the era of universal leukoreduction and pathogen inactivation: a contemporary review.

INTRODUCTION: Transfusion-transmitted cytomegalovirus remains a clinically significant risk in immunocompromised individuals, neonates, and transplant recipients. Despite widespread universal leukoreduction and pathogen-reduction technologies, uncertainty persists regarding residual risk and the role of CMV-seronegative blood components. This review summarizes the epidemiology, diagnostics, and prevention of transfusion-transmitted CMV. METHODS: A review of the international literature was conducted using PubMed, Scopus, and transfusion medicine databases. Studies on CMV seroprevalence, trans

Changes and factors influencing health-related quality of life after hysterectomy in premenopausal women with benign gynecologic conditions.

BACKGROUND/PURPOSE: A hysterectomy affects a woman's health. This study was performed to identify the factors that affect health-related quality of life (HRQoL) before and after hysterectomy in premenopausal women. METHODS: This prospective follow-up study recruited 38 women (age range, 33-52 years) who underwent abdominal hysterectomy for nonmalignant causes. SF-36 and self-rated health status were used to assess HRQoL before and after hysterectomy. Data were analyzed using descriptive statistics, nonparametric tests and the generalized estimating equation method for modeling the repeatedly

Emerging viral infections‑-a potential threat for blood supply in the 21st century.

During the last 25 years the safety of blood products has improved dramatically with regard to infectious risk, notably to the threat represented by retroviruses (HIV and human T‑cell lymphotropic virus) and hepatitis B and C viruses. However, both residual and emergent viral infections are still responsible for contaminations in recipients of blood products. Along with other viruses (human herpesvirus‑8, human parvovirus B19, hepatitis A and E viruses, etc.), special attention has recently been paid to emerging arboviruses, such as West Nile virus in North America, and Dengue and Chikungunya

INTERNATIONAL NORMALIZED RATIO AND ACTIVATED PARTIAL THROMBOPLASTIN TIME DO NOT PREDICT PLASMA TRANSFUSION IN LIVER TRANSPLANTATION.

BACKGROUND: Blood loss during liver transplantation (LT) remains a major concern associated with increased morbidity and reduced patient and graft survival. The high complexity of the procedure associated with the multifaceted origin of the bleeding urges early identification of high-risk patients and proper monitoring of hemostasis disorders in order to improve results. The accuracy of international normalized ratio (INR) and activated partial thromboplastin time (aPTT) to evaluate coagulation status in cirrhotic patients has been doubted. AIMS: The aim of this study was to investigate the a

High-dimensional analysis of injured patients reveals distinct circulating proteomic profiles in plasma vs. whole blood resuscitation.

Early blood product resuscitation is often essential for optimal trauma care. However, the effects of different products on the underlying trauma-induced coagulopathy and immune dysfunction are not well described. Here, we use high-dimensional analysis and causal modeling in a longitudinal study to explore the circulating proteomic response to plasma as a distinct component versus low-titer O whole blood (LTOWB), which contains plasma. We highlight the differential impacts of plasma and LTOWB on immune mediator levels and the distinct capacity of plasma to modulate coagulation by elevating fib
