As allogeneic blood transfusion (ABT) is not harmless, multiple alternatives to TSA (AABT) have emerged, but there is a huge variability with respect to their indications and appropriate use. This variability results from the interplay of a number of factors, which include physicians specialty, knowledge and preferences, degree of anaemia, transfusion policy, and AABT availability. Since the ABBT are not harmless and may not meet costeffectiveness criteria, such avariability is unacceptable. BACKGROUND: Blood transfusion in the management of severely injured patients can be lifesaving. These patients are susceptible to developing early coagulopathy, thus perpetuating bleeding.
OBJECTIVES: This article presents recent advances in both the civilian and military clinical arena to improve the treatment of trauma patients with severe hemorrhage, the use of agents to support coagulation, perspectives on restrictive transfusion strategies, and transfusion-related risks.
DISCUSSION: Massive blood transfusion is an adjunct to surgical care. The volume of blood products transfused and the

Basic values of blood circulation and respiration were compared in two groups of cats. The blood of group I animals was replaced by a 8% solution of human Hb completely free of stroma and procoaglulant activity. In group II animals the blood was diluted with polyglucine to the Hb level of 8 g%. The solution is well saturated in the lungs and releases O2 to tissues, but much less than Hb contained in red cells. The dissolved Hb did not completely meet the requirements of the organism in oxygen with the resultant hypoxia. Moderate anemic hypoxia caused by blood dilution with polyglucine was easi

BACKGROUND: The ideal resuscitation fluid for the trauma patient would be readily available to prehospital personnel, universally compatible, effective when given in small volumes, and capable of reversing tissue hypoxia in critical organ beds. Recently developed hemoglobin-based oxygen-carrying solutions possess many of these properties, but their ability to restore tissue oxygen after hemorrhagic shock has not been established. We postulated that a small-volume resuscitation with HBOC-201 (Biopure) would be more effective than either lactated Ringer's (LR) solution or hypertonic saline dextr

The development of a low-flow vascular prosthesis is a very topical issue. The authors present a pathway for the development of a prosthesis with optimal properties based on the idea of mimicking the characteristics of a biological model (saphenous vein graft) and programming these properties in the model of the prosthetic substitute. The vascular prosthesis presented consists of three layers - a non-absorbable scaffold representing vascular “media”, and two absorbable collagen layers - pseudointima and pseudoadventitia. The basic methods of physical testing are presented - the sin

OBJECTIVE: To assess the effects of in vitro hemodilution of canine blood with a low molecular weight hy-droxyethyl starch (HES 130/0.4) by means of thromboelastometry (TEM).
DESIGN: In vitro experimental study.
SETTING: University Teaching Hospital.
ANIMALS: Ten healthy adult staff-owned dogs were sampled for the purposes of the study. Samples were also collected from 45 clinically normal dogs to establish thromboelastometric reference intervals.
INTERVENTIONS: For each dog whole blood was collected by jugular venipuncture and placed into tubes containing 3.8% trisodium citrate and stored

A phospholipid vesicle encapsulating hemoglobin (Hb vesicle, HbV) has been developed to provide O(2)-carrying capacity to plasma expanders. Its ability to restore systemic and microcirculatory conditions after hemorrhagic shock was evaluated in the dorsal skinfold window preparation of conscious hamsters. The HbV was suspended in 8% human serum albumin (HSA) at Hb concentrations of 3.8 g/dl [HbV(3.8)/HSA] and 7.6 g/dl [HbV(7.6)/HSA]. Shock was induced by 50% blood withdrawal, and mean arterial pressure (MAP) at 40 mmHg was maintained for 1 h by the additional blood withdrawal. The hamsters rec

Methods and techniques of preoperative isovolaemic haemodilution in a district hospital are described. In 15 of 19 patients the retransfusion of autologous blood was adequate to compensate the intraoperative loss of blood after isovolaemic haemodilution. Fatalities of serious incidents were not observed. The performance of isovolaemic haemodilution presupposes the necessity of sufficient staff and technical equipment.
