Ultrasound nail imaging on patients with psoriasis and psoriatic arthritis compared with rheumatoid arthritis and control subjects.

OBJECTIVE: The objective of this study was too show findings at finger nails level revealed by high-frequency gray-scale ultrasound (US) and power Doppler in patients with psoriatic arthritis (PsA),and cutaneous psoriasis compared with rheumatoid arthritis and control subjects. METHODS: We studied 35 patients with PsA, 20 with cutaneous psoriasis, and control groups (28 control subjects and 27 patients with rheumatoid arthritis). All nails of both hands were observed (1097 nails, 3 excluded because of trauma). In every patient, we classified the morphologic abnormalities disclosed in ventral

Total anonychia congenita: a rare heterogeneic disorder.

Total anonychia congenita is a very rare disorder in which all the finger nails and the toenails are absent without significant bone anomalies. This condition is reported to have an autosomal dominant inheritance pattern. There are a variety of syndromes or conditions associated with anonychia congenita. In this study we present a monozygotic twin with isolated anonychia congenita totals. Interestingly there is no family history of anonychia and this condition seems to be caused by a sporadic mutation.

[Fixation of fractures of the fifth metacarpal (author's transl)].

The authors describe the treatment which they recommend for fractures of the fifth metacarpal. After a brief description of fractures of the proximal end of the diaphysis and of the head of the bone, they study in detail fractures of the neck of the fifth metacarpal. They found that conservative treatment is not very successful but that internal fixation tends to have iatrogenic complications. They recommend a transverse pinning of the bone after reduction by manual traction. Two or three pins are driven from the fifth to the fourth metacarpal through the head and the shaft. This treatment all

Acro-spondylo-pubic dysostosis associated with cataracts, microcephaly, and normal intelligence.

We report on an adult male with normal intelligence who exhibited an unusual combination of microcephaly, dysostoses of limbs, vertebrae, patellae, and pubic bone, camptodactyly of all fingers, and syndactyly of toes, absent nails on thumbs and some fingers, bilateral cataract, cryptorchidism, polythelia, and nipple-like skin pigmentations of shoulders and upper back. We have been unable to find a description of a similar combination of manifestations in literature. The cause of the anomalies remains unknown.

Squamous Cell Carcinoma of Finger Invading Middle Phalanx: Surgical Considerations.

Squamous cell carcinoma (SCC) of the nail unit is a rare but highly treatable malignancy. A tissue biopsy is recommended to establish the diagnosis and guide treatment. Mohs surgery is the standard of care to preserve tissue and function for early SCC. The authors report a 76-year-old patient with a very advanced and neglected SCC of the nail unit that grossly extended to the middle phalanx. No prior reports of such an extensive growth have been documented. Due to extensive skin and bone involvement, we proceeded with amputation at the level of the proximal phalanx to obtain negative margins.

Management of large segmental tibial defects using locking IM nail and absorbable mesh.

Tibia accounts for the majority of open fractures with long segmental defects. The management of such defects imposes several challenges due to their associations with extensive soft tissue damage and high infection risk. In this report, we describe a new technique for the reconstruction of tibia in a young patient with 10 cm defect. The patient had a history of Gustilo Anderson type IIIB open fracture following a motorcycle accident. He was initially managed with multiple debridements and application of an external fixator. The defect was treated with the insertion of an antibiotic-coated int

Fat release after femur nailing in the dog.

Internal fixation of fractures in the first few hours following injury has important advantages. It can diminish continued blood loss, improve patient mobility, and may help avoid pulmonary and cardiovascular complications of multiple trauma. Fear of increased risk of fat embolism has limited its use. We studied the release of fat during intramedullary reaming and nailing of the femur in dogs. Comparing fat release from an intact and a fractured femur in each dog, significantly more triglyceride was released from the intact bone than from the fractured femur. Thus, reaming a fractured bone pro

Accordion Maneuver for Delayed Regenerate Formation Following Pediatric Osteosarcoma Resection: A Case Report and Literature Review.

Background and Clinical Significance: Reconstruction of large segmental bone defects following oncologic resection in pediatric patients remains a major challenge. Although distraction osteogenesis with bone transport is a well-established biological reconstructive option, regenerate formation may be compromised in patients receiving chemotherapy. The accordion maneuver, consisting of alternating cycles of compression and distraction, has been described as a method to stimulate bone regeneration, primarily in association with external fixation systems. However, its use in internal bone transpo

Diffuse reflectance spectroscopy-enhanced drill for bone boundary detection.

Intramedullary nailing is a routine orthopedic procedure used for treating fractures of femoral or tibial shafts. A critical part of this procedure involves the drilling of pilot holes in both ends of the bone for the placement of the screws that will secure the IM rod to sections of the fractured bone. This step introduces a risk of soft tissue damage because the drill bit, if not stopped in time, can transverse the bone-tissue boundary into the overlying muscle, causing unnecessary injury and prolonging healing time due to periosteum damage. In this respect, detecting the bone-tissue boundar

Variability of human femoral geometry and its implications on nail design.

OBJECTIVES: The objectives of this study are to present a new method of quantifying variability in human femoral geometry and to use this data to optimize intramedullary nail geometry for a better fit within the femoral canal. METHODS: Computed tomography (CT) scans of forty intact adult human femora were divided according to race, sex, and age, and were reconstructed digitally into three-dimensional solid models. Geometric features were then measured and compared among groups using ANOVA. An average geometric model was generated, from which an optimal intramedullary (IM) nail curvature funct

Conservative treatment for refracture of the forearm with elastic stable intramedullary nails in situ in an adolescent: A case report.

Elastic stable intramedullary nailing (ESIN) for forearm shaft fractures in children and adolescents is generally associated with favorable outcomes. However, refracture with ESINs in situ is a rare complication. We report a rare case of refracture with implants in situ in a 14-year-old male patient. Six months after the initial surgery, the patient sustained a reinjury while playing soccer. Radiographs confirmed forearm shaft refracture of both the radius and ulna, and both intramedullary nails were bent at the fracture sites but remained unbroken. We performed emergent closed manual reductio

Principles of Fracture Healing and Fixation: A Literature Review.

Bone healing is a complex, dynamic process involving a series of well-coordinated stages, influenced by both mechanical and biological factors. The skeletal system, composed of inorganic (36%), organic (36%), and water (28%) components by volume, plays a crucial role in maintaining structural integrity and mineral homeostasis. Bone is classified into two main types based on microstructure: lamellar and woven bone, with lamellar bone being stronger and more durable. Factors such as inflammation, the periosteum, vascularity, and infection significantly impact healing outcomes. Moreover, fracture

Bilateral femoral shaft fractures in osteomalacia: A case report and literature review.

INTRODUCTION AND IMPORTANCE: When two non-traumatic femoral shaft fractures happen at the same time, they are usually not caused by high-energy trauma or medical conditions like osteomalacia caused by poor nutrition, long-term bisphosphonate medication use, or steroid use. Because this phenomenon is connected to inadequate bone mineralization and bone metabolism, it occurs when a bone is consistently under high stress. CASE PRESENTATION: A 40-year-old lady with a medical history of osteomalacia, including continuous vitamin D replacement therapy, showed up at the emergency room complaining of

Posterolateral plate fixation with Pantalarlock® is more stable than nail fixation in tibiotalocalcaneal arthrodesis in a biomechanical cadaver study.

BACKGROUND: Despite the promising results of ankle joint arthroplasty, the tibiotalocalcaneal (TTC) arthrodesis remains an established procedure in treatment of combined pathology of the ankle and subtalar joint. Despite the promising results in biomechanical investigations, nonunion rates of up to 24% are described in recent studies. The objective of this work was a comparative study of the biomechanical properties of the posterolateral plate fixation with retrograde intramedullary nail fixation. METHODS: Twenty four fresh-frozen human lower leg specimens (12 pairs) were used for the compara

Bone properties and biomechanics in patients with psoriatic disease: a cross-sectional study with high-resolution peripheral quantitative CT (HRpQCT).

BACKGROUND: Psoriatic disease has a complex effect on bone metabolism, resulting in both pathological bone formation and bone resorption. However, microstructural changes in cortical and trabecular compartments remain poorly understood. The aim of this study was to investigate the prevalence and determinants of bone microarchitectural damage in patients with psoriatic disease. METHODS: We performed a cross-sectional study in patients with psoriasis (PsO), psoriatic arthritis (PsA) and age-matched healthy controls (HCs) recruited into the Department of Dermatology and Rheumatology of Verona ce
