Locking compression plates for osteoporotic nonunions of the diaphyseal humerus.

Poor bone quality increases the technical difficulty and complications of operative treatment of nonunions and delayed unions of the diaphyseal humerus in older patients. Plates with screws that lock to the plate (transforming each screw into a fixed blade) are intended to improve the fixation of poor quality bone. Twenty-four patients (20 women, four men) with an average age of 72 years (range, 52-86 years) were followed up for a minimum of 12 months after locking compression plate fixation of an osteopenic delayed union (nine patients) or nonunion (15 patients) of the diaphyseal humerus. Double plating is associated with higher fixation strength than single plating in osteoporotic fractures of the scapular spine: a biomechanical study.

INTRODUCTION: The number of atraumatic stress fractures of the scapular spine associated with reverse shoulder arthroplasty is increasing. At present, there is no consensus regarding the optimal treatment strategy. Due to the already weakened bone, fractures of the scapular spine require a high fixation stability. Higher fixation strength may be achieved by double plating. The aim of this study was to evaluate the biomechanical principles of double plating in comparison to single plating for scapular spine fractures. METHODS: In this study, eight pairs (n = 16) of human shoulders were randomi

Finite-element stress analysis of the normal and osteoporotic lumbar vertebral body.

A finite-element model of an isolated elderly human L3 vertebral body was developed to study how material properties and loading conditions influence end-plate and cortical-shell displacements and stresses. The model consisted of an idealized geometric representation of an isolated vertebral body, with a 1-mm-thick end plate and cortical shell. For uniform compression, large tensile stresses occurred all around the cortical shell just below the end plate as a result of bending of the cortical shell as it supported the end plate. Large tensile bending stresses also developed in the inferior sur

Biomechanical effect of different plate-to-disc distance on surgical and adjacent segment in anterior cervical discectomy and fusion - a finite element analysis.

BACKGROUND: The plate-to-disc distance (PDD) is an important factor affecting the degeneration of adjacent segments after anterior cervical discectomy and fusion (ACDF). However, the most suitable PDD is controversial. This study examined the adjacent intervertebral disc stress, bone graft stress, titanium plate stress and screw stress to evaluate the biomechanical effect of different PDD on surgical segment and adjacent segment following C5/C6 ACDF. METHODS: We constructed 10 preoperative finite element models of intact C4-C7 segments and validated them in the present study. We simulated ACD

Traumatic irreducible dislocation of the fifth metatarsophalangeal joint in pediatrics: case report and clinical experience.

Dislocation of the metatarsophalangeal joint (MTPJ) by trauma commonly occurs in adults. Most dislocations of the MTPJ could be reduced by closed reduction. However, isolated traumatic irreducible dislocation of the fifth MTPJ is an extremely rare injury, particularly in children. We report the case of a 10-year-old boy with irreducible dislocation of the fifth MTPJ who presented with a dorsiflexion injury of the right foot 1 year previously. Closed reduction was attempted but failed. Computed tomography showed the dorsolateral dislocation of the fifth MTPJ. We performed an open reduction and

Impact of substrate choice on human osteoclast differentiation and secretome: Implications for targeted therapeutic development.

Osteoclasts (OCs) exhibit substrate-specific molecular adaptations crucial for bone remodeling. We utilized mass spectrometry and functional enrichment analysis to delineate the proteomic profiles of mature polarized OCs cultured on mineralized versus plastic plates. Our findings reveal that mineralized surfaces promote the expression of proteins specialized for bone resorption and matrix interaction, such as lysosomal enzymes and ion transporters. This environment induces a mature and resorptive phenotype in OCs, enriched in pathways like VEGF/VEGFR signaling and various cytokine pathways. Co

[Treatment of pseudarthroses of the olecranon with the anatomical hook plate].

We report the results in seven patients treated surgically for nonunion of the olecranon. Four nonunions occurred after primary treatment with tension band wiring and three nonunions occurred after open reduction internal fixation with plates and screws or lag screws only. All patients were treated with cancellous bone grafting and open reduction internal fixation using anatomical hook plates. All nonunions united within 6 months after the treatment. In all patients the range of motion of the elbow improved significantly.

Treatment of comminuted metaphyseal distal femoral fractures with a micromotion-balancing osteosynthesis: an animal study.

BACKGROUND: Locking plates are commonly used in the treatment of comminuted metaphyseal distal femoral fractures. However, locking plates form a strong structure and promote asymmetrical callus formation, which is not conducive for rapid fracture healing and may increase fracture risk. To overcome this, we designed a micromotion-balancing fixation system based on locking plates. METHODS: Six healthy pigs (Bama miniature pigs) were used to establish a model of bilateral comminuted distal femoral fracture (AO/ASIF: 33-C2). Standard drilling was performed on one of each pig's hind limbs (control

Mechanobiologically-optimized non-resorbable artificial bone for patient-matched scaffold-guided bone regeneration.

Scaffold-guided bone regeneration is poised to revolutionize the management of critical-sized bone defects. However, translation into clinical practice has been hampered by the focus on bioresorbable scaffolds where the rate of degradation needs to match the rate of bone formation and metal plates are required to overcome their mechanical limitations. Metal plates are problematic because they cause stress shielding and X-ray perturbation, increasing the likelihood of hardware failure and interfering with post-operative radiotherapy and imaging. Segmental defects of the mandible are challenging

Extracellular matrix protein gelatin provides higher expansion, reduces size heterogeneity, and maintains cell stiffness in a long-term culture of mesenchymal stem cells.

Extracellular matrices (ECM) present in our tissues play a significant role in maintaining tissue homeostasis through various physical and chemical cues such as topology, stiffness, and secretion of biochemicals. They are known to influence the behavior of resident stem cells. It is also known that ECM type and coating density on cell culture plates strongly influence in vitro cellular behavior. However, the influence of ECM protein coating on long-term mesenchymal stem cell expansion has not been studied yet. To address this gap, we cultured bone-marrow derived hMSCs for multiple passages on

Bone Formation and Anti-Inflammatory Properties of Iodine-Loaded Titanium Implants: An Animal Study.

Titanium implants are subjected to various surface treatments to improve their in vivo function. In this study, we evaluated the usefulness of titanium implants treated with acid, NaOH, CaCl2, heat, and ICl3 (Ac-NaCaThIo) in terms of in vivo bone-bonding strength, bone formation, and histological anti-inflammatory properties. Iodine-loaded experimental dental implants and commercial control dental implants were placed in rabbit femurs, and bone-bonding strength was evaluated by measuring the implant stability quotient (ISQ), bone formation using tissue specimens, and the effect of iodine using

The impact of plate length, fibula integrity and plate placement on tibial shaft fixation stability: a finite element study.

BACKGROUND: Tibial shaft fractures account for approximately 15% of long bone fractures. Locked plates with minimally invasive plate osteosynthesis techniques are used widely by surgeons. The purpose of this study is to investigate the impact of factors meaning the plate length, fibula integrity, and placement of the plate on the stability of tibial shaft fracture fixation. METHODS: A finite element model of the tibial shaft fracture was built. An axial force of 2500 N was applied to simulate the axial compressive load on an adult knee during single-limb stance. The equivalent von Mises stres

Distal third fibular aneurysmal bone cyst: en bloc resection and proximal third fibular reconstruction.

Treatment of aneurysmal bone cysts (ABCs) located in the distal third fibula adjacent to the growth plate, can be problematic. The first choice of treatment for these lesions has been cyst curettage and bone graft. Complications include the high recurrence rate, possible growth-plate injury, and possible ankle instability associated with that treatment led to the development of other surgical options. We used en bloc resection and proximal third fibular bone reconstruction to treat an 8-year-old female patient with a distal third fibular ABC close to the growth plate. There was no recurrence,

Radial Extracorporeal Shock Wave Treatment Promotes Bone Growth and Chondrogenesis in Cultured Fetal Rat Metatarsal Bones.

BACKGROUND: Substantial evidence exists to show the positive effects of radialextracorporeal shock wave therapy (ESWT) on bone formation. However, it is unknown whether rESWT can act locally at the growth plate level to stimulate linear bone growth. One way to achieve this is to stimulate chondrogenesis in the growth plate without depending on circulating systemic growth factors. We wished to see whether rESWT would stimulate metatarsal rat growth plates in the absence of vascularity and associated systemic growth factors. QUESTIONS/PURPOSES: To study the direct effects of rESWT on growth pla

Biomechanical performance of proximal tibia fracture plates: A review.

PURPOSE: Proximal fractures of the tibia (i.e., shin bone) are often treated using proximal tibia fracture plates (PTFPs) that are not always biomechanically optimal. This is a review of biomechanical papers that studied the effect of modifying PTFP plate and screw variables. METHODS: PubMed, Scopus, and Web of Science databases were searched for English-language papers published before February 2024 using the terms "biomechanics" plus "proximal or plateau" plus "tibia" plus "fracture or "plate." Eligibility criteria were applied: (1) biomechanical studies only; (2) optimization studies that

Should We Worry About the Inter-Implant Gap in the Tibia? A Finite Element Analysis of Revision TKA and Distal Plating.

Background and Objectives: The management of periprosthetic tibial fractures distal to revision Total Knee Arthroplasty (TKA) presents a biomechanical challenge, often requiring extramedullary locking plates when long stems preclude nailing. While in femoral fractures the gap between the stem and plate is a well-documented stress riser, requiring implant overlap to prevent an inter-implant fracture, this specific biomechanical scenario has not been studied in the tibia, and it remains unclear if the femoral dogma of mandatory overlap applies to the straight, centrically loaded tibial anatomy.

Locking plates for distal fibula fractures in young and elderly patients: A retrospective study.

BACKGROUND: Ankle fractures are common injuries in the young and elderly populations. To prevent post-traumatic arthritis, an anatomic reconstruction of the ankle structure is mandatory. Open reduction and internal fixation is the treatment of choice among orthopaedics. Conventional plates allow stability of the fracture if bone quality is present. Locking plates might offer an advantage for the treatment of lateral malleolar fracture in patients with comminution, severe instability, distal fractures, or osteoporotic bone. Our hospital introduced a new locking plate for fracture of the distal

Bone growth after replantation in children.

Bone growth and the development of epiphyseal plates are disturbed after the replantation of amputated extremities in children, but the potential for continued skeletal growth is almost always maintained in the replanted part. In this reported series of 12 children, all younger than 15 years old, 13 amputated parts of the upper limb have been successfully replanted. After long-term follow-up (from 21 to 216 months), bone growth of the replanted parts was clinically and radiologically evaluated. Two different growing segments were distinguished: the proximal bone segment, directly injured from
