Odontogenic brain abscess caused by Porphyromonas gingivalis and Streptococcus constellatus: a case report and review article.

BACKGROUND: Odontogenic brain abscess is a rare, but potentially fatal, central nervous system infection, with insidious onset and unclear etiology. METHODS: This case reports a 70-year-old male patient who developed an odontogenic brain abscess secondary to periodontal infection and underwent neurological surgery. Extract pus during surgery for the metagenomic next-generation sequencing (mNGS). RESULTS: The mNGS of pus samples obtained from brain abscess aspiration identified the periodontal pathogens Porphyromonas gingivalis and Streptococcus constellatus. Consequently, he was referred to …

Brain abscess size and the number of surgical interventions required: a 10-year single-center experience.

OBJECTIVE: The authors aimed to determine whether the size of a brain abscess on presentation was related to the number of surgical interventions required to manage it adequately. METHODS: Adult and pediatric patients admitted with a brain abscess who underwent surgical intervention between April 2013 and April 2023 at a single neurosurgical unit in the United Kingdom were included. Ordinal logistic regression analysis was conducted to assess whether the size of brain abscess on admission, age, or symptom duration correlated with the number of surgical interventions ultimately required. Furth…

Late-onset Streptococcus pneumoniae brain abscess following cystoperitoneal shunt placement for an arachnoid cyst: illustrative case.

BACKGROUND: Brain abscesses represent a focal intracranial infection characterized by the localized accumulation of pus within a vascularized capsule. While typically occurring in immunocompromised patients, they may also present in otherwise healthy individuals. CSF shunt-related infections commonly manifest as meningitis or ventriculitis; however, brain abscess formation remains rare, particularly with unusual pathogens and extended latency. OBSERVATIONS: The authors present the case of a 58-year-old female patient who developed a Streptococcus pneumoniae brain abscess 7 years after cystope…

Brain Abscess Following Definitive Radiotherapy in Patients With External Auditory Canal Carcinoma: Report of Two Cases.

While osteoradionecrosis of the temporal bone is a late complication of radiotherapy (RT) for external auditory canal carcinoma (EACC), brain abscesses are rare. We present two cases of EACC treated with definitive RT, both of whom subsequently developed brain abscesses. The first patient, a 65-year-old woman with right EACC invading the sigmoid sinus and dura mater, developed a cerebellar abscess eight years post-RT (60 Gy/30 Fr). This abscess, attributed to infection related to temporal bone osteoradionecrosis, was successfully managed with drainage and antibiotics. The second patient, a 45-…

Brain abscess following right maxillary odontogenic infection in a patient with diabetes mellitus: a case report.

Odontogenic infections are common in the oral and maxillofacial region but rarely progress to life-threatening intracranial complications such as brain abscess. We report a rare case of a right maxillary odontogenic infection leading to a brain abscess in a previously undiagnosed, uncontrolled diabetic patient. A 42-year-old male presented with facial swelling and pain after endodontic treatment of the right maxillary premolars and molars. Initial improvement was achieved with incision, drainage, and antibiotic therapy; however, neurological deterioration occurred within days. Imaging revealed…

Case report: Two case reports of cryptogenic brain abscess caused by Fusobacterium nucleatum and literature review.

Brain abscess originates from a localized cerebritis area of brain parenchyma, remaining a refractory infectious disease in the central nervous system. Causative pathogens can be wide-ranging, including bacteria, fungi, or parasites; thus, precise pathogen identification and individualized antimicrobial therapy determine patients' outcomes. Here, we report two cases where both patients only presented with limb dysfunction, but without symptoms, signs, or biological evidence of infection. Samples were obtained through brain stereoscopic surgeries and microbial identifications were performed to …

Campylobacter fetus seeding of a cavernous malformation resulting in brain abscess: case report and literature review.

Microbial seeding of a cerebral cavernous malformation is an extremely rare occurrence with only 3 cases reported in the literature thus far. Campylobacter fetus is an opportunistic pathogen that rarely causes neurological infection with only 3 cases of C. fetus cerebral abscesses and 38 cases of C. fetus meningitis reported in the literature. There have been no cases of cerebral cavernous malformation seeding by C. fetus reported to date. We report the first case of cerebral cavernous malformation seeding by C. fetus, a case occurring in a previously healthy 16-year-old female who presented w

Brain Abscess Caused by Nocardia farcinica and Diagnosed by Metagenomic Next-Generation Sequencing: A Case Report.

BACKGROUND: Brain abscesses caused by Nocardia farcinica are rare and difficult to diagnose. Conventional methods for diagnosing Nocardia species include blood culture, microscopy, and tissue slice, but the performance is not satisfied. We report a case of brain abscess due to N. farcinica diagnosed by metagenomic next-generation sequencing (mNGS). CASE PRESENTATION: We report a case of a 58-year-old man with brain abscess caused by N. farcinica. The patient had a history of pemphigus and required long-term methylprednisolone administration. No pathogen was detected in blood culture, cerebros…

Partial oral antibiotic treatment for bacterial brain abscess: an open-label randomized non-inferiority trial (ORAL).

BACKGROUND: The advised standard treatment for bacterial brain abscess following surgery is 6 to 8 weeks of intravenous (IV) antibiotic treatment, but an early switch to oral antibiotic treatment has been suggested to be equally effective. METHODS: This investigator-initiated, international, multi-center, parallel group, open-label, randomized (1:1 allocation) controlled trial will examine if oral treatment after 2 weeks of IV antibiotic therapy is non-inferior to standard 6-8 weeks of IV antibiotics for bacterial brain abscess in adults (≥ 18 years of age). The study will be conducted at hos…

Brain abscess caused by Salmonella Enteritidis following craniotomy for meningioma: A case report and literature review.

Salmonella intracranial infection is infrequently encountered in clinical practice. However, with prompt intervention and appropriate antimicrobial therapy, the outcome is usually favourable. A 56-year-old gentleman who worked as an organic fertilizer production supervisor underwent tumour resection for meningioma located at the left frontal temporoparietal region. The surgical procedure went smoothly, and he has prescribed dexamethasone thereafter. He was discharged well. However, a few days after that he developed a fever associated with pus discharged from the surgical wound. A computed tom

Brain Abscess Presenting as Prolonged Headache in a Patient with Amyotrophic Lateral Sclerosis under Mechanical Ventilation.

A 57-year-old woman with amyotrophic lateral sclerosis (ALS) receiving mechanical ventilation developed intractable right temporal headache. She was diagnosed with brain abscess secondary to chronic suppurative otitis media. In this case, the otitis media was caused by nasopharyngeal reflux associated with eustachian tube muscle weakness and a supine position. In addition, ALS patients under mechanical ventilation have a limited ability to convey their pain. Their complaints are often overlooked because many physicians do not know that pain is common in ALS. Physicians should recognize brain a…

Nocardial brain abscess mimicking lung cancer metastasis in immunocompetent patient with pulmonary nocardiasis: a case report.

Nocardia is a ubiquitous microorganism which can be the cause of local and disseminated infection in humans. Immunocompetent and immunocompromised patients both can be affected and Nocardia cyriacigeorgica was reported as a pathogen isolated in patients worldwide. In most cases, nocardiosis is present as pulmonary infection because inhalation is the primary way of bacterial exposure. Nocardial brain abscess occurs usually secondary to a septic focus elsewhere in the body. Considering the facts that the elderly population is growing, such as the number of immunocompromised patients together wit…

Brain abscess developing in a non-operated spontaneous intracerebral haemorrhage: a case report and literature review.

Brain abscesses are a rare but potentially lethal neurological lesions, generally occurring after septic episodes in immunodeficient patients or complicating neurosurgical procedures. Even though they are known complications of surgically treated intracerebral haemorrhages (ICH), the presence of a brain abscess at the site of an untreated ICH is a rare event. Such cases may result from haematogenous spread from distant foci or contiguous sites and are often preceded by episodes of sepsis and local infection. Immunodeficiency, AIDS, age, diabetes mellitus and vitamin-K deficiency are predisposi…

Spontaneous sphenoid sinus mucocele revealed by meningitis and brain abscess in a 12-year-old child.

Sphenoid sinus mucocele is an uncommon lesion related to inflammatory disease that is diagnosed after surgery or a traumatic event. This report describes an unusual case revealed by bacterial meningitis and cerebral abscess in a 12-year-old child. CT and MR imaging allowed precise extension to the skull base in preoperative management and follow-up investigations. Endoscopic transnasal marsupialisation of the mucocele and antibiotic therapy led to complete remission. There was no evidence of recurrence after 6 months, which suggests that sphenoid mucoceles, regardless of size and complications

Patent foramen ovale as a possible risk factor for cryptogenic brain abscess: report of two cases.

OBJECTIVE AND IMPORTANCE: Patent foramen ovale (PFO) has been suggested as a potential source of paradoxical embolism. A higher prevalence of PFO in ischemic stroke of unexplained cause has been recognized. Brain abscesses are commonly associated with a contiguous focus of infection, hematogenous spread from a distant focus, or cranial trauma. However, no predisposing factors, including a distant focus with unknown cause, are identified in approximately 15 to 30% of reported cases. CLINICAL PRESENTATION: We encountered two patients with brain abscess presumably caused by dental infections. Bo…

Diagnostic Performance of DWI and ADC Parameter in Differentiating Brain Abscess from Malignant Ring-Enhancing Brain Lesions: A Systematic Review and Meta-analysis.

PURPOSE: To assess the performance of diffusion-weighted imaging (DWI) and apparent diffusion coefficients (ADC) in the differentiation of brain abscesses (BA) from malignant ring-enhancing brain lesions (REBL). METHODS: The protocol was pre-registered at (https://osf.io/mjwek). We conducted a comprehensive search in PubMed, Embase, and Web of Science until April 5, 2025, for studies that used DWI for differentiation of malignant REBLs from BAs. The risk of bias in the included studies was evaluated using QUADAS-2. ADC were compared between malignant REBLs and BAs using a random-effects model…
