Ventral brain stem compression in pediatric and young adult patients with Chiari I malformations.

OBJECTIVE: The purposes of this study were as follows: 1) to determine the incidence and degree of ventral brain stem compression (VBSC) in pediatric and young adult patients with Chiari I malformations, and 2) to correlate VBSC with other imaging and clinical factors to help determine what amount of VBSC is successfully treated with a posterior decompressive procedure alone. METHODS: The magnetic resonance images and clinical histories of 40 pediatric and young adult patients with Chiari I malformations were analyzed for subjective grade of VBSC, distance of tonsillar descent, odontoid's rel…

Acute obstructive hydrocephalus due to brain-stem edema caused by hyperosmotic insult. Case report.

The case of a 63-year-old man with acute obstructive hydrocephalus is presented. To the authors' knowledge, this is the first reported case of acute obstructive hydrocephalus associated with nonketotic hyperosmolar diabetic coma. It is believed that the plasma hyperosmolality resulted in osmotic endothelial injury leading to brain-stem edema.

Pediatric brain stem infarction caused by a basilar and vertebral artery occlusion: case report.

Basilar and right vertebral artery occlusion, which caused a brain stem infarction, occurred in an 11-year-old boy. There have been nine previous cases of basilar artery occlusion with steno-occlusive lesion in the vertebral artery. It is suspected that dissection of the vertebral artery at the atlantoaxial level and a gradual progression of arterial dissection to the basilar artery were the major causes of this infarction.

[Acute lymphoblastic leukemia with extensive demyelinated lesion of the brain stem during complete remission].

A case of acute lymphoblastic leukemia (ALL) which showed extensive demyelinated lesion of the brain stem during complete remission is presented. A 13 year-old girl who was diagnosed as ALL in February, 1987 was treated according to the TCCSG L84-11 protocol, which induced complete remission in June, 1987. Her condition had been stable until April, 1988 when she abruptly complained of gait disturbance, vertigo and anorexia. She was subsequently admitted to our hospital. At the time of admission, she had ataxic gait, horizontal nystagmus towards right at dextroversion, bilateral exaggerated pat…

Production of generalized learning deficit and permanent growth stunting by bilateral brain stem lesions.

Bilateral lesions of the globus pallidus, ventrolateral thalamus, substantia nigra, or the median raphe produce a generalized learning deficit in rats. Bilateral lesions of the dorsomedial hypothalamic nuclei stunt growth in rats without significantly disturbing endocrine functions and without producing a generalized learning deficit. Globus pallidus, ventrolateral thalamus, substantia nigra, median raphe, and dorsomedial hypothalamic nuclei lesions were produced in weanling Sprague-Dawley rats to compare their effect on physical growth. At approximately 72 d of age, all lesions had resulted i

Possible role of brain stem respiratory neurons in mediating vomiting during space motion sickness.

Vomiting is produced primarily by the coordinated action of the respiratory muscles. The object of this study was to determine if brain stem expiratory neurons control abdominal muscle activity during vomiting. We recorded the activity of 27 ventral respiratory group (VRG) expiratory neurons, which are known to be of primary importance for control of abdominal muscle activity during respiration. During fictive vomiting produced by emetic stimulants in decerebrate, paralyzed cats, only one-third of these neurons had the appropriate firing pattern to contribute to abdominal muscle activity durin…

Isolated brain-stem death. Case report with demonstration of preserved visual evoked potentials (VEPs).

Brain-stem death is a state in which brain-stem functions have ceased whereas the hemispheres still have electrical activity. We report a patient with a haematoma over the cerebellum and the pons who showed a preserved electroencephalogram and VEPs whereas all clinical signs of cerebral death was present. Normal VEPs in isolated brain-stem death have not yet been reported. We conclude that in any case of infratentorial lesion, an electroencephalogram should be recorded for the diagnosis of brain death.

[Brain stem auditory evoked potentials for assessment of isolated fourth ventricle in three patients].

Isolated fourth ventricle is an untoward but interesting complication caused by the shunt procedure, which has been recognized since the introduction of CT scanning. Auditory brain stem evoked potentials (BAEPs) were recorded in a patient with isolated fourth ventricle for assessment of brain stem function. Case 1. A 9-year-old boy. He had undergone repair of the lumbosacral meningocele at birth, followed by shunting procedures, including repeated shunt revision, for hydrocephalus which subsequently developed. CT scanning on admission revealed typical findings of isolated fourth ventricle, whi…

Auditory brain stem responses of premature infants to bone-conducted stimuli: a feasibility study.

The feasibility of bone conduction auditory brain stem response (ABR) audiometry in intensive care nursery neonates was investigated. Forty premature infants were tested with both air- and bone-conducted stimuli. Bone-conducted stimuli resulted in more identifiable ABRs and a greater number of subjects passing the hearing screening. The findings of this study suggest that bone conduction ABR audiometry is a feasible technique with premature infants. Due to the lower frequency composition of the bone-conducted click, it may be more effective than an air-conducted click when the immature cochlea…

Isolated Third Nerve Palsy and Alternate Hemiparesis in Brain Stem Stroke: A Case Report.

A very rare sign of brain stem stroke is Weber's Syndrome. Ipsilateral third nerve palsy and contralateral hemiparesis are the hallmarks of Weber's Syndrome. Midbrain infarction is the most frequent cause of Weber's Syndrome. The perforating branches of the basilar bifurcation or occlusion of the posterior cerebral artery commonly cause the infarction. We report the case of a 58-year-old female diagnosed with Weber's syndrome. The risk factors were hypertension and dyslipidemia. This patient presented with acute right-side motor weakness and left medial gaze palsy (third nerve palsy). A brain …

Placenta-derived mesenchymal stem cells attenuate acute brain injury and neuroinflammation after intracerebral hemorrhage.

Intracerebral hemorrhage (ICH) is a severe stroke subtype with high mortality and limited treatment options. This study evaluated the safety and therapeutic potential of human placenta-derived mesenchymal stem cells (PD-MSCs) in acute ICH. PD-MSCs were isolated, characterized, and assessed for tumorigenicity by subcutaneous implantation in nude mice. Acute ICH was induced in rats by collagenase injection, followed by in situ PD-MSC transplantation. Neurological function, brain water content, and hematoma volume were evaluated, and perihematomal biological changes were examined using immunofluo

Advanced Stem Cell Therapy: 3D-Bioprinted Brain-Like Transplants for Alzheimer's Disease-Like Dementia.

Alzheimer's disease (AD) is a neurodegenerative disorder that lacks effective treatments and urgently requires innovative therapeutic strategies. Although stem cell therapy has demonstrated efficacy in preclinical and clinical studies, it faces challenges such as low cell survival (<5%) and uncontrolled glial differentiation. This study aims to develop a 3D-bioprinted neural patch to enhance stem cell therapy for AD. The hypothesis is that a supportive bioengineered microenvironment would improve cell integration and neuronal differentiation, leading to functional recovery. A tri-component bio

Generating and characterizing human telencephalic brain organoids from stem cell-derived single neural rosettes.

We have developed a method for generating human telencephalic organoids from stem cell-derived isolated single neural rosettes. The use of single neural rosettes for generating organoids offers several important advantages. First, it mimics the development of neural tissue from a singular neural tube in vivo. Second, single neural rosette-derived organoids exhibit a relatively consistent and reproducible composition of telencephalic neural cells. Finally, single neural rosette-derived organoids demonstrate predictable organization of the identified neural cells around a single neural rosette-d

Hippocampal Neural Stem Cell Exosomes Promote Brain Resilience against the Impact of Tau Oligomers.

A promising therapeutic intervention for preventing the onset and progression of Alzheimer's disease is to protect and improve synaptic resilience, a well-established early vulnerability associated with the toxic effects of oligomers of amyloid β (AβO) and Tau (TauO). We have previously reported that exosomes from hippocampal neural stem cells (NSCs) protect synapses against AβO. Here, we demonstrate how exosomes can also shield against TauO toxicity in adult mice synapses, potentially benefiting primary and secondary tauopathies. Exosomes from hippocampal NSCs (NSCexo) or mature neurons (MNex

Apelin-13 can regulate adipose-derived mesenchymal stem cells to improve traumatic brain injury.

Traumatic brain injury (TBI) is one of the most serious neurological diseases worldwide. At present, there is no effective treatment for TBI. The regenerative effects of adipose-derived mesenchymal stem cells (ADSCs) on neuronal injury have garnered considerable concern in the scientific community over the past decade. Apelin-13, a key member of the apelin family, has anti-apoptotic, anti-inflammatory, and antioxidative stress effects. ADSCs can modify the microenvironment to enhance neuronal survival through secreting regulatory factors. Apelin-13 can regulate the microenvironment of ADSC dif

Age-related myelin deficits in the auditory brain stem contribute to cocktail-party deficits.

Age-related hearing loss consists of both peripheral and central components and is an increasing global health concern. While peripheral hearing loss is well understood, central hearing loss- age-related changes in the central auditory pathways resulting in a listener's inability to process sound correctly -remains poorly understood. In this study, we focus on the pathway from the cochlear nucleus to the medial nucleus of the trapezoid body (MNTB), which depends on heavily myelinated axons for microsecond-level temporal precision required for sound localization. Using a combination of auditory

Mesenchymal Stem Cells Overexpressing FGF21 Preserve Blood-Brain Barrier Integrity in Experimental Ischemic Stroke.

Blood-brain barrier (BBB) disruption is a prominent pathophysiological mechanism in stroke. Transplantation of mesenchymal stem cells (MSCs) preserves BBB integrity following ischemic stroke. Fibroblast growth factor 21 (FGF21) has been shown to be a potent neuroprotective agent that reduces neuroinflammation and protects against BBB leakage. In this study, we assessed the effects of transplantation of MSCs overexpressing FGF21 (MSCs-FGF21) on ischemia-induced neurological deficits and BBB breakdown. MSCs-FGF21 was injected into the rat brain via the intracerebroventricular route 24 h after mi
