Mechanical Circulatory Support in Paraganglioma Induced Cardiogenic Shock and Intestinal Ischemia: Lessons from a Complex Case and Narrative Review.

Background: The catecholamine-induced hypertensive crisis is a rare, life-threatening condition caused by excessive catecholamine release, often resulting in cardiogenic shock and multiorgan failure. Management is challenging, especially when hemodynamic instability persists despite standard medical therapy. Methods: We conducted a narrative review of published articles between 2013 and 2025. The search was conducted in MEDLINE (PubMed, Scholar and Embase). We also presented a case managed at our reference center. Results: Overall, 42 studies including 69 patients were included. ECMO was the m

Extracorporeal Left Ventricular Assist Device as a Bridge to Surgery for Ventricular Septal Rupture After Acute Myocardial Infarction.

Ventricular septal rupture (VSR) after acute myocardial infarction (AMI) is a rare but often fatal complication. Surgery is considered the preferred treatment, although the optimal timing is discussed. The immediate preoperative hemodynamic status significantly impacts postoperative outcomes, making mechanical circulatory support (MCS) devices crucial for perioperative hemodynamic stability. We present the case of a 61-year-old woman with no remarkable cardiological history admitted to our hospital with a diagnosis of AMI and VSR. Due to hemodynamic instability and cardiogenic shock, we utiliz

Mechanical circulatory support devices in advanced heart failure: 2020 and beyond.

Substantial progress in the field of mechanical circulatory support (MCS) has expanded the treatment options for patients with advanced-stage heart failure (HF). Currently available MCS devices can be implanted percutaneously or surgically. They can also be configured to support the left, right, or both ventricles, offering varying levels of circulatory support. Short-term temporary MCS devices are primarily used in high-risk percutaneous coronary intervention, cardiogenic shock, and post-cardiac arrest, while durable left ventricular assist systems (LVAS) are increasingly utilized either as a

Impact of mitral valve intervention with left ventricular assist device implantation on postoperative outcomes and morphologic change.

Although mitral regurgitation (MR) is prevalent in patients with end-stage heart failure, the impact of mitral valve (MV) surgery on outcomes after left ventricular assist device (LVAD) implantation and morphologic changes of MV remains unclear. We retrospectively reviewed 74 patients who underwent LVAD implantation as a bridge to transplant. Of these, 11 (15%) underwent MV repair concomitant with or prior to LVAD implantation, while 27 patients with preoperative significant (moderate or greater) MR did not undergo concomitant MV surgery. The mean interval between LVAD implantation and the las

Conversion of Percutaneous Cholecystostomy to Internal Transmural Gallbladder Drainage Using an Endoscopic Ultrasound-Guided, Lumen-Apposing Metal Stent.

Patients with acute cholecystitis sometimes require placement of percutaneous cholecystostomy catheters, either as a bridge to surgery or as primary therapy. In patients who cannot undergo surgery, subsequent removal of the catheter can lead to recurrence of cholecystitis, whereas leaving the drain in place can cause adverse events. We investigated internalization of percutaneous cholecystostomy drainage catheters, using endoscopic ultrasound (EUS)-guided placement of lumen-apposing metal stents (LAMS) as an alternative treatment strategy. Seven patients (median age, 57 years; 6 men) underwent

Atrial Flow Regulator: Bridging Pediatric Patients With Pulmonary Arterial Hypertension to Lung Transplantation.

BACKGROUND: Pulmonary arterial hypertension (PAH) in pediatric patients remains challenging, particularly in symptomatic patients with right heart failure despite optimal medical therapy. The use of an atrial flow regulator (AFR) to decompress the right ventricle has offered a new therapeutic approach for those patients and allows bridging for lung transplantation (LTX). Data on pretransplant decision-making, perioperative considerations, and post-transplant management in pediatric patients undergoing LTX are limited. CASE PRESENTATION: We report 2 pediatric patients with severe symptomatic P

Adapting and testing of DeprEnd EMDR therapy for major depressive disorder: a study protocol of mixed method randomized controlled trial.

BACKGROUND: Major depressive disorder (MDD) is a significant contributor to the global burden of disease, with a high prevalence in Pakistan. MDD is treatable, and eye movement desensitization and reprocessing (EMDR) therapy is recognized as an effective treatment worldwide. However, EMDR therapy, rooted in Western psychological frameworks, may require cultural and methodological adaptation to fit clients' needs in Pakistan. Notably, there is a lack of research on adapting EMDR therapy in Pakistan, and scarce studies on its effectiveness through online modalities. To bridge these research gaps

Real-World Patterns of Immunomodulator Use in Patients with Inflammatory Bowel Disease Receiving Biologic Therapy Under a 54-Week Reimbursement Cap: A 12-Year Retrospective Single-Center Cohort from Taiwan.

BACKGROUND: Recent studies have shown that concomitant immunomodulator (IMM) use does not significantly affect remission in patients with inflammatory bowel disease (IBD) treated with non-anti-tumor necrosis factor (non-anti-TNF) biologics. We evaluated the real-world role of IMMs in IBD patients receiving biologic therapy, including potential use as bridge therapy - continuing IMMs to maintain remission during mandated biologic treatment intervals - and patterns of IMM withdrawal and reinitiation. MATERIALS AND METHODS: This retrospective, single-center study included IBD patients receiving …

Novel medical therapies for pulmonary arterial hypertension.

Available targeted therapies for pulmonary arterial hypertension are capable only of slowing progression of the disease and a cure remains elusive. However with the improved understanding of the pulmonary vascular remodeling that characterizes the disease, there is optimism that the disconnect between preclinical and clinical studies may be bridged with some of the newer therapies that are now at different stages of clinical evaluation. This article examines the evidence behind these new candidate treatments that may become part of the arsenal available for clinicians managing this devastating

Beyond Scar Removal: Reprogramming the Glial Scar for Neural Repair.

Spinal cord injury (SCI) research has long treated the glial scar as the central obstacle to regeneration. That view is useful, but it is now too narrow. The scar is not a passive wall. It is an evolving repair tissue that contains inflammation, stabilizes the lesion, remodels extracellular matrix, and, under many conditions, restricts axonal growth. The next phase of SCI therapy should therefore move from scar suppression to scar reprogramming. In this Opinion, I argue that the field should prioritize temporally precise, spatially targeted, and biomarker-guided interventions that preserve the

Compression therapy and heart failure: a scoping review of the existing evidence.

The completion of a scoping review within the area of compression therapy and heart failure offers an insight into the present literature in this area, alongside offering the ability to connect this existing knowledge to chronic oedema/lymphoedema when both conditions co-exist. The evidence obtained included national agreed guidelines, consensus documents and existing primary/secondary research. The review identified existing evidence that suggests that the application of compression therapy in those with heart failure may be appropriate, but is dependent upon staging and stability. However, t

Device therapy and cardiac transplantation for end-stage heart failure.

The prevalence of heart failure is increasing, and the prognosis of end-stage heart failure remains dismal. The gold-standard therapy in end-stage heart failure remains cardiac transplantation at the present time, but there is a great excess of eligible candidates compared with the number of donor organs. Advances in mechanical support, the development of the left ventricular assist device (LVAD), and the total artificial heart has reduced mortality and morbidity in patients awaiting transplantation, and LVADs are now approved as an strategy for destination therapy. Miniaturization, increased

A Sulfur-Bridging Sulfonate-Modified Zinc(II) Phthalocyanine Nanoliposome Possessing Hybrid Type I and Type II Photoreactions with Efficient Photodynamic Anticancer Effects.

Phthalocyanines are potentially promising photosensitizers (PSs) for photodynamic therapy (PDT), but the inherent defects such as aggregation-caused quenching effects and non-specific toxicity severely hinder their further application in PDT. Herein, we synthesized two zinc(II) phthalocyanines (PcSA and PcOA) monosubstituted with a sulphonate group in the alpha position with "O bridge" and "S bridge" as bonds and prepared a liposomal nanophotosensitizer (PcSA@Lip) by thin-film hydration method to regulate the aggregation of PcSA in the aqueous solution and enhance its tumor targeting ability.

Tackling the gap in platelet inhibition with oral antiplatelet agents in high-risk patients undergoing percutaneous coronary intervention.

Introduction: Oral P2Y12 inhibitors represent the mainstay therapy for the prevention of thrombotic complications in patients presenting with an acute coronary syndrome and/or undergoing percutaneous coronary intervention (PCI). However, the onset of antiplatelet action of the oral P2Y12 inhibitors is affected by their need to be absorbed in the gastrointestinal (GI) tract before becoming systemically available.Areas covered: Following oral intake of P2Y12 inhibitors, the timeframe required for GI absorption leads to a window of inadequate antiplatelet protection during which patients are at i

Sustained Use of the Impella 5.0 Heart Pump Enables Bridge to Clinical Decisions in 34 Patients.

We studied whether sustained hemodynamic support (>7 d) with the Impella 5.0 heart pump can be used as a bridge to clinical decisions in patients who present with cardiogenic shock, and whether such support can improve their outcomes. We retrospectively reviewed cases of patients who had Impella 5.0 support at our hospital from August 2017 through May 2019. Thirty-four patients (23 with cardiogenic shock and 11 with severely decompensated heart failure) underwent sustained support for a mean duration of 11.7 ± 9.3 days (range, ≤48 d). Of 29 patients (85.3%) who survived to next therapy, 15 wer
