The syndrome of normal electrocardiograms, accessory pathways and paroxysmal tachycardias.

Tachycardias may be associated with the pre-excitation syndrome, a short PR interval or a prolonged QT interval. This report describes seven patients whose electrocardiograms (ECGs) did not reveal any of the above-named conditions. All of the patients presented with syncopal episodes, and the history of palpitations was elicited in five of the patients. His bundle electrogram studies in all of the patients showed an essentially unchanged AH interval with atrial pacing rates up to 180 beats/min. This can only be explained by a total bypass of the AV node. Two of the patients were also paced fro

Distinct intracardiac electrogram waveforms with perforation during left bundle branch area pacing implantation.

AIMS: Perforation during left bundle branch area pacing (LBBAP) results in a fall in the current of injury (COI) amplitude in the unipolar unfiltered electrogram (iEGM), but systematic waveform analyses have not been performed. Our aim was to investigate unipolar iEGM waveforms during perforation and to compare them to those at the final lead position. METHODS AND RESULTS: The iEGMS of consecutive patients who had perforation during LBBAP implantation were systematically analysed. A total of 92 patients with perforation were included. In the unfiltered channel, sensed COI amplitude was lower

Safety and Efficacy of Stylet-Driven Leads for Left Bundle Branch Area Pacing in Octogenarians.

BACKGROUND: Left bundle branch area pacing (LBBAP) is emerging as a physiological alternative to conventional pacing, particularly in elderly patients. Stylet-driven leads (SDL) offer unique procedural advantages, but data on their safety and efficacy in octogenarians remain sparse. OBJECTIVE: This study evaluates the feasibility, safety, and outcomes of SDL for LBBAP in octogenarians. METHODS: A retrospective, single-center study included 117 octogenarian patients (≥80 years) and 308 younger patients (<80 years) who underwent LBBAP with SDL between May 2022 and December 2023. Baseline chara

Safety and performance of a novel defibrillation lead for left bundle branch area placement: Primary results of the LEADR LBBAP clinical trial.

BACKGROUND: Right ventricular placement of the OmniaSecure defibrillation lead has demonstrated high defibrillation success and low occurrence of lead-related major complications. With the advent of left bundle branch area pacing (LBBAP) as a physiological pacing option, the use of the OmniaSecure lead may be a viable alternative for LBBAP in the implantable cardioverter-defibrillator population. OBJECTIVE: This study aimed to understand the defibrillation efficacy at implant and the safety of the OmniaSecure lead for LBBAP. METHODS: The global Lead Evaluation for Defibrillation and Reliabil

Understanding electromechanics of left bundle branch area pacing: Association between electrical synchrony, mechanical synchrony, and myocardial work parameters.

INTRODUCTION: During left bundle branch area pacing (LBBAP), several markers of electrical synchrony, (V6 R wave peak time [RWPT], aVL-RWPT, and the V6-V1 interpeak interval), are commonly used to assess LBB capture. Nevertheless, the relationship between these electrocardiographic (ECG) measurements and mechanical synchrony remains poorly understood. OBJECTIVE: We aimed to analyze the association between electrical parameters from the paced QRS (p-QRS) complex and mechanical performance assessed through 2-dimensional (2D) strain and myocardial work (MW) indices, following LBBAP implantation.

Retrograde Conduction in Left Bundle Branch Block: Insights From Left Bundle Branch Pacing.

BACKGROUND: Biventricular pacing is a well-established therapy for patients with heart failure (HF), left bundle branch block (LBBB) and left ventricular (LV) dysfunction. Left bundle branch pacing (LBBP) has emerged as an alternative to biventricular pacing. OBJECTIVES: The aim of this study was to assess the retrograde conduction properties of the left bundle branch in patients with nonischemic cardiomyopathy and LBBB during LBBP and its clinical implications. METHODS: Patients undergoing successful LBBP for nonischemic cardiomyopathy with LV ejection fraction (LVEF) ≤35% and LBBB were inc

Ablation-Induced Change in the Course of Fascicular Tachycardia.

BACKGROUND: Multiform fascicular tachycardia (FT) was recently described as a ventricular tachycardia (VT) that has a reentrant mechanism using multiple fascicular branches and produces alternate fascicular VT forms. Ablating the respective fascicle may cause a change in the reentrant circuit resulting in a change in morphology. Ablation of the septal fascicle is crucial for successful treatment. OBJECTIVES: To describe four cases of FT in which ablation induced a change in QRS morphologies and aggravated clinical course. METHODS: Four out of 57 consecutive FT cases at three institutions wer

Implications of 2:1 atrioventricular block during typical atrioventricular nodal reentrant tachycardia.

OBJECTIVE: The effects of 2:1 AV block (AVB) on AV nodal reentrant tachycardia (AVNRT) remain to be elucidated. This study was performed to localize the site of 2:1 AVB and elucidate the effects of 2:1 AVB on typical AVNRT. METHODS: The His bundle (HB) electrograms during typical AVNRT with 2:1 AV block were reviewed in 24 patients. It was hypothesized that if 2:1 AVB at the HB or below changed tachycardia cycle length (TCL), the lower turnaround point of the reentrant circuit (RC) might be located within the HB and parts of the HB might be involved in the RC. RESULTS: A HB potential was abs

Atrioventricular nodal reentry in the Wolff-Parkinson-White syndrome.

Although supraventricular tachycardia in the Wolff-Parkinson-White (WPW) syndrome is generally due to atrioventricular reentry, the presence of the accessory pathway does not preclude other mechanisms of tachycardia. We observed AV nodal reentry in three of 95 consecutive patients (3.1%) referred for assessment of arrhythmias associated with WPW syndrome. The unique observation of spontaneous transition from atrioventricular reentry to AV nodal reentry at a similar cycle length was observed in one patient and is the subject of this report.

Electrocardiographic criteria for differentiating aberrancy and ventricular extrasystole in chronic atrial fibrillation: validation by intracardiac recordings.

Many electrocardiographic criteria have been proposed for the differentiation of ventricular extrasystole and supraventricular conduction with aberrancy in atrial fibrillation but the validity of these have not been confirmed by intracardiac studies. We recorded His bundle electrograms in nineteen patients (eleven men, eight women) referred for diagnosis of abnormal QRS complexes in the context of chronic atrial fibrillation. Of 1,068 wide QRS complexes analyzed, 91% proved to be of ventricular origin. Electrocardiographic criteria which were specific for ventricular extrasystole included: lef

[Transient right bundle branch block during anesthesia].

Transient right bundle branch block occasionally occurs under anesthesia. We report a case of heart rate dependent right bundle branch block during sevoflurane anesthesia. A 74-year-old man was scheduled for partial hepatectomy. Past medical history was unremarkable, and preoperative electrocardiogram showed no conduction abnormalities. Operation was performed with epidural and general anesthesia. Five hours after induction of general anesthesia, complete right bundle branch block (CRBBB) occurred gradually without any hemodynamic changes. Although CRBBB persisted in his electrocardiogram (ECG

[A case of Ebstein's anomaly treated by tricuspid valve replacement].

A 8-year-old girl with Ebstein's anomaly was operated on because of progressive heart failure caused by severe tricuspid regurgitation. At the operation, the tricuspid leaflets were found to be destroyed, and the annulus was very enlarged. The enlarged true tricuspid annulus was constricted using the DeVega method, and the tricuspid valve was replaced on the true annulus using a porcine valve. To avoid injury to the auriculoventricular bundle, the suture line was shifted to the false annulus at the portion close to the course of His' bundle. Postoperative course was uneventful in satisfactory

Initial Experience, Safety, and Feasibility of Left Bundle Branch Area Pacing: A Multicenter Prospective Study.

OBJECTIVES: This study sought to evaluate the safety and feasibility of conduction system pacing by performing left bundle branch area pacing (LBBAP). BACKGROUND: There are limited data from single centers showing that LBBAP may circumvent the technical and electrophysiological challenges encountered with His bundle pacing. METHODS: Patients referred for pacemaker implantation at 2 centers between February 1, 2019, and March 31, 2020, were considered for LBBAP. LBBAP was performed by implanting a lumen-less, exposed helix lead approximately 2 cm distal to the His bundle and deep into the sep

His overdrive pacing during supraventricular tachycardia: a novel maneuver for distinguishing atrioventricular nodal reentrant tachycardia from atrioventricular reciprocating tachycardia.

BACKGROUND: Because the His bundle is intrinsic to the circuit in orthodromic reciprocating tachycardia and remote from that of atrioventricular nodal reentrant tachycardia (AVNRT), pacing the His bundle during supraventricular tachycardia (SVT) may be useful to distinguish these arrhythmias. OBJECTIVE: The purpose of this study was to test the hypothesis that His overdrive pacing (HOP) would affect SVT immediately for orthodromic reciprocating tachycardia and in a delayed manner for AVNRT. METHODS: Once SVT was induced, HOP was performed by pacing the His bundle 10-30 ms faster than the SVT

Acute Onset of Heart Failure and Renal Failure Due to Pacemaker Syndrome.

Here, we report the outcome of an 87-year-old man with permanent non-valvular atrial fibrillation who initially presented with complete heart block and received a single right ventricle lead pacemaker programmed to ventricular demand pacing (VVIR). Over the next 10 months, the patient was readmitted to the hospital four times with recurrent edema, pleural effusions, and ascites. He was diagnosed with new onset systolic heart failure with mid-range (40-49%) ejection fraction and cardiorenal syndrome requiring dialysis. The underlying cause of his presentation was determined to be pacemaker synd

[Nodoventricular Mahaim fibres. 2 cases confirmed by electrophysiological study].

Two adolescents with no obvious cardiac were investigated for paroxysmal tachycardia at a rate of about 200/mn with wide QRS complexes showing left bundle branch block, reduced by vagal manoeuvres. Atrial pacing showed shortening of the HV interval associated with widening of the QRS complex to appearances identical to those of the tachycardia, suggesting right ventricular preexcitation masked in sinus rhythm. The progressive prolongation of the atrio-ventricular conduction time with increasing prematurity of the extrastimulus and atrioventricular block after intravenous injection of ATP were
