Tight regulation of serum concentrations of calcium and phosphate is indispensable for maintaining normal physiological condition. Imbalance of this regulation leads to pathophysiological disorders including heart disease, chronic kidney disease, and ectopic calcification. Formation and mineralization of bone and tooth are greatly influenced by calcium and phosphate metabolism since both organs are mainly consist of calcium-phosphate. Calcium and phosphate homeostasis is under hormonal control on its target organs such as kidney, bone and intestine. Calcium and phosphate are absorbed in intest…

Regulation of calcium flux in the heart is a key process that affects cardiac excitability and contractility. Degenerative diseases, such as coronary artery disease, have long been recognized to alter the physiology of intracellular calcium regulation, leading to contractile dysfunction or arrhythmias. Since the discovery of the first gene mutation associated with catecholaminergic polymorphic ventricular tachycardia (CPVT) in 2001, a new area of interest in this field has emerged--the genetic abnormalities of key components of the calcium regulatory system. Such anomalies cause a variety of g…

Pregnancy and lactation are associated with profound changes in calcium physiology. This can make interpretation of biochemical tests difficult during these times. Diseases of bone and calcium metabolism are uncommon during pregnancy but can be severe when they occur. These conditions are complicated by the need for dual consideration for the maternal and foetal health. These conditions are also much less well studied than related conditions that occur outside of pregnancy resulting in a reduction in evidence for and confidence in recommendations relating to management. This chapter will revie…

Vitamin D and calcium are indispensable for the maintenance of bone mass and integrity. Active vitamin D3 has been most widely used in Japan for the treatment of osteoporosis. These drugs mildly increase bone mineral density, but it has little consistent effects on bone metabolic markers. Although weak, there is also some clinical evidence for their efficacy as a fracture-preventing drug. Recent reports suggest that vitamin D3 not only enhances intestinal calcium absorption but may also improve neuromuscular function to reduce the number of falls, thereby contributing to fracture prevention. B…

The kinetics of interdialytic calcium (Ca) absorption and intradialytic flux are examined with particular emphasis on achievement of near zero Ca mass balance over complete dialysis cycles. It is concluded that a rational approach to management of mineral metabolism in hemodialysis consists of: (1) appropriate doses of vitamin D3 analogs to facilitate now normal Ca absorption and avoid both Ca overload and depletion; (2) adequate phosphorus (P) binder to normalize serum calcium and the Ca P product; (3) in most instances modest reduction of dialysate Ca to the level of 2.00-2.25 are appropriat…

Verapamil, a Ca(2+) entry blocker, can induce bronchorelaxation and bronchoconstriction. The mechanism of verapamil-induced bronchoconstriction is poorly understood. The present study determines the direct effect of verapamil on smooth muscle of isolated ovine airways and analyzes the mechanisms involved. Isolated tracheal strips were suspended in organ baths containing Krebs solution for isometric tension recording. Tissue responses to verapamil as assessed by basal tone were examined in the presence or absence of epithelium. The effects of verapamil on carbachol and cooling-induced contracti

Detrimental effects of salinity on plants are known to be partially alleviated by external Ca(2+). Previously we demonstrated that in citrus cells, phospholipid hydroperoxide glutathione peroxidase (GPX1) is induced by salt and its activation can be monitored by pGPX1::GUS fusion in transformed tobacco cells. In this paper we further characterized the induction of GPX1 by additional treatments, which are known to affect Ca(2+) transport. Omission of Ca(2+) changed the pattern of the transient salt-induced expression of GPX1 and chelation of Ca(2+) by EGTA, or treatment with caffeine, abolished

Abnormalities of calcium, magnesium and phosphorus are common in hospitalized patients. Infrequently patients might present in the outpatient settings with non-specific symptoms that might be due to abnormalities of divalent cation (magnesium, calcium) or phosphorous metabolism. Several inherited disorders have been identified that result in renal or intestinal wasting of these elements. Physicians need to have a thorough understanding of the mechanism of calcium, magnesium and phosphorous metabolism and diagnoses disorders due to excess or deficiency of these elements. Prompt identification a…

This article assesses the nature of the sarcoplasmic reticulum (SR) in uterine smooth muscle. Modern imagining techniques have revealed new information about the location and density of Ca storage and release. Release mechanisms, including IP(3) and Ca itself, via ryanodine receptors (RyR), as well as possible roles for cyclic ADP ribose, and the contribution of the SR to relaxation are detailed. The role of the SR Ca-ATPase in both decay of the Ca transient and maintaining Ca homeostasis is reviewed. Recent data on the role of local Ca signals from the SR in contributing to membrane excitabil

The calcium (Ca(2+))-sensing receptor (CaR) belongs to family C of the G-protein coupled receptors (GPCRs). The receptor is activated by physiological levels of Ca(2+) (and Mg(2+)) and positively modulated by a range of proteinogenic L-alpha-amino acids. Recently, several synthetic allosteric modulators of the receptor have been developed, which either act as positive modulators (termed calcimimetics) or negative modulators (termed calcilytics). These ligands do not activate the wild-type receptor directly, but rather shift the concentration-response curves of Ca(2+) to the left or right, resp…

The plasma membrane calcium ATPase (PMCA) uses energy to pump calcium (Ca2+) ions out of the cytosol into the extracellular milieu, usually against a strong chemical gradient. This energy expenditure is necessary to maintain a relatively low intracellular net Ca2+ load. Mammals have four genes (ATP2B1-ATP2B4), encoding the proteins PMCA1 through PMCA4. Transcripts from each of these genes are alternatively spliced to generate several variant proteins that are in turn post-translationally modified in a variety of ways. Expressed ubiquitously and with some level of functional redundancy in most …

In the last 10 years, it has been well documented that mineral metabolism abnormalities in dialysis patients are associated with an enhanced risk of morbidity and mortality for cardiovascular disease. Extraskeletal calcifications represent one of the major risk factors involved in the pathogenesis of cardiovascular disease in this population. In fact, secondary hyperparathyroidism and hyperphosphatemia associate with increased cardiovascular mortality in uremic patients for two reasons: first for the passive deposition of calcium and phosphate in soft tissues; second for the active role of ino…

The calcium-sensing receptor (CASR) adjusts the extracellular calcium set point regulating PTH secretion and renal calcium excretion. The receptor is expressed in several tissues and is also involved in other cellular functions such as proliferation, differentiation and other hormonal secretion. High extracellular calcium levels activate the receptor resulting in modulation of several signaling pathways depending on the target tissues. Mutations in the CASR gene can result in gain or loss of receptor function. Gain of function mutations are associated to Autossomal dominant hypocalcemia and Ba…

In many hemodialysis patients at our hospital on May 2003, serum calcium levels were higher and intact PTH levels were lower than the target level of the K/DOQI guidelines. We could decrease doses of calcium-based phosphate binders by using sevelamer hydrochloride, and that could lower the level of calcium and raise the level of intact PTH. The number of patients who satisfy all the targets for calcium, phosphate, Ca x P and int-PTH, increased from 1.7% to 14.7% as a result of diet therapy and a change in their dose of medicine. It is natural that diet greatly influences on the control of calc…

CONTEXT: Just a decade after the the calcium-sensing receptor (CaR) was identified, pharmacological manipulation of the CaR is about to enter routine practice. For hyperparathyroid states, calcimimetics, which increase activation of the CaR, have been licensed in Europe and the USA. Calcilytics, which decrease CaR function and increase secretion of parathyroid hormone (PTH), might allow the anabolic effects of PTH on bone to be harnessed for the prevention and treatment of osteoporosis.
STARTING POINT: In a multicentre randomised double-blind placebo-controlled study, Munro Peacock and collea…

To identify barriers to following calcium recommendations among women with reduced bone mineral density (BMD), four focus group sessions were held with 30 postmenopausal women diagnosed with low BMD. Key concepts and themes were derived from transcripts. Participants were aware of the importance of calcium to their bone health, and were attempting to follow calcium intake recommendations. Several major themes associated with the ability to obtain adequate calcium were identified, including knowledge and confidence in actions, lifestyle and food preferences, and side effects and conflict with o…

The discovery and cloning of the calcium-sensing receptor (CaR) in 1993 has led to a better understanding of the regulation of calcium homoeostasis. Following activation by extracellular calcium ions, the CaR triggers a cascade of intracellular events. These events result in the release of secondary messengers, which have a number of biological effects, the most important of which is a reduction in parathyroid hormone (PTH) secretion. The way in which calcium acts on the CaR varies depending on the cell type. In the parathyroid gland cell, activation of the CaR by elevated serum levels of calc…
