Symptomatic epidural hematoma caused by lumbar synovial cyst rupture: report of two cases and review of the literature.

OBJECTIVE AND IMPORTANCE: Juxtafacet cysts of the lumbar spine are a recognized cause of back pain, radicular symptoms, neurogenic claudication, and, less often, cauda equina compression syndrome. Hemorrhagic rupture of a juxtafacet cyst and the resulting epidural hematoma may cause chronic and/or acute cauda equina compression. CLINICAL PRESENTATION: We report two cases of ruptured hemorrhagic lumbar synovial cysts resulting in epidural hematoma and progressive symptomatic cauda equina compression. INTERVENTION: Surgical excision of the epidural hematoma and the associated cyst resulted in

Functional outcome after experimental cauda equina compression.

Spinal nerve roots often sustain compression injuries. We used a Wistar rat model of the cauda equina syndrome to investigate such injuries. Rapid transient compression of the cauda equina was produced using a balloon catheter. The results were assessed by daily neurological examination and somatosensory evoked potential (SEP) recording before surgery and ten weeks after decompression. Compression of the spinal nerves induced changes in the SEP which persisted for up to ten weeks after decompression, but it had no effect on the final neurological outcome. Our study shows the importance of earl…

[Intermittent claudication by the narrowing of the spinal canal].

The cases of four patients with intermittent claudication due to compression of the cauda equina are reported. The cause of this syndrome is considered to be a compression of the cauda equina by a thickening of the ligamentum flavum. The clinical, neuro-radiological and surgical characteristics are discussed.

Epidural lipomatosis in elderly patient: A rare cause of cauda equina compression.

BACKGROUND: The most common cause of cauda equina compression in the elderly is lumbar spinal stenosis. Epidural lipomatosis is an additiona … this was confirmed on the MRI myelogram study. Following a decompressive laminectomy, the patient's cauda equina syndrome resolved. CONCLUSION: Recent weight gain with increased neurogenic claudication and the onset of a cauda equina syndrome may herald the presence of significant lumbar epidural lipomatosis. Here, laminectomy for excision of the excessive epidural fa …

SHADES of grey - The challenge of 'grumbling' cauda equina symptoms in older adults with lumbar spinal stenosis.

Diagnosing cauda equina syndrome is challenging in older adults with lumbar spinal stenosis. Understanding these challenges is vital for clinicians who are faced with difficult decisions about when to refer for investigation or surgical management. This is a growing clinical issue because of the escalating prevalence of lumbar spinal stenosis in our ageing population, and increasing demands on healthcare services including imaging and surgical services. This professional issue explores the challenges and evidence gaps relating to cauda equina syndrome in older adults with lumbar spinal stenosi…

Redundant nerve roots of the cauda equina.

The authors have critically reviewed 8 cases which have come under their observation of a lumbosciatic syndrome due to redundant nerve roots of the cauda equina. Some of the clinical and myeloradiculographic features appear to be characteristic of this rare syndrome. Surgical intervention by decompressing the nerve roots of the cauda equina offers a very favourable prognosis.

Pathology of the conus medullaris and cauda equina. Beyond the usual suspects.

BACKGROUND: Pathologies affecting the conus medullaris and cauda equina can present with overlapping clinical symptoms, making an accurate diagnosis essential. Conus medullaris syndrome results from damage at the T12-L2 level, while cauda equina syndrome arises from nerve root compression below the conus. Both conditions may cause motor deficits, sensory disturbances, and autonomic dysfunction, necessitating a detailed differential diagnosis. OBJECTIVE: This educational review highlights common and rare etiologies of conus medullaris and cauda equina lesions, emphasizing imaging characteristi…

Neurological Phenotype and MRI Severity as Predictors of Duloxetine Response in Lumbar Spinal Stenosis: A Retrospective Cohort Study.

Background/Objectives: Lumbar spinal canal stenosis (LSS) is a major cause of neurological disability and is frequently accompanied by neuropathic pain (NeP). Duloxetine is widely used for NeP, but its clinical role in LSS and the determinants of treatment response remain unclear. This study aimed to identify predictors of duloxetine efficacy in LSS. Methods: We retrospectively analyzed 145 patients with LSS who received duloxetine for ≥3 months (median dose 40 mg) with at least 1-year follow-up. Patients were classified into those with radicular pain or cauda equina syndrome. Treatment respon…

Dorsal compression of the epidural cord due to free sequestral lumbar prolapse. Diagnostic problems in magnetic resonance imaging and computed tomography.

Due to the anatomical situation, intervertebral disc herniation usually results in compression of the anterior epidural space, with lateral or medial irritation of nerve root or cauda equina. Rare locations are an intra- or extraforaminal position or dislocation dorsally. Three patients with dorsal cauda equina compression caused by a sequestered herniated nucleus pulposus (HNP) are reported. The patients complained mainly of severe back pain. In two patients nondermatome-related leg pain was observed; one patient suffered additionally from incomplete cauda equina syndrome. In all cases magnet…

Infarction of the conus medullaris.

We encountered a patient with acute cauda equina syndrome. The clinical course, laboratory data and imaging study, including magnetic resonance imaging (MRI) strongly suggested that this episode was caused by a vascular disorder of the conus medullaris. We emphasized the diagnostic usefulness of MRI in the vascular disease of the conus medullaris as well as the clinical importance of this disease in the differential diagnosis of acute cauda equina syndrome.

Symptomatic lumbosacral perineural cysts: A report of three cases and review of literature.

Lumbosacral perineural cysts (Tarlov's cysts) are nerve root cysts, which are usually asymptomatic and are detected incidentally on imaging. These cysts are rare with an incidence of 4.6%. We report three cases of Lumbosacral Tarlov's cysts, which presented with cauda equina syndrome and radicular pain syndrome. Two of our patients had symptoms of cauda equina syndrome, and one had acute sciatica. Complete excision of the cyst was achieved in two patients and marsupialization of the cyst was done in another patient due to its large size and dense adherence to the sacral nerve roots. All the pa…

Chemodectoma of the cauda equina.

We report a new case of chemodectoma of the cauda equina, in a 52-year-old male who presented with low back pain and sciatica, then rapidly developed cauda equina syndrome. Magnetic resonance imaging demonstrated a tumor at the L2-L3 level. Complete excision was performed and the tumor was found to be a chemodectoma. Chemodectomas are rare neural crest tumors that are usually located at the neck. About 70 cases involving the cauda equina have been reported. Most are benign, although local recurrences occur in 4% of cases. A case with a cerebellar metastasis has been reported. Long-term follow-…
