Transmanubrial osteomuscular sparing approach: different indications.

The surgery of the cervico-thoracic inlet is a challenge for the thoracic surgeon. Several authors have described different approaches - with different degrees of invasiveness - to this anatomical area. We used the transmanubrial osteomuscular sparing approach described by Grünenwald and Spaggiari for six patients with five different indications: 1) left anterior Pancoast tumor; 2) glomus tumor originating from the inferior trunk (C8-T1 roots) of the right brachial plexus; 3) T1 vertebral tumor; 4) right internal jugular chain, Pirogoff confluence and subclavian artery metastatic lymph nodes f

Development of respiratory muscle contractile fatigue in the course of hyperpnoea.

To assess the development of inspiratory and expiratory muscle fatigue during normocapnic hyperpnoea, we studied fourteen healthy men performing 8min hyperpnoea, 6min pause, 8min hyperpnoea, etc., until task failure. Twitch transdiaphragmatic (P(di,tw)) and gastric (P(ga,tw)) pressures were measured during cervical and thoracic magnetic nerve stimulation, before hyperpnoea, after every 8min of hyperpnoea, and at task failure (i.e., at 25.3+/-4.7min). P(di,tw) decreased during the first 16min (-28+/-7%, p<0.001) and P(ga,tw) during the first 8min (-20+/-7%, p<0.001) of hyperpnoea without furthe

Is Routine Postoperative Anticoagulation Necessary in All Patients after First Rib Resection for Paget-Schroetter Syndrome?

BACKGROUND: Definitive treatment of Paget-Schroetter syndrome (PSS) involves first rib resection (FRR), division of the anterior scalene mus … thletes, 13 presented with documented pulmonary embolism in addition to local symptoms, and 3 had a cervical rib fused to the first rib. Patients underwent FRR a median of 50 (4 days to 18 years) days after their initial symptoms, and a median of 22 (1 day to 9 months) days after their percutaneous intervention. Postoperative VPT was required in 23 patients an …

High-energy blunt traumatic vertebro-vertebral arteriovenous fistula with retropharyngeal hematoma: a case report.

BACKGROUND: We report a case of high-energy blunt traumatic vertebro-vertebral arteriovenous fistula with retropharyngeal hematoma following C2 vertebral fracture after a motor vehicle crash. Although there is currently no established guideline to treat blunt traumatic vertebro-vertebral arteriovenous fistula, effective transarterial embolization to sacrifice the parent artery succeeded in complete obliteration of the vertebro-vertebral arteriovenous fistula with no ischemic complications. CASE PRESENTATION: A 74-year-old Japanese woman injured in a motor vehicle crash was transferred to our

Natural history of spinal anomalies and scoliosis associated with esophageal atresia.

OBJECTIVE: Clinical characteristics of spinal deformities that commonly occur in patients with esophageal atresia (EA) are unclear. The objective of this study was to assess the incidence and natural history of spinal anomalies and scoliosis in patients with EA. METHODS: A population-based cohort of 100 adults who had an operation for EA in our hospital were examined clinically and radiographically for spinal deformities. The results were compared with data obtained from normal population-based controls. RESULTS: Vertebral anomalies were observed in 45 patients, predominating in the cervical

Pancoast tumor presenting with multiple joint pains: a case report.

BACKGROUND: Pancoast tumors represent a unique subset of lung cancers wherein a primary neoplasm arises in the lung's apex and invades the surrounding soft tissues. One of the main challenges in the diagnosis and treatment of these apical lung cancers is that they are usually not visualized on initial chest x-ray and, by the time the patient presents with symptoms, the tumor has almost always invaded nearby structures. CASE PRESENTATION: Herein we report a case of a 58-year-old nonsmoking African American male who presented to the neurology clinic with a history of multiple chronic joint pain

Diagnosing a possible case of juvenile idiopathic arthritis: A palaeopathology study from the Ishkeenickh River Cave Site.

In the summer of 1967, a relatively complete skeleton of a juvenile was excavated from the Ishkeenickh River Cave Site (now the Ksi Hlginx River) in British Columbia, Canada, dating from approximately 2000 years ago. This individual, likely aged between 10 and 12 years at time of death, displays osteological features consistent with a chronic arthropathy. The most striking pathological changes are on the spine, with bony ankylosis present on several of the cervical and thoracic vertebrae. Lesions can also be observed on the ribs, the ilia, the femora, and a tibia. The arthritic conditions of j

A proposed inhalation reference concentration for methanol.

A biologically based approach was taken to developing an inhalation Reference Concentration (RfC) for methanol, a high production volume chemical with many commercial applications, including use as an alternative fuel for motor vehicles and as a hydrogen source for fuel cells. Benchmark Dose methodology was applied to the most sensitive toxic endpoint for assessing potential health risks in humans, cervical rib malformation data obtained using CD-1 mice. The concentration of methanol in circulating blood was employed as the dose metric, and the maximum likelihood estimate of the blood methanol…

Arnold-Chiari malformation and associated anomalies in a dicephalic newborn calf.

A full-term female newborn calf of the Japanese black short-horn breed which was two-headed (dicephalus) and had two incomplete necks was carefully dissected. The calf was one of twins, the other being a normal male. There was no record of reproductive problems with the cow. The left and right heads were of almost the same size and shape. Each head had a nose, mouth, eyeballs, and ears. The brains were very congested. That is to say, in both the left and right brains, the longitudinal fissure was unclear, and these was narrower pattern in the cerebral gyri. The surfaces of the two brains displ

Unilateral biportal endoscopy for the treatment of high-grade thoracic spinal meningioma: a case report and review of the literature.

Thoracic spinal segment intradural extramedullary (IDEM) tumors have the risk of disability, and early detection can usually be treated by surgery. The thoracic spinal canal is narrower than the cervical and lumbar spine, with ribs connected on both sides, so the operable space for surgery is narrow, intraoperative exposure is difficult, so it is worthwhile to summarize and improve how to completely resect the thoracic IDEM tumors, and to minimize the surgical-related neurological dysfunction. Currently, most of the surgical approaches are open laminectomy approach and microscopic surgery, and

Vertebral anomalies in craniofacial microsomia: a systematic review.

Craniofacial microsomia (CFM) is characterized by a heterogeneous underdevelopment of the facial structures arising from the first and second branchial arches, but extracraniofacial malformations such as vertebral anomalies also occur. This systematic review provides an overview of the literature on the types and prevalence of vertebral anomalies found in patients with CFM. A systematic search was conducted. Data on the number of patients, patient characteristics, types and prevalence of vertebral anomalies, and other associations between CFM and vertebral anomalies were extracted from the art

Morphological observations and morphometric analysis in three human fetuses with bilateral cervical cystic hygroma.

Three human fetuses (crown-rump length, CRL, ranging from 71 to 77 mm), presenting bilateral cervical cystic hygroma were examined. The specimens were cleared and double-stained with alcian blue and alizarin red S for detecting the ossification growth patterns in the vertebral column, ribs, ischium, limbs, and face. Longitudinal measurements of some long bones in the upper (humerus, ulna, radius) and lower (femur, tibia, fibula) limb were taken. The values of both the total length (TL) and the ossified part (OL) of each long bone, as well as the OL/TL per cent ratio were considered. Reference

A modified supraclavicular approach for resection of a subclavian aneurysm.

The incidence of right aneurysms of the innominate or subclavian arteries is not very high. The majority of these aneurysms is placed in the distal segment of subclavian arteries. These aneurysms are involved in the complex of the thoracic outlet compressive neurovascular syndrome, especially in occurrence of the cervical rib. All aneurysms in this region can rupture and embolize. This is the main purpose of surgical management of this clinical entity. The aim of this study is to refer an interesting case history of a relatively young patient with a right aneurysm of the middle portion of left…

A mechanistic approach for the calculation of intervertebral mobility in mammals based on vertebrae osteometry.

In this paper, we develop and validate an osteometry-based mechanistic approach to calculation of available range of motion (aROM) in presacral intervertebral joints in sagittal bending (SB), lateral bending (LB), and axial rotation (AR). Our basic assumption was the existence of a mechanistic interrelation between the geometry of zygapophysial articular facets and aROM. Trigonometric formulae are developed for aROM calculation, of which the general principle is that the angle of rotation is given by the ratio of the arc length of motion to the radius of this arc. We tested a number of alterna
