Radionuclide angiography and cholescintigraphy were performed with a bolus injection of technetium-99m disofenin in 65 patients with suspected acute cholecystitis. Acute cholecystitis was surgically confirmed in 23 of 25 cases in which radionuclide angiographic findings were positive (i.e., showed focal increased flow to the gallbladder region) (positive predictive value, 92%). Sensitivity and specificity of radionuclide angiography for detecting acute cholecystitis were 72% and 94% for cholescintigraphy. Three patients with scintigraphically visible gallbladders (at 45 minutes, 2.4 hours, and…

Acute acalculous cholecystitis (AAC) is usually seen as a complication of major surgery or trauma. Although this entity is well-known in the surgical literature, little has been written about it in the radiologic literature. A review of patient records from 1975 through 1982 revealed 16 patients with pathologically confirmed AAC on whom at least 1 sonographic study had been performed. Thickening of the gallbladder wall, a subserosal "halo" of edema, pericholecystic abscess, and marked gallbladder distention were consistent findings in AAC. In the proper clinical setting, these otherwise nonspe…

Three children presenting with HAV hepatitis had an initial clinical onset suggestive of acute cholecystitis (pain and guarding in the right hypochondrium, fever and delayed jaundice) associated with important ultrasonographic abnormalities, also very suggestive of acute cholecystitis: bladder wall thickness greater than 10 mm (3 cases), the presence of 2 or 3 layers of different echogenicities (3 cases), presence of an ultrasonographic Murphy's sign (one case), contents of the gallbladder echogenic (one case). The authors discuss the hypothesis of an actual initial acute cholecystitis.

Acute acalculous cholecystitis has been described following surgery unrelated to the biliary tract and after multiple trauma (DuPriest et al., 1979; Gately & Thomas, 1983; Herlin et al., 1982; Howard, 1981; Long et al., 1978; Orlando et al., 1983). Mortality rate is high, probably in part reflecting delayed diagnosis and treatment, since symptoms and signs of the primary disease often mask the symptoms of cholecystitis. Increased awareness of this rare condition might lead to more accurate diagnosis and treatment and thereby improved outcome. In this report three patients who developed acute a…

Acute cholecystitis is a surgical disease. However, if the operative indication is strict and univocal, the timing of operation still debatable. The purpose of this study of 343 calculous acute cholecystitis is to point out the marked tendency of the early surgical therapy. Such attitude may be encouraged by the observed change on roentgenographic studies' strategy (gallbladder opacification supplemented by echography) and the obtained good results.

The authors consider the questions involved in the treatment of acute cholecystitis. They show their casuistry and experiences. As to the therapeutical behaviour to be followed, they are inclined to an eclectic attitude adopted about acute cholecystitis, preferring an early or postposed operation, and never resorting to tardy operations.

Ultrasound findings in 2 cases of acute emphysematous cholecystitis are described. Reverberations were demonstrated within the acoustical shadow produced by gas in the gallbladder. Since reverberations are rarely seen within shadows produced by gallstones, a "reverberation shadow" arising from the gallbladder (in the absence of a biliary-enteric anastomosis or fistula) should suggest emphysematous cholecystitis; however, the diagnosis should be confirmed by abdominal radiography.

The clinical picture and morphological changes of the bile cyst were compared in 120 patients operated for acute cholecystitis. In 9% of cases no agreement was observed between the clinical picture of the disease and morphological changes of the bile cyst wall.

Gallstone formation around metallic foreign bodies is an unusual cause of acute cholecystitis. Two cases of penetrating abdominal trauma are described in which metallic fragments served as the nidus for calculus formation and later precipitated an acute abdominal problem.
