Abdominal Imaging Findings in COVID-19: Preliminary Observations.

Patients with a cholecystostomy tube placed (n = 4) had negative bacterial cultures. Conclusion Bowel abnormalities and gallbladder bile stasis were common findings on abdominal images of patients with coronavirus disease 2019. Patients who underwent laparotomy often had ischemia, …

Beware of the interval cholecystectomy.

BACKGROUND: Despite limited data regarding the indications and effectiveness of percutaneous cholecystostomy (PC) in the treatment of acute cholecystitis (AC), usage has increased by over 500% since 1994. Many of these patients subsequently undergo interval cholecystectomy (IC), a procedure that has not been rigorously evaluated. This aim of this study was to quantify the morbidity and mortality associated with the IC. METHODS: We included all consecutive adult patients (>18 years old) who underwent PC and IC from January 2008 to December 2013. Conversion rate, length of operation, biliary in…

Surgical management of acute cholecystitis.

BACKGROUND: Acute cholecystitis occurs in approximately 1% of patients with known gallstones. It presents as a surgical emergency and usuall … udies would suggest that cholecystectomy is more effective and of lower mortality than percutaneous cholecystostomy. There is not enough evidence to support the routine use of robotic surgery, single-incision laparoscopic cholecystectomy or natural orifice transluminal endoscopic surgery (NOTES) in the treatment of acute cholecystitis. CONCLUSIONS: Trial evidenc …

What the Eyes Don't See, the Heart Does Grieve Over.

INTRODUCTION: A 71 year old Peruvian woman presented with blurry vision and bilateral eye pain and had evidence of bilateral endophthalmitis on eye examination. Her past medical history was notable for multiple abdominal surgeries within the preceding months, including an incarcerated inguinal hernia repair which required an ileostomy placement, and cholecystitis requiring cholecystostomy tube placement. Over her multiple long hospitalizations, she developed bacteremia and fungemia on several occasions, with organisms including Enterobacter cloacae, Klebsiella pneumoniae, Bacteroides fragilis,…

Outcomes and Costs After Robotic vs Laparoscopic Complex Elective Cholecystectomy.

IMPORTANCE: As the use of robotic cholecystectomy increases, its value remains poorly defined for complex elective cholecystectomy (CEC), wh … . CEC was defined by preoperative criteria: prior aborted or partial cholecystectomy, presence of a cholecystostomy tube, and/or history of gallbladder perforation or fistula. EXPOSURE: Robotic-assisted vs laparoscopic cholecystectomy. MAIN OUTCOMES AND MEASURES: The primary outcome was a composite of an unplanned postoperative endoscopic retrograde cholangiopa …

Is live problem better than dead cert?

Cholecystoduodenal fistula is a type of biliary enteric fistula that can occur as a result of chronic cholecystitis. The majority of cases are diagnosed and their subsequent management planned intraoperatively. Previous reports have focused on successful laparoscopic management. We report three cases of elective cholecystectomy that were diagnosed intraoperatively with a cholecystoduodenal fistula (CDF). The first two cases were managed laparoscopically. In case 3 further continuation of surgery would have subjected the patient to significant risk of further harm. Instead the patient underwent

Mirizzi's syndrome: identification and management strategy.

Fourteen cases of Mirizzi's syndrome are presented here. Clinical presentation was pain (14), jaundice (14), fever (10) and peritonitis (1). A clinical diagnosis of choledocholithiasis was considered in all the patients. Pre-operative diagnosis of Mirizzi's syndrome was made in five patients on the basis of cholangiogram and the remaining cases were diagnosed at surgery. The stage (type) of Mirizzi's syndrome was based on the extent of erosion of the common bile duct. Four patients had type I, seven type II and three type III lesions. Associated choledocholithiasis was present in five and acut

Acute Acalculous Cholecystitis in the Setting of Negative Ultrasound and Computed Tomography Scan of the Abdomen.

Acute acalculous cholecystitis (AAC) is most commonly seen after surgery in critically ill patients. Early diagnosis and treatment is the ke … e high clinical suspicion, a HIDA scan performed which was positive for AAC. The patient then had a cholecystostomy tube placed by the interventional radiology team. The patient improved rapidly and was eventually discharged with a 14-day course of Bactrim DS (Roche Pharmaceuticals, Nutley, NJ) and metronidazole, and four weeks of outpatient follow up with genera …

EUS-guided gallbladder drainage for rescue treatment of malignant distal biliary obstruction after unsuccessful ERCP.

BACKGROUND AND AIMS: EUS-guided bile duct drainage (EUS-BD) is a well-recognized rescue biliary drainage method after unsuccessful ERCP. EUS-guided gallbladder drainage (EUS-GBD) was recently used to treat acute cholecystitis. The aim of this study was to assess the efficacy and safety of EUS-GBD for malignant biliary stricture-induced obstructive jaundice after unsuccessful ERCP as well as unsuccessful or impractical EUS-BD. METHODS: Between January 2006 and October 2014, 12 patients with obstructive jaundice due to unresectable malignant distal biliary stricture underwent EUS-GBD after ERCP

Percutaneous management of bile-plug syndrome: a case report.

Bile-plug syndrome is defined as an obstruction of the common bile duct by bile sludge in full-term infants. It is a correctable cause of obstructive jaundice in infants and is generally treated surgically. Here, we present a case of a 5-month-old infant with bile-plug syndrome, which was treated by percutaneous biliary interventions. To the best of our knowledge, percutaneous treatment of bile-plug syndrome is reported for the first time in an infant. This minimal invasive treatment modality may be a useful alternative to surgery in infants with bile-plug syndrome.

Impact of the Tokyo guidelines on the management of patients with acute calculous cholecystitis.

BACKGROUND AND AIM: Prompt treatments for acute calculous cholecystitis can reduce both mortality and morbidity. The aim of this retrospective study was to assess the impact of the Tokyo guidelines on management of patients with acute cholecystitis. METHODS: The records of patients admitted due to acute calculous cholecystitis were collected between January 2007 and June 2008. Exclusion criteria included acalculous, hepatobiliary malignancy, younger than 18 years old and mortality unrelated to cholecystitis. These 235 patients were classified into three groups; grade I, grade II and grade III

Gallstones. Who are good candidates for nonsurgical treatment?

Although surgery remains the ultimate treatment for symptomatic gallstone disease, the number of nonsurgical alternatives continues to grow. It is important to consider these options. The patient's age and overall health and the composition, number, and size of gallstones are major factors in determining whether alternative methods of therapy are appropriate. Many patients are eliminated from consideration when criteria for nonsurgical therapy are carefully followed. For example, oral dissolution therapy is relatively effective, but only about 20% to 25% of patients are candidates.
