Fluid assessment in dialysis patients by point-of-care magnetic relaxometry.

Magnetic resonance imaging (MRI) is a powerful diagnostic tool, but its use is restricted to the scanner suite. Here, we demonstrate that a bedside nuclear magnetic resonance (NMR) sensor can assess fluid status changes in individuals at a fraction of the time and cost compared to MRI. Our study recruited patients with end-stage renal disease (ESRD) who were regularly receiving hemodialysis treatments with intradialytic fluid removal as a model of volume overload and healthy controls as a model of euvolemia. Quantitative T 2 measurements of the lower leg of patients with ESRD immediately befor

Gingival bleeding and lower number of natural teeth are predictive factors of low muscle strength in obese adults: a cross-sectional study.

INTRODUCTION: Obesity is a systemic chronic disease associated with age-related skeletal muscle weakness, particularly in older adults. Chronic oral diseases share risk factors with critical non-communicable, chronic systemic diseases, including obesity. Poor oral health, increased fat mass, and muscle weakness are linked to adverse health outcomes and substantial economic burdens worldwide. This study developed predictive models of skeletal muscle strength (SMS) based on demographic characteristics, anthropometric and body composition measurements, blood biochemical examinations, and oral hea

Patient-Centered Medical Home Recognition and Diabetes Control Among Health Centers: Exploring the Role of Enabling Services.

The patient-centered medical home (PCMH) model has been considered a promising approach to improve chronic care delivery, particularly among patients with diabetes. There is theoretical support to suggest that certain nonmedical services, such as enabling services (eg, case management, social work, transportation), embedded within PCMH could be contributing to successful model implementation. It remains unclear whether PCMH recognition or enabling services are related to diabetes control. Federally Qualified Health Centers (FQHCs) are an important setting in which to study this relationship gi

Military veterans and behavioral medicine: introduction to the special issue.

This special issue of the Journal of Behavioral Medicine highlights emerging research at the intersection of military veteran populations and behavioral medicine. Military service, while associated with numerous strengths and opportunities for growth, also exposes service members and veterans to a range of unique psychological, behavioral, social, and physical health challenges across the life course. Veterans may experience elevated rates of a variety of mental and behavioral health conditions, including posttraumatic stress disorder, chronic pain, sleep disturbances, substance use disorders,

New to care: demands on a health system when homeless veterans are enrolled in a medical home model.

OBJECTIVES: We compared service use among homeless and nonhomeless veterans newly enrolled in a medical home model and identified patterns of use among homeless veterans associated with reductions in emergency department (ED) use. METHODS: We used case-control matching with a nested cohort analysis to measure 6-month health services use, new diagnoses, and care use patterns in veterans at the Providence, Rhode Island, Veterans Affairs Medical Center from 2008 to 2011. RESULTS: We followed 127 homeless and 106 nonhomeless veterans. Both groups had similar rates of chronic medical and mental h

Community engagement and chronic viral hepatitis public health interventions: a systematic review, meta-analysis, and complementary crowdsourcing open call.

BACKGROUND: Chronic viral hepatitis causes a high burden of morbidity and mortality, especially in low- and middle-income countries (LMICs). While community engagement, which involves stakeholders in addressing health-related issues, has shown promise to enhance hepatitis outcomes, evidence on its impact remains limited. To summarize the current state of knowledge on this topic we performed a systematic review and a crowdsourcing open call. METHODS: A parallel mixed-methods approach was used in this study. The systematic review included publications that evaluated a community-engaged interven

Colectomy Complicated by High-Output Ileostomy Managed in a Virtual Hybrid Hospital-at-Home Program.

Chronically ill patients with superimposed acute illness requiring hospitalization are more likely to develop an extended length of stay, hospital-acquired infections, and adverse events throughout their hospitalization. An excellent alternative to managing this population of patients in the traditional bricks-and-mortal (BAM) hospital is the hospital-at-home (HaH) model. The Advanced Care at Home (ACH) program is Mayo Clinic's HaH model that provides acute and postacute care to high-acuity patients in their homes rather than in the traditional hospital and skilled nursing facility. We report

Anxiety as a Psychological Modulator of Endogenous Pain Inhibition in Chronic Neck Pain: Implications for Precision Rehabilitation.

OBJECTIVE: Chronic neck pain (CNP) is a highly prevalent musculoskeletal disorder with significant personal and socioeconomic impact. In conjunction with low back pain, CNP accounts for the highest healthcare expenditures in the United States and contributes to 3551 disability‑adjusted life years per 100,000 population globally. This study explores the relationship between psychological factors and pain processing mechanisms, assessed through Quantitative Sensory Testing (QST), in individuals with CNP. DESIGN: Cohort study. SETTING: Hospital-based comprehensive outpatient rehabilitation cent

Long-term prediction of functional independence using adjusted and unadjusted single items of the functional independence measure (FIM) at discharge from rehabilitation.

CONTEXT: Being able to survive in the long-term independently is of concern to patients with spinal cord injury (SCI), their relatives, and to those providing or planning health care, especially at rehabilitation discharge. Most previous studies have attempted to predict functional dependency in activities of daily living within one year after injury. OBJECTIVES: (1) build 18 different predictive models, each model using one FIM (Functional Independence Measure) item, assessed at discharge, as independent predictor of total FIM score at chronic phase (3-6 years post-injury) (2) build three di

Transgenerational effects of social stress on social behavior, corticosterone, oxytocin, and prolactin in rats.

Social stressors such as depressed maternal care and family conflict are robust challenges which can have long-term physiological and behavioral effects on offspring and future generations. The current study investigates the transgenerational effects of an ethologically relevant chronic social stress on the behavior and endocrinology of juvenile and adult rats. Exposure to chronic social stress during lactation impairs maternal care in F0 lactating dams and the maternal care of the F1 offspring of those stressed F0 dams. The overall hypothesis was that the male and female F2 offspring of stres

Stress, autonomic imbalance, and the prediction of metabolic risk: A model and a proposal for research.

BACKGROUND: Devising novel prevention strategies for metabolic disorders will depend in part on the careful elucidation of the common pathways for developing metabolic risks. The neurovisceral integration model has proposed that autonomic imbalance plays an important role in the pathway from acute and chronic stress to cardiovascular disease. Though generally overlooked by clinicians, autonomic imbalance (sympathetic overactivity and/or parasympathetic underactivity) can be measured and modified by methods that are available in primary care. METHOD: This review applies the neurovisceral integ

Linear scleroderma: a Neuman nursing perspective.

Although quite a rare pediatric disease, linear scleroderma (LS), a chronic disease, may leave scarring and may have devastating effects on a child and family. This article provides an overview of the disease, presents a case history of a boy with LS, and discusses how nurses and other health-team members assisted the child and family in managing the illness. The description of the nursing care is based on the Neuman systems model and on the nursing diagnoses of fear, disturbed self-concept, and impaired physical mobility.

Evaluation of a behavioral medicine consultative treatment for chronic, ruminative vomiting.

This case study evaluated a multicomponent behavioral medicine treatment package for chronic ruminating and vomiting in a 13-year-old child with multiple disabilities. Treatment incorporated dietary and medication manipulations combined with behavioral intervention. The treatment package was designed and evaluated via a consultative model in a residential-care setting. The child's ruminative vomiting decreased steadily with implementation of treatment and reduced rates were maintained 2 years later. This case illustrates collaboration between physicians and psychologists in developing comprehe

A Generalist-Deliverable Bio-Psycho-Social Model for Pediatric Chronic Daily Headache: A 24-Month Retrospective Study.

BACKGROUND: Chronic daily headache (CDH) in youth is a functionally disabling and diagnostically heterogeneous condition that often requires multifaceted intervention. Real-world care is frequently fragmented-either limited to pharmacologic management or constrained by limited access to behavioral services. We evaluated the effectiveness of a generalist-deliverable bio-psycho-social (BPS) management model for children and adolescents with CDH, addressing both pharmacologic and contextual factors in the absence of subspecialty mental health services. METHODS: This retrospective, single-center
