A Decision-Making Algorithm for Remote Digital Assessments of Alzheimer's Disease.

INTRODUCTION: Remote digital assessments (RDAs) such as voice recording, video and motor sensors, olfactory, hearing, and vision screenings are now starting to be employed to complement classical biomarker and clinical evidence to identify patients in the early AD stages. Choosing which RDA can be proposed to individual patients is not trivial and often time-consuming. This position paper presents a decision-making algorithm for using RDA during teleconsultations in memory clinic settings. METHOD: The algorithm was developed by an expert panel following the Delphi methodology. RESULTS: The d

How frequently MRI modifies thoracolumbar fractures' classification or decision-making? A systematic review and meta-analysis.

PURPOSE: To provide the first meta-analysis of the impact of magnetic resonance imaging (MRI) on thoracolumbar fractures (TLFs) classification and decision-making. METHODS: A systematic review was conducted following PRISMA guidelines. We searched PubMed, Scopus, Cochrane, and Web of Science from inception to June 30, 2023 for studies evaluating the change in TLFs classification and treatment decisions after MRI. The studies extracted key findings, objectives, and patient population. A meta-analysis was performed for the pooled frequency of change in AO fracture classification or treatment de

Risk stratification for sudden death in congenital heart disease: bridging evidence, uncertainty, and individual decision-making.

PURPOSE OF REVIEW: Sudden cardiac death (SCD) remains a feared and difficult-to-predict outcome in patients with congenital heart disease (CHD). This review examines the latest evidence in risk stratification, with a focus on limitations of existing models and the mechanistic and statistical complexities that hinder individualized decision-making. RECENT FINDINGS: New multivariable risk scores for repaired tetralogy of Fallot and systemic right ventricle have improved prognostic resolution. Artificial intelligence-enabled ECG algorithms have shown promise in early identification of high-risk

Medical Decision-Making for Children in Families with Siblings: parental discretion and its limits.

This article examines how parents should make health decisions for one child when they may have a negative impact on the health interests or other interests of their siblings. The authors discuss three health decisions made by the parents of Alex Jones, a child with developmental disabilities with two older neurotypical siblings over the course of eight years. First, Alex's parents must decide whether to conduct sequencing on his siblings to help determine if there is a genetic cause for Alex's developmental disabilities. Second, Alex's parents must decide whether to move to another town to ma

Decision-Making in Surgery.

Surgeons face unique challenges in perioperative decision-making and communication with patients and families. In cardiothoracic surgery, the stakes are high, life and death decisions must be made quickly, and surgeons often lack a longstanding relationship with patients and families prior to intervention. This review considers specific challenges in the preoperative period followed by those faced postoperatively. While preoperative deliberation and informed consent focus on reaching a decision between 2 or more alternative approaches, the most vexing postoperative decisions often involve the

Surgical Pearls for Safe and Precise Decision-Making During Modified Selective Neurectomy Surgery.

In this short communication: (1) A transcutaneous electrical nerve stimulaton stimulator is a cost-effective solution for the predictable stimulation of mimicry and for precise decision-making. (2) Positioning a simple shatterproof mirror in front of an operated facial half enables the primary surgeon to simultaneously see and stimulate at the operative site and to scrutinize the mimicry for decision-making. (3) A misconception in the literature regarding colors used for tagging "branches for transection" and "branches for preservation" is clarified, and a third tag-color is suggested for time

Improving decision-making in complicated or rare cases-An approach towards Evidence-based Veterinary Medicine in small animal reproduction.

The concepts of Evidence-based Veterinary Medicine (EBVM) provide a methodological and systematic approach to include the best evidence from research into clinical decision-making. These concepts include steps as the search and assessment of relevant research findings and consideration of individual aspects. In addition, owners and other persons involved in animal health care should be included in shared decision-making. Some breeders have good basic knowledge concerning breeding management and characteristics of diseases and concerning advantages and disadvantages of different therapeutic app…

Ophtimus-V2-Tx: a compact domain-specific LLM for ophthalmic diagnosis and treatment planning.

Large language models (LLMs) show promise for clinical decision support but often struggle with case-specific reasoning. We present Ophtimus-V2-Tx, an 8-billion-parameter ophthalmology-specialized LLM fine-tuned on more than 10,000 case reports. Evaluation is conducted on a pre-collected dataset. Alongside text metrics (ROUGE-L, BLEU, METEOR) and a semantic similarity score, we use CliBench to map outputs to standardized codes (ICD-10-CM, ATC, ICD-10-PCS) and compute hierarchical F1 (L1-L4 and Full), with code mapping used strictly as an evaluation tool. Ophtimus-V2-Tx is competitive with a st

Medicine and heuristics: cognitive biases and medical decision-making.

INTRODUCTION: Clinical decision-making is a daily practice conducted by medical practitioners, yet the processes surrounding it are poorly understood. The influence of 'shortcuts' in clinical decision-making, known as heuristics, remains unknown. This paper explores heuristics and the valuable role they play in medical practice, as well as offering potential solutions to minimize the risk of incorrect decision-making. METHOD: The quasi-systematic review was conducted according to modified PRISMA guidelines utilizing the electronic databases Medline, Embase and Cinahl. All English language pap…

Facilitating active participation in anticoagulant decisions in advanced kidney disease: co-production of a question prompt list.

BACKGROUND: People with chronic kidney disease are at increased risk of thrombotic and bleeding episodes making anticoagulant treatment decisions challenging. Currently, there are no support tools for people with chronic kidney disease regarding anticoagulant therapy decisions. This work aimed to co-produce materials to support shared-decision making when considering anticoagulant use in advanced chronic kidney disease. METHODS: Focus groups were undertaken to explore the views of people with kidney disease towards anticoagulant prescribing. Data was thematically analysed based on Makoul and

The development and feasibility of a personal health-optimization system for people with bipolar disorder.

BACKGROUND: People with bipolar disorder often experience ill health and have considerably reduced life expectancies. Suboptimal treatment is common and includes a lack of effective medicines, overtreatment, and non-adherence to medical interventions and lifestyle measures. E- and m-health applications support patients in optimizing their treatment but often exhibit conceptual and technical shortcomings. The objective of this work was to develop and test the usability of a system targeting suboptimal treatment and compare the service to other genres and strategies. METHODS: Based on the frame

Factors Influencing Physicians' Code Status Decision-Making: An Experimental Vignette-Based Survey.

RATIONALE: Worldwide hospitals increasingly require a code status to be established for every inpatient. Establishing a code status should be a shared decision-making process between the inpatient and the hospital physician. However, there is no consensus on how code status should be established at admission and which factors influence the decision-making process. AIMS AND OBJECTIVES: To identify physician and patient characteristics influencing physicians' code status decision-making. METHODS: Experimental, vignette-based survey in a tertiary university hospital, using a self-report online

[Frailty assessment strengthens decision-making about treatment level and palliative care].

Frailty is common among older adults admitted with multimorbidity and functional decline and has major implications for prognosis, treatment goals and quality of life. Systematic frailty assessment can support clinical and ethical decision-making, facilitate timely palliative care and prevent overtreatment. When combined with structured conversations about goals of care and interdisciplinary collaboration, this review finds, frailty assessment enables more person-centred and dignified care trajectories across sectors.

The Role of Clinical Judgment in Psychiatry.

Clinical judgment is currently perceived as an intuitive art that is going to be replaced by growing technology and artificial intelligence. Even though patients look for good clinical judgment when they seek medical attention and clinicians rely on it, the topic is seldom mentioned and discussed in the literature. In their everyday practice, psychiatrists use observation, description, and classification; test explanatory hypotheses; and formulate clinical decisions based on clinical judgment. The aim of this review was to examine the current role of clinical judgment in psychiatry. We first o
