OBJECTIVE: To evaluate the effectiveness of ultrasound guided injection of botulinum toxin A into the muscles of mastication; the masseter, temporalis and the lateral pterygoid, in a closed reduction approach of condylar and sub-condylar fractures, to optimize fracture reduction, reduce the length of intermaxillary fixation (IMF) and to facilitate postoperative functional recovery.
MATERIALS AND METHODS: Sixteen medically healthy patients aged 18-35 years, presenting with unilateral subcondylar fracture were randomly assigned into two groups. The randomized controlled clinical trial was compl

The authors describe their experience with treatment of two neurologically intact male patients, aged 70 and 74 years, presenting with combined lateral atlanto-axial dislocation and odontoid type II fracture. The mechanism of these two craniovertebral junction injuries had been lateroflexion of the neck. The initial attempt at closed reduction using axial traction failed. The authors succeeded with a closed reduction maneuver under general anaesthesia in the first patient in whom long-term stability was achieved by transarticular C2-C1 fixation, together with interlaminar fusion. In the second

CASE: A 12-year-old boy with an isolated both bone forearm fracture was treated with closed reduction and flexible intramedullary nailing 11 days postinjury and after the loss of initial fracture alignment. On nail insertion, an intraoperative distal radial physeal fracture was encountered, thereby warranting modification in treatment.
CONCLUSION: Physeal injury is a rare intraoperative complication of intramedullary fixation of forearm fractures. This report provides insight regarding the management and prevention of this rare complication.

Displaced fractures at the distal end of the middle phalanx require reduction if function is to be restored at the distal joint. Fractures at the base of the distal phalanx take on additional significance because of the insertions of the flexor and extensor tendons. The palmar articular fracture, when displaced, needs surgical replacement to restore flexor profundus function. In contrast, the displaced dorsal lip fracture, the mallet fracture, can be treated closed with the expectation of excellent results.

The Bosworth fracture is a pattern of fracture-dislocation of the ankle joint with fixed dislocation of the proximal part of the fibula behind the posterior malleolus. It is a rare injury, which requires high index of suspicion for diagnosis. Fixed dislocation of the fibula does not allow for closed reduction, so open reduction and fixation of fibula or tibia is required. We present a case report of a patient who suffered a Bosworth fracture, with associated medial malleolus fracture. She underwent surgical treatment with open reduction and internal fixation of the fibula and tibia. We also pe

Distal ulnar physeal fractures are uncommon in pediatric patients, and isolated Salter-Harris IV (SHIV) injuries of the distal ulna are particularly rare. These injuries may raise concern for distal radioulnar joint (DRUJ) incongruity, limited forearm rotation, and future growth disturbance. Management strategies vary depending on fracture displacement, reducibility, and joint stability. A 14-year-old male patient presented with left wrist pain three days after a bicycle accident. Examination demonstrated tenderness over the distal ulna and restricted wrist and forearm motion, including pronat

BACKGROUND: Optimal management for a pulseless supracondylar humerus fracture associated with anterior interosseous nerve (AIN) or median nerve injury is unclear. The purpose of this study was to determine the incidence of pulseless supracondylar humerus fractures associated with AIN or median nerve injury, to assess open versus closed surgical management, to determine factors associated with the need for neurovascular intervention, and to report the outcome.
METHODS: A retrospective review was performed at 4 pediatric trauma hospitals on all patients who sustained a Gartland III or IV suprac

PURPOSE: To evaluate the correlation between avascular necrosis (AVN) and the amount (severity) and direction (translation and angulation) of initial displacement of pediatric femoral neck fractures.
METHODS: We retrospectively reviewed 108 pediatric patients (mean age 10.3 ± 4.1 years) with femoral neck fractures. The amount of initial translation (T) and angulation (A) was measured on anteroposterior (AP; TAP% and AAP) and lateral (TL% and AL) radiographs. The direction of translation was determined on AP (medial or lateral) and lateral radiographs (anterior or posterior). Furthermore, the

Phalangeal fractures are common among foot injuries, with the majority amenable to conservative treatment. This case involved a fracture of the toe combined with intra-articular sesamoid entrapment. To prevent nonunion, surgical intervention was recommended. We present a rare case of distal phalanx fracture of the toe with interphalangeal joint sesamoid entrapment, review the intraoperative challenges encountered, and summarize postoperative insights with recommendations. We report a 41-year-old female construction worker who presented with a crush injury to her right great toe. Diagnosis was

BACKGROUND: This retrospective comparative study was performed to evaluate the clinical outcomes after surgical treatment for Bosworth fracture-dislocation, and to analyse the prognostic factors related to postoperative outcomes.
METHODS: Fifteen patients were followed for ≥2 years after Bosworth fracture-dislocation. Twenty-five ankle fracture-dislocations as control group were enrolled to compare clinical outcomes. Clinical evaluation consisted of the AOFAS and Olerud-Molander scores. Patient and injury factors were analysed to identify the outcome predictors.
RESULTS: There were no signif

Only four cases of triplane distal radius fracture have been described in the literature. We report the first that has been treated with open reduction and internal fixation. A 15-year-old boy sustained a triplane fracture of the distal radius. After an unsuccessful attempt at closed reduction, open reduction and internal fixation were performed. Six months after injury, the patient is asymptomatic, but radiographs show a partial growth arrest. The goal of treatment is to achieve anatomic reduction of the articular surface. In this case interposed periosteum prevented anatomic reduction, requi

Traumatic fracture-dislocation of the hip usually warrants prompt management by closed manipulative reduction. In some patients, debris malpositioned between the femoral head and the acetabular dome obstructs a completely concentric reduction of the injured hip. To avoid damage to the articular surfaces, the debris between them should be removed in a timely fashion. Techniques for removal include open approaches with or without fracture fixation or hip arthroscopy. Fracture fixation and hip arthroscopy have associated risks and potential complications, may require special equipment, and may no

Fracture of the femoral head after hip dislocation is a relatively rare injury and is often associated with a poor functional outcome. Most result from high impact motor vehicle accidents. We report the case of a soldier who trapped his foot in a traversing Warrior turret and suffered a severe abduction injury to his hip. He sustained a Pipkin Type I fracture-dislocation of the hip not amenable to closed reduction and required transfer to the American Hospital in Baghdad where he underwent emergency open reduction and internal fixation of the fracture. This case report illustrates an unusual m

Diaphyseal fractures of the metacarpals and phalanges are common injuries that can lead to impairment of hand function. The fracture pattern and soft-tissue injury vary with the mechanism of injury. The imbalance of the flexor and extensor forces created by displaced fractures will often produce a secondary angulatory deformity. Nonoperative treatment is indicated for reducible and stable fracture configurations. Irreducible or unstable fracture patterns require open or closed reduction and fixation. Reduction must be assessed in flexion and extension to ensure correct rotatory alignment. Frac

The treatment of subcondylar mandible fractures is a topic of debate and can be variable even though these fractures are commonly seen. Historically, the treatment algorithm was between open reduction and closed treatment. Now, recent technical advances regarding the use of the endoscope in the field of craniofacial surgery provide additional treatment options. This article aimed to evaluate 3 current management strategies: closed reduction with maxillomandibular fixation, open reduction with internal fixation, and endoscopic-assisted reduction with internal fixation. We present our rationale

Twenty-four patients underwent attempted closed or limited open reduction of displaced acetabular fractures. If reduction was successful, the fractures were stabilized with percutaneous screws. Group 1 was composed of elderly patients with complex fractures and radiographic findings that were felt to be predictive of post-traumatic arthritis. In these patients, percutaneous screw fixation was used to improve fracture anatomy, allow mobilization and total hip replacement later, if necessary. In group 1, anatomical reduction was not felt to be a necessity. Group 2 was composed of young patients

The authors report four cases of Jeffery type 2 lesions in children. Despite the probable mechanism of such lesions, none of the cases reported here displayed a dislocation of the elbow. In the first observation the diagnosis was suspected only 15 days after injury. Two other cases were initially treated by closed external manipulations, one with percutaneous Kirschner wire leverage and the other with closed intramedullary pinning, but both cases displayed a 180 degree reversal of the radial head, suspected only secondarily. The first exhaustive review of the literature on the subject, shows t

A male child aged three years and three months presented after stubbing his right fifth toe. Imaging revealed a dorsolateral dislocation of the proximal interphalangeal joint (PIPJ). After failed attempts at closed reduction, open reduction and internal fixation was pursued. At the one-year follow-up, the patient was found to be doing well clinically and radiographically. These types of injuries require a high degree of clinical suspicion to obtain the proper imaging. The interposition of adjacent soft tissues can render these injuries irreducible. When irreducible, open reduction and pin fixa
