Motor intrusion errors as a novel marker of neurodegeneration and executive dysfunction in Alzheimer's disease.

Objective: With the development of disease-modifying therapies for Alzheimer's disease, continued clarification of clinical indicators of disease progression is needed. Based on prior observations of motor intrusion errors in amyloid-positive individuals, we sought to characterize differences in clinical, cognitive, and neuroimaging variables between amyloid-positive participants with and without motor intrusions. Method: We examined 284 amyloid-positive participants with mild cognitive impairment or mild dementia evaluated within a multidisciplinary memory clinic setting. Participants with an

Rumination in response to repugnant obsessions: Catching the sneakiest of compulsions.

Rumination is a mental process characterized by the repetitive analysis of concerns without taking concrete or helpful action. It has been shown to be transdiagnostic, contributing to the maintenance and exacerbation of symptoms across various mental disorders, including obsessive-compulsive disorder (OCD). Historically conceptualized as part of the obsessional domain due to its repetitive and intrusive qualities, rumination is now better understood as a covert compulsion-one that reinforces the overimportance of otherwise normal, unwanted intrusions, particularly in the case of repugnant obse

DELIRIUM IN THE ELDERLY.

Delirium is a common and serious neuropsychiatric syndrome in older adults, characterized by acute and fluctuating disturbances in attention, awareness, and cognition. It is associated with multiple adverse outcomes, including increased mortality, functional decline, long-term cognitive impairment, and institutionalization. This review synthesizes current knowledge on the pathophysiology, risk factors, clinical presentation, diagnosis, prevention, treatment, and prognosis of delirium in elderly patients. Delirium arises from complex interactions between predisposing vulnerabilities (such as de

Longitudinal associations between posttraumatic stress disorder, separation anxiety, and rumination among adolescents: Disentangling within-person and between-person associations.

OBJECTIVE: Posttraumatic stress disorder (PTSD) and separation anxiety often show high comorbidity after trauma, which complicates the course of disease and treatment response. However, it is unclear how PTSD and separation anxiety comorbidity occur. According to the cognitive model of PTSD, rumination may be a key cognitive process linking the two constructs. Thus, this study aimed to examine the relation between PTSD and separation anxiety and to assess the mediating role of rumination in this relation among adolescents after trauma. METHOD: A follow-up survey was conducted among 204 adoles

Does disrupting the orbitofrontal cortex alter sensitivity to punishment? A potential mechanism of compulsivity.

Abnormal orbitofrontal cortex (OFC) activity is one of the most common findings from neuroimaging studies of individuals with compulsive disorders such as substance use disorder and obsessive-compulsive disorder. The nature of this abnormality is complex, however, with some studies reporting the OFC to be over-active in compulsive individuals relative to controls, whereas other studies report it being under-active, and a further set of studies reporting OFC abnormality in both directions within the same individuals. The OFC has been implicated in a broad range of cognitive processes such as de

Personality and Alzheimer's disease: An integrative review.

The objective of this article is to review and integrate interrelated areas of research on personality and Alzheimer's disease (AD). Prospective studies indicate that individuals who score higher on conscientiousness (more responsible and self-disciplined) and lower on neuroticism (less anxious and vulnerable to stress) have a reduced risk of developing dementia, even in the presence of AD neuropathology. Personality is also related to measures of cognitive performance and cognitive decline, with effect sizes similar to those of other clinical, lifestyle, and behavioral risk factors. These ass

Trauma-related psychiatric and behavioral conditions are uniquely associated with sustained attention dysfunction.

OBJECTIVE: It is increasingly recognized that trauma victims, particularly Veterans, have co-occurring psychological and physical conditions that impact cognition, especially the domains of sustained attention and executive functioning. Although previous work has generally attempted to isolate the unique cognitive effects of common combat-related comorbidities, less work has been done to examine how these conditions co-occur, and whether unique cognitive signatures accompany certain clinical combinations. METHOD: To address this gap, we examined how several deployment-related conditions were

A measure of cognitions specific to seasonal depression: Development and validation of the Seasonal Beliefs Questionnaire.

We introduce the Seasonal Beliefs Questionnaire (SBQ), a self-report inventory of maladaptive thoughts about the seasons, light availability, and weather conditions, proposed to constitute a unique cognitive vulnerability to winter seasonal affective disorder (SAD; Rohan, Roecklein, & Haaga, 2009). Potential items were derived from a qualitative analysis of self-reported thoughts during SAD-tailored cognitive-behavioral therapy (CBT-SAD) and subsequently refined based on qualitative feedback from 48 SAD patients. In the psychometric study (N = 536 college students), exploratory and confirmator

Peer mentorship for physically injured veterans in healthcare settings during ongoing military conflict: benefits and barriers.

BACKGROUND: Peer mentorship by individuals with lived experience is increasingly recognized as supporting recovery after severe injury. Yet evidence on its role in hospital-based rehabilitation remains limited, particularly in cases of severe war-related physical injury during ongoing military conflict. This study examines the benefits and barriers of peer mentorship within the Israel Defense Forces Casualties Affairs Unit, which trains previously injured veterans to accompany newly injured soldiers during hospitalization in healthcare settings as part of their reserve duty. METHODS: The stud

A Pilot Study of Changes in Medial Temporal Lobe Fractional Amplitude of Low Frequency Fluctuations after Sildenafil Administration in Patients with Alzheimer's Disease.

Alzheimer's disease (AD) is the most common cause of neurodegenerative cognitive impairment, defined by abnormal accumulations of amyloid-β and tau. Approaches directly targeting these proteins have not resulted in a disease modifying therapy. Neurovascular unit dysfunction is a feature of AD offering an alternative target for intervention. Sildenafil, a phosphodiesterase 5 (PDE5) inhibitor, improves cognitive functioning in mouse models of AD. Recent work in AD patients has demonstrated increased cerebral blood flow, as well as brain oxygen utilization after a single dose of sildenafil. Its e

Factors impacting bladder underactivity and clinical implications.

Lower urinary tract symptoms in the voiding phase can be due to an underactive bladder, but are usually similar to symptoms of infravesical obstruction or dysfunctional voiding. The underactive bladder can be caused by an impaired detrusor contraction but also by a derangement of local and central neuro-cognitive regulatory systems or an impairment of bladder sensation. Potential risk factors of bladder underactivity include: ageing, diabetes, neurogenic disease, cardiovascular disease, obstruction and psychological causes. Comprehensive diagnostic and detection techniques for an underactive d
