Middle mesenteric artery: Angiographic and three-dimensional computed tomography findings.

The middle mesenteric artery, also known as the third mesenteric artery, is a very rare anomaly. Several anatomical variations of middle mesenteric artery have been reported; in these reports, the right colic artery and/or middle colic artery often originate directly from the aorta. Here, we report a middle mesenteric artery in which the middle colic artery originated directly from the abdominal aorta. We also provide three-dimensional computed tomography and angiography findings and discuss anatomical and embryological considerations.

Ursodeoxycholic acid exerts no beneficial effect in patients with symptomatic gallstones awaiting cholecystectomy.

Ursodeoxycholic acid (UDCA) and impaired gallbladder motility purportedly reduce biliary pain and acute cholecystitis in patients with gallstones. However, the effect of UDCA in this setting has not been studied prospectively. This issue is important, as in several countries (including the Netherlands) scheduling problems result in long waiting periods for elective cholecystectomy. We conducted a randomized, double-blind, placebo-controlled trial on effects of UDCA in 177 highly symptomatic patients with gallstones scheduled for cholecystectomy. Patients were stratified for colic number in the…

A SICE (Società Italiana di Chirurgia Endoscopica e Nuove Tecnologie) observational prospective multicenter study on lymphadenectomy during right hemicolectomy: Should anatomical variability of the right colic artery influence the surgical strategy?-CoDIG 2 database (ColonDx Italian Group).

Colon cancer is a worldwide common disease in both gender. Surgery is the best option for the treatment of advanced colon cancer without distant metasisis, but some aspects are still debated, such as the extent of lymphadenectomy. In Japanese guidelines the gold standard was D3 dissection to remove the central lymphonodes (203,213,223), but in 2009 Hoenberger et al. introduced the concept of complete mesocolic excision (CME) in which surgical dissection should follow the embryological planes in order to remove mesentery entirely. This way to prevent leakege of cancer cells and collect more lym…

Cecal rupture by Anoplocephala perfoliata infection in a thoroughbred horse in Seoul Race Park, South Korea.

A 7-year-old Thoroughbred horse was admitted to the Equine Hospital, Korea Racing Association with signs of colic. Based on the size of impactions, the clinical signs, the results of abdominal paracentesis and medical treatment, the prognosis was poor. The horse died 3 hours later following hopeless discharge. At necropsy, the caecum and large colon were fully filled with fecal contents and there was a rupture (10 cm in dia) in the latero- ventral caecum. The mucosa of the ileo-caecal and caeco- colic valves appeared to the hyperemic, edematous and ulcerous. There were many tapeworms in the af…

Monitoring acute equine visceral pain with the Equine Utrecht University Scale for Composite Pain Assessment (EQUUS-COMPASS) and the Equine Utrecht University Scale for Facial Assessment of Pain (EQUUS-FAP): A validation study.

This study presents the validation of two recently described pain scales, the Equine Utrecht University Scale for Composite Pain Assessment (EQUUS-COMPASS) and the Equine Utrecht University Scale for Facial Assessment of Pain (EQUUS-FAP), in horses with acute colic. A follow-up cohort study of 46 adult horses (n = 23 with acute colic; n = 23 healthy control horses) was performed for validation and refinement of the constructed scales. Both pain scales showed statistically significant differences between horses with colic and healthy control horses, and between horses with colic that could be t…

[Symptomatic gallstone disease: an indication for surgery].

Three patients, men in the ages of 58, 66 and 56 years, respectively, had experienced 'warning colics' a considerable time before gallstone complications or severe recurrent colic. Ultrasonographically proven gallstones had not led to cholecystectomy. The 58-year-old man died of sepsis due to infected pancreatic necrosis; the other men underwent laparoscopic cholecystectomy, after which they recovered fully. Approximately 10-5% of the adult Dutch population have gallstones, but only 10% will develop symptoms. The annual risk for developing complicated gallstone disease is 1-2% in asymptomatic …

Rupture of abdominal aortic aneurysm into sigmoid colon: a case report.

Primary aorto-colic fistula is rarely reported in the literature. Although infrequently encountered, it is an important complication since it is usually fatal unless detected. Primary aorto-colic fistula is a spontaneous rupture of abdominal aortic aneurysm into the lumen of the adjacent colon loop. Here we report a case of primary aorto-colic fistula in a 54-year old male. The fistulated sigmoid colon was repaired by end-to-end anastomosis. Despite inotropic support, the patient died of sepsis and multiorgan failure on the first postoperative day.

An anatomic basis to prevent ischemia of the colon during operations upon the aorta.

According to standard practice, the inferior mesenteric artery, when sacrificed, should always be ligated at its origin, proximal to the left colic branch. However, it should not be sacrificed and ligated but instead, be reimplanted if any doubt exists regarding the adequacy of the collateral circulation. The patient is in a high risk category for development of ischemia of the colon if there is an occlusion or absence of the marginal artery at the splenic flexure; absence of the middle colic artery; occlusion of one or both hypogastric arteries, or enlargement of the left colic artery, sugges…

Ultrasound of the equine acute abdomen.

Abdominal ultrasound is an invaluable aid in the evaluation of the colic patient but can be heavily influenced by patient preparation, individual horse-to-horse variation, availability of ultrasound transducers, technique, experience level of the examiner, and complexity of the abdominal disorder. This article describes ultrasonographic anatomy of the normal equine abdomen and technique for examination of the equine colic patient. Common abnormalities of the stomach, small intestine, large intestine, and peritoneal cavity are described along with other abnormalities that may be discovered with…

Analysis of vascular anatomy and lymph node metastases warrants radical segmental bowel resection for colon cancer.

To determine the indications for limited colon cancer surgery in each location, we reviewed the arterial branching patterns and lymph node metastases along the course of specific vascular trunks in 344 colon cancer patients who had undergone preoperative angiography and colectomy with lymph node dissection. Our conclusions are follows: Because the ileocecal artery always arises from the superior mesenteric artery and lymph node metastases of cecum cancer were limited to nodes along the ileocolic artery, cecum cancer can be cured by ileocecal resection. The right colic artery has various origin…

[Angiographic study of the colonic vessels with a view to esophagoplasty].

Variants of the colic vessels were studied angiographically in 34 cadavers, drawing up an original classification of the colic arteries arising from the lower mesenteric artery. Sixteen variants of the left colic artery were found, which were grouped into five main types, the classical type being encountered in only 22.6% of the cases. In one case the lower mesenteric artery was absent. In coloesophagoplasty better results were obtained with the isoperistaltic transverse colon than with the upper ileocolon in 64% of the cases and similar results in 20%; in 16% of the cases the latter operation…

Lymph node metastases in splenic flexure colon cancer: Is subtotal colectomy warranted?

BACKGROUND AND OBJECTIVES: Tumors of the splenic flexure (TSF) can be associated with metastatic lymph nodes (LN) along the left colic pedicle, but also along the superior mesenteric vessels. We aimed to detail the anatomical distribution of metastatic LNs in patients undergoing elective subtotal colectomy for TSF. METHOD: Between 2000 and 2016, 65 patients were included. At pathological analysis, LNs were classified into two groups: locoregional LN (along the left colic artery) and distant LN (along the middle colic, right colic, and ileocolic arteries). RESULTS: The median number of LNs ex…

Anomalous inferior mesenteric artery supplying the ascending, transverse, descending, and sigmoid colons.

We have encountered in our anatomical practice the first case and an extremely rare second case in which the ascending, transverse, descending, and sigmoid colons were supplied by the inferior mesenteric artery. The causes of colic artery anomalies are generally explained in conjunction with the development of the superior mesenteric artery, which is intimately related to embryonic elongation and midgut rotation. However, this embryological model was inapplicable to both cases. This difficulty motivated us to seek possible relationships with reported anomalous inferior mesenteric arteries in a…

Diagnosis of papillary stenosis by calibration: follow-up 15 to 25 years after sphincteroplasty.

In 178 patients undergoing elective cholecystectomy for biliary colic, the papilla was calibrated with a no. 10 French catheter through the cystic duct. The catheter passed through the papilla into the duodenum in 94 patients. Six (6 percent) had common duct stones. No postcholecystectomy colic occurred. In 37 patients the catheter was obstructed by papillary stenosis and sphincteroplasty was done. Common duct stones occurred in 17 patients (46 percent), jaundice in 7 and dilatation of the common duct in 6. Cholangiograms failed to detect small stones in seven patients and were of minimal valu…

Protocol digest of a randomized controlled trial comparing transverse colectomy with extended right hemicolectomy for Stage II/III transverse colon cancer: JCOG2304 (TACTICS).

Transverse colon cancer is traditionally treated with extended right hemicolectomy (EHC), which involves division of the middle colic, right colic, and ileocolic arteries, along with removal of the terminal ileum and right colon. Transverse colectomy (TC) is a limited resection removing only 10 cm of bowel on either side of the tumor, along with resection of the middle colic artery and regional lymphadenectomy. This approach may contribute to the preservation of bowel function without compromising oncologic adequacy. This multicenter, randomized, phase III trial is designed to evaluate the non…

Compression of cystic duct caused by focal nodular hyperplasia.

A 29-year-old female presenting with symptoms of biliary colic was found to have a liver mass compressing the cystic duct. Due to the anatomical placement of the growth, the compressed duct produced symptoms mimicking acute cholecystitis. The mass was diagnosed as focal nodular hyperplasia (FNH) upon biopsy. FNH is commonly found incidentally with nonhepatic clinical presentation or during an unrelated surgical procedure. The scope of this paper is to bring awareness to uncommon causes of biliary colic. To our knowledge, there has been one other paper published with FNH being the primary cause…
