Safety and feasibility of laparoscopic colorectal cancer surgery in the geriatric age group: A single-centre outcome.

BACKGROUND: The increasing prevalence of colorectal cancer among the elderly necessitates exploring surgical options that minimise trauma and expedite recovery. Laparoscopic colorectal surgery has emerged as a promising approach, yet its safety and efficacy in patients aged 65 and above require further investigation. This study aims to evaluate the perioperative outcomes, oncologic adequacy and post-operative recovery associated with laparoscopic colorectal cancer surgery in patients aged 65 years and older. PATIENTS AND METHODS: A retrospective analysis was conducted on elderly patients who …

Mapping the evolving trends of robotic surgery in gastric and colorectal cancer: a clinical trial landscape analysis.

Robot-assisted surgery provides important support for the treatment of gastric cancer (GC) and colorectal cancer (CRC) with its minimally invasive advantages. Through a systematic analysis of 188 clinical trials registered on ClinicalTrials.gov, this study aims to synthesize the research landscape in the field, identify evidence gaps, and provide a basis for optimizing clinical practice and guiding the design of future clinical trials. The analysis of 71 interventional trials with robotic surgery as the core intervention shows that the research activity has increased significantly after 2020.

Selective internal radiation therapy as a bridge to curative-intent surgery in bulky colorectal liver metastases.

Achieving secondary resectability remains a major challenge in patients with initially unresectable colorectal liver metastases (CRLM). We report a 46-year-old patient with rectal adenocarcinoma and three large synchronous unresectable liver metastases who achieved a durable response following an intensive multimodal strategy. FOLFOX plus panitumumab induced partial regression, while selective internal radiation therapy (SIRT) led to extensive tumor necrosis, enabling curative-intent extended left hepatectomy combined with microwave ablation. The patient remains in remission without systemic t

Psychometric Assessment of PROMIS-29 as a Measure of Recovery After Colorectal Surgery: A Prospective Cohort Study.

BACKGROUND: The PROMIS-29 questionnaire assesses general aspects of physical and mental health that may be relevant to surgical recovery. Although this tool has been endorsed by expert consensus for use in perioperative care, evidence regarding its psychometric performance in this context is limited. This study aimed to assess the content validity, internal consistency, construct validity, and responsiveness of PROMIS-29 as a measure of recovery after colorectal surgery. METHODS: This study was conducted according to the COnsensus-based Standards for the selection of health Measurement INstru…

The Prognostic Impact of the Number of Colorectal Liver Metastases At the Time of Presentation Among Patients Undergoing Surgery.

BACKGROUND: The prognostic impact of tumor number among surgically treated colorectal liver metastases (CLMs) remains incompletely understood, particularly in patients with extensive disease. METHODS: We retrospectively analyzed 303 patients who underwent initial hepatectomy for CLMs between 2008 and 2024. Prognostic factors for overall survival (OS) were evaluated using multivariable Cox regression analysis. Estimated mortality hazard at three and five years after surgery was modeled according to the number of liver lesions. RESULTS: Tumor number (hazard ratio [HR], 1.02; 95% confidence int

Laparoscopic surgery for colorectal cancer in patients with liver cirrhosis: a comparative study on risk of open conversion.

BACKGROUND: Laparoscopic colorectal cancer (CRC) surgery is widely accepted due to its advantages over open surgery. However, its safety and feasibility in patients with liver cirrhosis (LC) remain unclear. This study aimed to evaluate the surgical outcomes of laparoscopic CRC surgery in LC patients compared to non-cirrhotic patients, focusing on the rate of open conversion. METHODS: This retrospective study analyzed 2523 patients who underwent elective laparoscopic CRC resection between January 2015 and November 2022 at a tertiary hospital. Fifty-five patients with LC were compared with non-

Impact of metabolic liver disease on colorectal cancer and liver metastases: Pathophysiology and implications for hepatobiliary surgery.

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) is increasing globally and affects a growing proportion of patients with colorectal cancer (CRC). Beyond shared risk factors such as obesity and metabolic syndrome, MASLD appears to actively shape a pro-metastatic hepatic microenvironment, with important implications for colorectal liver metastases (CRLM), particularly in the surgical setting. METHODS: This narrative review synthesizes epidemiologic, mechanistic, imaging, and clinical data on the interplay between MASLD and CRC/CRLM, with a focus on hepatobiliary sur

Economic, resource-use, and adoption signals in the robotic colorectal surgery literature: a bibliometric and visualization study.

Robotic colorectal surgery has expanded from technical feasibility to broad clinical adoption, but its economic value remains context dependent because costs interact with case selection, conversion, complications, length of stay, long-term outcomes, and health-system structure. We performed a value-oriented bibliometric and visualization analysis of Web of Science Core Collection records retrieved on August 7, 2026, using search concepts spanning economic value, resource use, adoption, implementation, access, and equity. English-language Articles and Review Articles were analyzed with R/bibli…

Benchmarking to validate the implementation of a robotic colorectal cancer surgery program. Proposal for a new method to createbenchmarks.

Benchmarking facilitates the assessment of surgical quality by comparing outcomes with standards of excellence. However, robotic colorectal cancer surgery (RCCS) benchmarks remain limited. The objective of this study is to assess whether the outcomes of our RCCS program meet benchmark standards and to propose a new method to create benchmarks. We conducted a retrospective observational study of 97 patients with colorectal tumors who underwent robot-assisted surgery between June-2019 and May-2024. Results for rectal cancer (n = 45), colon cancer (n = 47), and a subgroup of right hemicolectomy (

Evaluating a digital prehabilitation tool in patients with colorectal surgery: protocol for a multisite randomised controlled trial.

INTRODUCTION: Colorectal cancer is a leading cause of cancer mortality in the USA and occurs most frequently in older adults. These patients are at increased risk of adverse outcomes following major cancer surgery. While prehabilitation has been shown to mitigate this risk, multiple barriers to implementation remain. Our team created a digital tool co-designed with older adults that employs an algorithm to assess patient-specific geriatric vulnerabilities and generate personalised prehabilitation programmes before surgery. METHODS AND ANALYSIS: We have designed a multisite, unblinded randomis

Risk of Venous Thromboembolism After Colorectal Cancer Surgery in Patients With and Without Inflammatory Bowel Disease.

INTRODUCTION: Inflammatory bowel disease (IBD) is associated with elevated postoperative mortality in patients undergoing colorectal cancer (CRC) surgery. Venous thromboembolism (VTE) may partially contribute to this elevated mortality. We investigated VTE risk in patients with and without IBD undergoing their first CRC surgery. METHODS: We conducted a population-based cohort study using Danish health registries (1996-2021), including all patients undergoing first-time CRC surgery (n = 83,950). Patients with a prior IBD diagnosis were defined as exposed. We calculated the 365-day cumulative r

The Impact of Sarcopenia on Postoperative Outcomes in Colorectal Cancer Surgery: An Updated Systematic Review and Meta-Analysis.

Sarcopenia is thought to be a marker for underlying frailty and malnutrition, contributing to poor functional status and suboptimal healing postoperatively. We aimed to complete an updated systematic review and meta-analysis evaluating the impact of sarcopenia on short- and long-term outcomes following colorectal cancer surgery. We searched MEDLINE, Embase, and CENTRAL up to September 2023. Studies that compared sarcopenic and non-sarcopenic patients' short- and long-term outcomes following curative intent elective surgery for colorectal cancer were included. The main outcomes included postope

Practice Parameters for Evaluation and Management of Pruritus Ani on Behalf of the Italian Society of Colorectal Surgery (SICCR).

Pruritus ani (PA) is an unpleasant sensation that leads to scratching and which can seriously compromise patients' quality of life. PA can be secondary to anorectal diseases, systemic disorders, infectious diseases, dermatological conditions, irritative conditions, and psychogenic diseases. However, in many cases, PA appears to be idiopathic. Diagnostic work-up should include medical history collection, psychological evaluation, and clinical examination including anoscopy, laboratory findings, skin swabs, skin scraping, and patch test. Skin biopsy may be useful to clarify doubtful cases or if
