The development of contact hypersensitivity to temporary tattoos has been on an increase all over the world. Skin painting with henna is a traditional practice in the Moslems and Hindu, and has recently been increasingly used in western countries. Black henna is obtained by adding paraphenylenediamine to the natural occurring henna. The risk of sensitization increases with the length of contact and increase of the concentration. A patient was referred for urgent evaluation of cyanosis. Although she had symptoms and signs of an upper respiratory tract infection, she had a normal cardiorespiratory examination and normal transcutaneous oxygen saturation. Further evaluation revealed a benign cause for the cyanosis.

By the maximization test, using 0.5% D&C Yellow no. 11 in pet., 15 of 20 human volunteers became contact sensitized. All reacted to a challenge concentration of 1000 ppm and one down to 1 ppm. The high allergenic potential of this dyestuff was confirmed. There were no cross reactions to purified samples of D&C Yellow no. 10. In contrast, 5% solutions of pure D&C Yellow no. 10 failed to sensitize human subjects. Cross reactions to this substance in persons sensitized to D&C Yellow no. 11 are due to impurities.

We report herein a woman sensitive to nylon stockings of only brown or tan shade. Her situation is interesting for several reasons: she does not react to paraphenylediamine and she reacts to red and yellow dyes, but not to blue or the finish; however, when finish is applied to dyed blanks (red, yellow, or blue) she reacts vigorously to all three. We suspect the finish reacts with the dyes to either expose or produce an antigen previously not available to the skin.
