Evaluation of ketoconazole and itraconazole for treatment of disseminated cryptococcosis in cats.

During the first part of a study, cats were inoculated with Cryptococcus neoformans via the following routes: intradermal, intranasal, IV, and intracisternal. Only use of the IV route of inoculation consistently induced disseminated cryptococcosis. In the second part of the study, disseminated cryptococcosis was experimentally induced in cats via IV inoculation of C neoformans. One month after inoculation, 3 cats were treated with ketoconazole (10 mg/kg of body weight/d) and 3 cats were treated with itraconazole (10 mg/kg/d) for 3 months. One of the ketoconazole-treated and 2 of the itraconazo…

Formation of palisading granulomas in a patient with chronic cutaneous cryptococcosis.

We describe a unique case of a 57-year-old man who developed a chronic cryptococcal infection with cutaneous lesions that showed palisading granulomas. To our knowledge, this is the first report of this entity in the skin. We also discuss the differential diagnosis of palisading granulomas.

Cryptococcosis involving the eye and central nervous system of a dog.

Cryptococcus neoformans meningoencephalitis was diagnosed as the cause of stupor and generalized seizures in a 2-year-old Cocker Spaniel. Unilateral granulomatous chorioretinitis was observed ophthalmoscopically, and isolation of C neoformans from CSF confirmed the antemortem diagnosis. The dog was euthanatized and necropsied. Multifocal lesions were seen throughout the lungs, nasal turbinates, cerebral cortex, and the optic nerve of each eye. Microscopically, the multifocal lesions were granulomas consisting of lymphocytes, macrophages, plasma cells, and cryptococcal organisms. Infection may

Pulmonary cryptococcosis: a report of two cases and review of the literature.

Two cases of primary pulmonary cryptococcosis are described. In both cases diagnosis was established using specimens obtained at bronchoscopy and in one case confirmed by detecting cryptococcal antigen. Knowledge of the natural history of the condition allowed rational decisions to be made regarding treatment. Difficulties surrounding the diagnosis and treatment of this unusual condition are discussed.

Cryptococcal pyelonephritis and disseminated cryptococcosis in a renal transplant recipient.

Symptomatic cryptococcal pyelonephritis, meningitis, and disseminated cryptococcosis are described in a renal cadaver transplant recipient who subsequently died of Klebsiella pneumoniae sepsis. The presence of cryptococcuria and a subsequent positive CSF India ink stain led to the initial diagnosis of disseminated cryptococcosis. Therapy with 0.511 g of amphotericin B and 112.5 g of flucytosine for four weeks did not eradicate Cryptococcus from the kidney and was associated with hepatotoxicity. The importance of urinary examination and culture for C neoformans is emphasized. Cryptococcal pyelo…

Suppression of cryptococcosis and histoplasmosis by ketoconazole in athymic nude mice.

Congenitally athymic nude (nu/nu) mice are more susceptible to disseminated cryptococcosis and histoplasmosis than their heterozygous (nu/ + ) thymus-bearing litter-mates. The therapeutic efficacy of ketoconazole, an orally absorbable antifungal agent, was evaluated in nu/nu and nu/ + mice infected intraperitoneally with Cryptococcus neoformans and Histoplasma capsulatum. Two- to five-week courses of ketoconazole significantly prolonged the survival of nu/nu mice infected with either fungus in dose-dependent fashion, but death eventually followed discontinuance of therapy. More significant pro…

Postpartum pulmonary cryptococcosis combined with pulmonary embolism: a case report.

Pulmonary cryptococcosis is an opportunistic infection of the lungs caused by cryptococcus, usually occurring in immunosuppressed patients. Pulmonary embolism is a type of venous thromboembolism. Pregnancy and postpartum are risk factors for pulmonary embolism. To date, no cases of pulmonary cryptococcosis co-occurring with pulmonary embolism in the postpartum period have been documented. This case report describes a 39-year-old female who was diagnosed with HIV-negative pulmonary cryptococcosis with pulmonary embolism 42 days postpartum and experienced improvement in her condition after activ…

Clinical imaging diagnosis of pulmonary cryptococcosis with different therapeutic responses.

We aimed to differentiate and evaluate the clinical imaging features of pulmonary cryptococcosis with different therapeutic responses. The clinical imaging data of 70 patients with PC (complete response PC 37 cases and incomplete response PC 33 cases) were collected and compared to determine the independent risk factors for different therapeutic responses, and their diagnostic performances were verified by receiver operating characteristic curve analysis. Compared with complete response PC, incomplete response PC was more common with immunosuppression (10.8% vs. 45.5%, P = 0.001), intermediate…

Diagnosis of disseminated cryptococcosis via iliac bone marrow aspirate analysis.

HIV infection frequently affects multiple systems, with hematological manifestations being the most prevalent. In some cases, cryptococcosis serves as the initial manifestation and a cause of infection involving HIV-positive patients. This case report describes a patient with thrombocytopenia who incidentally discovered Cryptococcus infiltrating the bone marrow upon bone marrow smear examination, highlighting that examining bone marrow is essential in diagnosing pancytopenia resulting from opportunistic fungal infections like cryptococcosis, especially in individuals with compromised immune sy…

Bilateral adrenal cryptococcosis causing adrenal insufficiency in an immunocompetent patient.

Cryptococcosis usually occurs in immunocompromised patients and presents as meningitis and lung disease. Adrenal gland involvement may be observed, yet primary adrenal insufficiency by cryptococcal infection is infrequent. We present a case of a middle-aged immunocompetent man with primary adrenal insufficiency and bilateral adrenal lesions, splenomegaly, and miliary mottling in the lungs on imaging. No evidence of meningitis was witnessed. The clinico-radiological findings led toward the differential diagnosis of disseminated tuberculosis or fungal infection. Detection of cryptococcus organis…
