INTRODUCTION: Tuberculosis spondylitis or Pott's disease is the most common destructive form of skeletal tuberculosis. The most commonly affected site is the thoracolumbar vertebra. Once invading the adjacent structures of the vertebrae and intervertebral discs ultimately form an abscess causing spinal cord compression, vertebral collapse, and severe kyphotic deformity.
PRESENTATION OF CASE: We present the first-ever case done in King Fahd Military Medical Complex in Dhahran, Saudi Arabia, of an 18-year-old male diagnosed with tuberculosis spondylitis who was complaining of progressive upper

OBJECTIVE: Microvascular decompression(MVD)surgery has been established as a standard treatment for hemifacial spasm. However, because decompression surgery results in unfavorable outcomes in some cases, a more critical monitoring strategy is required. To improve surgical outcome for hemifacial spasms, abnormal muscle response(AMR)has been proposed as a tool for intraoperative electrophysiological monitoring during MVD surgery. Here, we report a single case of surgical MVD monitoring using artery wall stimulating electromyography(AWS-EMG). AWS-EMG was developed as a new monitoring method in ad

OBJECTIVE: To introduce our experience of preventing further episodes of idiopathic recurrent facial palsy by facial nerve decompression via middle cranial fossa approach.
METHODS: Twelve cases (surgery group) who had idiopathic recurrent facial palsy underwent facial nerve decompression via middle cranial fossa approach, and 6 cases (control group) who declined surgery accepted conservative treatment. Further episodes of facial palsy and final outcomes of facial nerve were recorded. Facial nerve function was assessed by House-Brackmann facial nerve grade system.
RESULTS: Only 8.3% of the pa

An 81-year-old man presented with poor visual acuity of the left eye, swelling of the left eyelid, and elevation of the left intraocular pressure after contusion of the left palpebral portion. CT revealed left ocular proptosis and left intraorbital hematoma. Traumatic optic neuropathy was suspected, and emergent optic nerve decompression was performed through extradural anterior clinoidectomy followed by optic canal release. Postoperatively, his left visual acuity was markedly improved and the elevated intraocular pressure decreased. Postoperative CT demonstrated improvement of the left ocular

BACKGROUND: Isolated deep lateral and combined medial orbital wall decompressions (balanced decompression) are well accepted for treatment of disfiguring proptosis and compressive optic neuropathy in patients with Graves' orbitopathy. However, cerebrospinal fluid leakage and/or optic nerve injury occasionally occur during these operations.
PURPOSE: To describe the anatomy of the deep lateral and medial orbital walls and its surgical implications in orbital decompression.
METHODS: We reviewed literature on the anatomy of the deep lateral and medical orbital walls. In addition, we performed ca

BACKGROUND: Although nerve decompression surgery is an effective treatment for refractory occipital neuralgia, a proportion of patients experience recurrence of pain and undergo reoperation. This study analyzes the incidence, risk factors, and outcomes of reoperation following primary greater occipital nerve (GON) decompression.
METHODS: A total of 215 patients who underwent 399 primary GON decompressions were prospectively enrolled. Data included patient demographics, medical and surgical history, reoperation rates, intraoperative findings, surgical technique, and postoperative outcomes in t

This is a review of the literature on the diagnosis and management of the ossification of the posterior longitudinal ligament (OPLL) of the cervical spine. The pathophysiology and clinical manifestations of cervical OPLL are related to the discogenic dynamic factor of the spine. The signs and symptoms of OPLL include numbness in the hands and/or legs, neck pain, awkward hand movements, and/or gait disturbance, which are not necessarily progressive. Apparent neurological deficit may be an indication of surgery in patients with cervical OPLL because of the low efficacy of conservative treatment

BackgroundInterlaminar and sublaminar decompressions are evolving alternatives to traditional lumbar laminectomy for managing symptomatic lumbar stenosis, but comparative patient-reported outcomes remain underexplored. This study evaluates differences in Patient-Reported Outcomes Measurement Information System (PROMIS) pain interference and physical function (PF) scores between interlaminar and sublaminar lumbar decompression.MethodsWe conducted a retrospective cohort study of 74 patients (18-85 years) who underwent lumbar decompression without fusion at a Level I Trauma Center from January 20

BACKGROUND: A scarcity of data has been reported on tandem thoracic lumbar stenosis, which might be related to either the rarity or underdiagnosis of the condition. We have presented a systematic review of the clinical presentation, diagnosis, and treatment patterns for patients with symptomatic tandem thoracic and lumbar stenosis.
METHODS: A PubMed/MEDLINE search was performed to find reports of patients with symptomatic tandem thoracic and lumbar stenosis.
RESULTS: The review identified 10 studies with a total of 48 patients with tandem thoracic and lumbar stenosis. Most patients (n = 41;

INTRODUCTION: Although many surgical treatment strategies for Chiari malformation type I (CM-I) have been reported, the most appropriate surgical technique remains controversial. It is wholly ascribable to the complicacy of pathological condition in CM-I. Recently, intraoperative neurophysiologic monitoring (INM) is becoming prevalent in spinal surgery. Indeed, motor-evoked potentials (MEPs) monitoring and somatosensory-evoked potentials (SSEPs) monitoring are standard tools to minimize the risk of neurologic injury and postoperative deficits. The most recent study suggested that multimodality

OBJECTIVE: The purpose of this report is to describe the surgical methodology and effectiveness of minimally invasive oblique lumbar interbody fusion (OLIF) assisted by spinal endoscopy, which can treat disk herniation from the central to contralateral foramen. OLIF showed indirect decompression effects on reduction of spondylolisthesis and a foraminal widening effect on disk height restoration.
METHODS: In this study, the indirect decompression effect of OLIF was augmented by direct endoscopic decompression and spinal endoscopy for removal of herniated disk materials.
RESULTS: Twelve patien

Simple decompression of the ulnar nerve at the elbow has not been shown to reduce nerve strain in cadavers. In this study, ulnar nerve strain at the elbow was measured intraoperatively in 11 patients with cubital tunnel syndrome, before and after simple decompression. Statistical analysis was performed using a paired Student's t-test. Mean ulnar nerve strain before and after simple decompression was 30.5% (range 9% to 69%) and 5.5% (range -2% to 11%), respectively; this difference was statistically significant (p < 0.01) with a statistical power of 96%. Simple decompression reduced ulnar nerve

The diagnosis and treatment of neurogenic thoracic outlet syndrome (nTOS) remain challenging. Although consensus-based clinical frameworks exist, they lack robust externa validation. This study conducts a review of nTOS publications in the PubMed, Embase, and Web of Science databases, with a search cutoff date of December 2025. Current evidence suggests that the incidence of nTOS may be underestimated. Diagnosis should be based on a comprehensive evaluation, including medical history, systematic physical examination, ancillary examinations, diagnostic muscle block, and exclusion of other possi

Acute traumatic cardiac tamponade is a time-critical-but potentially reversible-cause of circulatory collapse. The surgical, operative, nursing and anaesthesiology staff most capable and best trained to perform the procedure are often not immediately available, requiring emergency medical staff to perform this life-saving intervention. When tamponade causes refractory shock or peri-arrest, left anterolateral thoracotomy remains the preferred technique for definitive pericardial decompression, with extension to a clamshell thoracotomy when required.

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy and a frequent indication for hand surgery. Symptoms range from intermittent nocturnal paraesthesia to persistent sensory loss and thenar muscle weakness, often with functional impact and socioeconomic burden. Diagnosis is clinical, supported by neurophysiological exams. Mild CTS is usually managed conservatively, while surgical decompression provides the most effective and durable relief in moderate to severe cases. This review finds variation in diagnostic and treatment pathways highlighting the need for standardised care.

BACKGROUND: Cauda equina syndrome (CES) is a rare but life-altering disease resulting from compression of the nerve roots at the spinal cord's terminus. CES typically presents with low back pain, sciatica, sensorimotor deficits, and bowel and bladder dysfunction. Owing to its rarity, the condition is often missed, leading to significant morbidity and potential legal implications for physicians.
METHODS: This review synthesizes the current literature on CES, including its epidemiology, pathophysiology, classifications, and management strategies. Emphasis is placed on the diagnosis and treatmen

Median neuropathy at the wrist, commonly referred to as carpal tunnel syndrome (CTS), is the most common entrapment neuropathy. It is caused by chronic compression of the median nerve at the wrist within the space-limited carpal tunnel. Risk factors that contribute to the etiology of compression include female gender, obesity, work-related factors, and underlying medical conditions, such as hypothyroidism, pregnancy, and amyloidosis. The diagnosis is made on clinical grounds, although these can be confounded by anatomical variations. Electrodiagnostic studies, which are specific and sensitive
