Development of chronic actinic dermatitis in a patient with atopic dermatitis: a case report and a review of reported cases.

A 30-year-old Japanese woman who had suffered from atopic dermatitis (AD) since childhood was referred for the evaluation of itchy erythema in sun-exposed regions, which persisted for 3 years. Phototesting revealed that the minimal erythema dose (MED) to ultraviolet (UV)B radiation was 20 mJ cm-2 and MED to UVA radiation was 10 J cm-2. Phototesting with visible light revealed no positive reaction. The skin lesion was diagnosed as chronic actinic dermatitis (CAD). She was asked to strictly avoid exposure to sunlight; thereafter, her skin symptoms disappeared. There have been 18 reported cases o

Patch Testing in Toilet Seat Dermatitis: A Case Series of Six Patients with Paediatric Atopic Dermatitis.

BACKGROUND: Toilet seat dermatitis (TSD) is a localized form of dermatitis that commonly occurs in the buttocks and upper posterior aspect of the thigh in school-going children. Previously, it was mostly seen due to allergic contact dermatitis to wood, but recently, many other allergens in toilet seats along with the possibility of irritant contact dermatitis (ICD) to detergents were reported. MATERIALS AND METHODS: Six cases diagnosed clinically as atopic dermatitis per Hanifin and Rajka criteria presented with posterior thigh dermatitis. Patch testing using the Indian Standard Series, toile

Variable impact of dupilumab on patch testing results and allergic contact dermatitis in adults with atopic dermatitis.

BACKGROUND: Previous case reports and series suggested that dupilumab may be an effective treatment for allergic contact dermatitis (ACD). Little is known about the impact of dupilumab on patch test results and comorbid ACD in patients with atopic dermatitis (AD). OBJECTIVE: Determine the impact of dupilumab on patch testing results and improvement of ACD in patients with AD. METHODS: A retrospective study of patients with AD treated with dupilumab who underwent patch testing (n = 7) or had concomitant ACD (n = 6). RESULTS: In all, 7 patients with AD were patch tested while taking dupilumab

Consensus-based European guidelines for treatment of atopic eczema (atopic dermatitis) in adults and children: part II.

This guideline was developed as a joint interdisciplinary European project, including physicians from all relevant disciplines as well as patients. It is a consensus-based guideline, taking available evidence from other guidelines, systematic reviews and published studies into account. This second part of the guideline covers antimicrobial therapy, systemic treatment, allergen-specific immunotherapy, complementary medicine, psychosomatic counselling and educational interventions, whereas the first part covers methods, patient perspective, general measures and avoidance strategies, basic emolli

Association of Mitochondrial DNA Copy Number With Atopic Dermatitis in Children.

BACKGROUND: Mitochondrial DNA copy number (mtDNA-CN), a marker associated with mitochondrial function and cellular health, has been implicated in several chronic inflammatory diseases. Atopic dermatitis is a common chronic inflammatory skin disorder, yet the relationship between mtDNA-CN and childhood atopic dermatitis remains unclear. This study aimed to investigate the association between mtDNA-CN and atopic dermatitis in a population-based cohort of school-aged children. METHODS: We included 1329 singleton children (56.4% boys; mean age ± standard deviation: 6.5 ± 0.5 years) from the Longi

Successful treatment of eosinophilic esophagitis with upadacitinib prescribed for atopic dermatitis.

We describe a pediatric patient treated with upadacitinib for atopic dermatitis (AD) who subsequently achieved sustained clinical and histologic remission of eosinophilic esophagitis (EoE). Upadacitinib is an oral small molecule selective Janus kinase 1 inhibitor that inhibits janus kinase-signal transduction and activation of transcription (JAK-STAT) pathway with wide inhibitory effects on the immune system, including Th2 signaling pathways. Therefore, it leads to broad suppression of inflammatory cytokines, including interleukin-4 (IL-4), interleukin-13 (IL-13), and thymic stromal lymphopoie

Topical Therapy for Atopic Dermatitis: What is New and the New Paradigm.

Atopic dermatitis (AD) is a chronic inflammatory skin disorder that requires a complex management strategy, which often involves multiple and diverse topicals and systemic treatment regimens. While topical steroids and more recently calcineurin inhibitors have been the mainstay therapy for mild-to-moderate disease, recent advances in the understanding of AD pathogenesis have led to the development of different new targets, rapidly widening our therapeutic armamentarium. This review summarizes their efficacy and safety data. We also review topical optimization strategies, including the recently

Psoriasis and Atopic Dermatitis Overlap: Pathogenesis and Therapeutic Considerations for the Clinician.

Psoriasis (PsO) and atopic dermatitis (AD) are chronic, inflammatory skin diseases with distinct, yet occasionally overlapping, immunological profiles. PsO is characterized by T helper (Th) 1/Th17 immune pathways, while AD predominantly involves Th2 responses. However, a subset of patients exhibit features of both conditions, presenting a diagnostic and therapeutic challenge. This review aims to comprehensively assess the pathogenesis, clinical presentation, and therapeutic considerations of the PsO-AD overlap phenotype. This unique clinical entity is characterized by mixed Th17/Th2 immune res

The role and management of dietary and environmental factors in pediatric atopic dermatitis.

BACKGROUND: The pathogenesis of pediatric atopic dermatitis (AD) is complex, and the role of dietary and environmental factors in its onset and progression warrants systematic investigation. OBJECTIVE: To explore the associations between dietary allergen sensitization, environmental factors, and the development of pediatric AD through a retrospective case-control analysis, and to evaluate the clinical efficacy of a multidimensional intervention strategy. METHODS: This study employed a two-stage design. The first stage was a retrospective case-control study, enrolling 535 pediatric AD patient

Gut microbiota contributes to the therapeutic effect of acupuncture in atopic dermatitis.

UNLABELLED: The gut microbiome is increasingly recognized as a central regulator of immune homeostasis, metabolic balance, and therapeutic outcomes. Atopic dermatitis (AD), a chronic inflammatory skin disease, is closely linked to gut microbial dysbiosis. Traditionally regarded as a neurostimulatory therapy, acupuncture (Acu) has demonstrated increasing efficacy in alleviating AD symptoms and improving gastrointestinal function. These observations suggest that the therapeutic effects of Acu in AD may be mediated, in part, by modulation of the gut microbiome. In this study, AD patients were str

Atopic Dermatitis and Markers of Early Cardiovascular Risk in Children and Adolescents.

IMPORTANCE: Early identification of individuals at elevated risk of cardiovascular disease and preventive treatment during childhood may reduce cardiovascular disease in adults with chronic inflammatory diseases. Children with atopic dermatitis (AD) may have elevated cardiovascular risk, but studies to date have not accounted for heterogeneity in disease activity and severity across childhood. OBJECTIVES: To evaluate whether active and more severe AD across childhood and adolescence are associated with cardiovascular risk. DESIGN, SETTING, AND PARTICIPANTS: This longitudinal cohort study use

Tofacitinib-Associated Acne in a Young Patient With Severe Atopic Dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory skin disease that may require systemic therapy in patients with moderate-to-severe or refractory disease. Tofacitinib, a Janus kinase (JAK) inhibitor, has demonstrated efficacy in the treatment of severe inflammatory dermatoses, including refractory AD. However, its use may be associated with various adverse events, and acneiform eruptions have increasingly been reported with JAK inhibitor therapy. We report the case of an 18-year-old female patient with severe AD who was treated with tofacitinib, resulting in marked improvement of her eczematou

Skin Barrier Dysfunction in Contact and Atopic Dermatitis: A Comparative Review.

The epidermis, the outermost layer of the skin, consists of 5 distinct layers, with the stratum corneum (SC) serving as the primary barrier. Disruption of skin barrier leads to inflammatory skin conditions, including irritant contact dermatitis (ICD), allergic contact dermatitis (ACD), and atopic dermatitis (AD). ICD arises from direct damage to the SC by irritants, while ACD is mediated by a type IV hypersensitivity. AD is characterized by chronic inflammation and genetic defects that compromise barrier integrity. Accordingly, strategies aimed at barrier restoration, such as the use of cerami

Beyond the dichotomy: understanding the overlap between atopic dermatitis and psoriasis.

Atopic dermatitis and psoriasis have traditionally been considered distinct inflammatory skin diseases with unique pathogenic mechanisms. However, accumulating evidence suggests significant overlap in their immunological pathways, metabolic features, and microbiome characteristics, challenging this conventional dichotomy. This review comprehensively examines the complex relationship between psoriasis and atopic dermatitis, with particular emphasis on their shared and distinct pathogenic mechanisms. We analyze the immunological networks, metabolic pathways, and microbial factors contributing to
