The adverse effects of a variety of drugs on psoriasis are well documented, and they generally result in aggravation of the disease. We report the case of a 75-year old man who developed a psoriatic erythroderma and a generalized pustular psoriasis two weeks after the beginning of an antimalarial treatment. The psoriasis responded poorly to etretinate alone, and an etretinate-methotrexate combination was needed to cure the patient. Some psoriasis vulgaris plaques persisted for six months after the acute complication. Thus, chloroquine should be entered in the list of drugs which may induce pso

It has been proposed that toxins and other bacterial protein products of Staphylococcus aureus can act as triggers or persistence factors in several inflammatory skin diseases. In this study, we examined the S. aureus isolates from the skin of patients with atopic dermatitis and psoriasis. We found that the bacterial isolates from these patients exhibited either characteristic superantigenic toxins or thermolabile toxins believed to be staphylococcal alpha-toxin. All of these staphylococcal strains also secreted extracellular staphylococcal protein A. We found significant differences in the ac

The clinical diagnosis of psoriasis is relatively easy, especially when the lesions consist of erythematous, silvery white scaly, sharply demarcated, indurated plaques, distributed symmetrically on the extensor surfaces of limbs, the lower back and the scalp. These clinical features reflect the histopathological findings observed in active lesions, characterized by parakeratosis, acanthosis of the epidermal ridges, tortuous and dilated blood vessels, and perivascular leukocytic infiltrate; the Munro microabscess and the spongiform pustule of Kogoj are diagnostic. Diagnostic doubts, however, ma

BACKGROUND: Drug reaction with eosinophilia and systemic symptoms (DRESS) describes a heterogeneous group of severe adverse reactions to medications. The cutaneous phenotype has a number of guises, accompanied by a variety of systemic features including fever, haematological abnormalities and visceral involvement, most commonly the liver. Clinical markers of prognosis have not been identified.
OBJECTIVES: To assess the cutaneous signs and dermatopathological features of DRESS in order to identify potential prognostic markers.
METHODS: We reviewed the clinical features, dermatopathology and o

BACKGROUND: Erythrodermic pemphigus foliaceus (EPF) is a severe, often refractory autoimmune disease.
METHOD: We report successful treatment of refractory EPF in a 59-year-old male using low-dose rituximab (RTX) and intravenous immunoglobulin (IVIG).
RESULTS: Initial high-dose corticosteroids and IVIG failed, complicated by infections. A modified regimen of RTX (0.5 g) and IVIG (0.4 g/kg/day for three days) every two weeks for two cycles induced remission, with a mild relapse at six months successfully retreated. No infections occurred during 15-month follow-up.
CONCLUSION: This combined th
