Nitazoxanide alleviates collagen-induced arthritis by inhibiting STAT3-dependent Th17 cell differentiation and inflammatory cytokines production.

Nitazoxanide (NTZ), an FDA-approved anti-parasitic drug, has shown anti-inflammatory effects in rheumatoid arthritis (RA) fibroblast-like synoviocytes, yet its impact on immune cell function and its potential synergy with methotrexate (MTX) remains unclear. In this study, using a murine collagen-induced arthritis (CIA) model, we demonstrated that NTZ synergizes with MTX to delay arthritis onset and ameliorate joint pathology, accompanied by reduced inflammatory cell infiltration, synovial pannus formation, and bone destruction. Moreover, NTZ significantly suppressed inflammatory cytokine produ

Engineered Nanofiber-Based Nerve Guidance Conduit Facilitates the Restoration of Peripheral Nerve Injury through Enhanced Vascularization.

Peripheral nerve injuries (PNI) can disrupt the innervation of target organs, leading to diverse dysfunctions. While autologous nerve grafts are often regarded as the gold standard for treatment, their use is limited by donor nerve availability and potential complications at the injury site. Therefore, nerve guidance conduits (NGCs) constructed from biomaterials are favoured as an alternative for nerve repair. Angiogenesis provides the necessary prerequisites and microenvironmental support for neural regeneration. In this study, the extracted endothelial cell-derived matrix (ECd) was modified

Development and early feasibility testing of machine-learning algorithms to non-invasively assess hemoglobin levels.

The HeMonitor study evaluated the feasibility and accuracy of non-invasive hemoglobin (Hb) assessment using image-based techniques and machine learning in patients with hematologic malignancies. A total of 367 patients with hematologic malignancies and 184 healśśthy donors were enrolled, with fingernail and eyelid photographs collected and analyzed using Light Gradient-Boosting Machine (LightGBM) regression models. The best-performing model achieved a residual standard deviation of ±1.02 mmol/L for Hb prediction. Our framework further explored a two-stage concept combining (i) a non-invasive i

Revisiting optimal static lung preservation techniques in an era when machine perfusion is transforming organ recovery: thermoregulation is still an important concept.

Recent initiatives have led to outstanding increases in lung procurement employing donation after circulatory death (DCD) and transplantation of DCD lungs. Although this shift to expand donor resources is encouraging, outcomes after lung transplantation, particularly after DCD lung transplantation, have lagged behind the notable success seen for transplantation of abdominal organs such as kidneys and livers. Additionally, improvements through the use of machine perfusion (MP) in lung transplantation protocols have not been as clearly appreciated or consistently demonstrated as in abdominal org

An in silico mechanoregulatory model of depth-dependent adaptations to mechanical loading in intact and damaged cartilage: a proof of concept study.

Osteoarthritis induces profound structural degeneration of articular cartilage, with existing treatments remaining largely ineffective. This study pioneers a mechanoregulatory model utilizing histology-based finite element analysis to predict depth-dependent glycosaminoglycan (GAG) adaptations in both intact and damaged human cartilage under mechanical loading. Uniquely calibrated through rigorous one-week longitudinal in vitro experiments in intact cartilage, our model correctly predicts depth-dependent GAG content adaptation, also in damaged cartilage. Notably, the model reveals potential ef

CRISPR-Cas9 homology-independent targeted integration of exons 1-19 restores full-length dystrophin in mice.

Duchenne muscular dystrophy is an X-linked disorder typically caused by out-of-frame mutations in the DMD gene. Most of these are deletions of one or more exons, which can theoretically be corrected through CRISPR-Cas9-mediated knockin. Homology-independent targeted integration is a mechanism for achieving such a knockin without reliance on homology-directed repair pathways, which are inactive in muscle. We designed a system based on insertion into intron 19 of a DNA fragment containing a pre-spliced mega-exon encoding DMD exons 1-19, along with the MHCK7 promoter, and delivered it via a pair

Human Leukocyte Antigen Genotyping of Idiopathic Subglottic Stenosis.

OBJECTIVE: Despite recent scientific inquiry, idiopathic subglottic stenosis (iSGS) remains an enigmatic disease. The consistent demographics of the affected population suggest genetic factors may contribute to disease susceptibility. Given the inflammation observed in the affected proximal airway mucosa, we interrogated disease association with human leukocyte antigen (HLA) polymorphisms. Polymorphisms in the HLA locus have previously been shown to influence individuals' susceptibility to distinct inflammatory diseases. METHODS: High-resolution HLA typing of 37 iSGS patients was compared wit

Recent advances in porcine cardiac xenotransplantation: from aortic valve replacement to heart transplantation.

INTRODUCTION: Cardiac xenotransplantation presents significant potential to the field of heart failure by addressing the high demand for donor organs. The availability of xenograft hearts would substantially augment the number of life-saving organs available to patients and may ultimately liberalize eligibility criteria for transplantation. AREAS COVERED: In this review, we will discuss the need for cardiac xenotransplantation and the history of research and clinical practice in this field. Specifically, we address immunologic concepts and clinical lessons learned from heart valve replacement

Maternal Low Volume Circulation Relates to Normotensive and Preeclamptic Fetal Growth Restriction.

This narrative review summarizes current evidence on the association between maternal low volume circulation and poor fetal growth. Though much work has been devoted to the study of cardiac output and peripheral vascular resistance, a low intravascular volume may explain why high vascular resistance causes hypertension in women with preeclampsia (PE) that is associated with fetal growth restriction (FGR) and, at the same time, presents with normotension in FGR itself. Normotensive women with small for gestational age babies show normal gestational blood volume expansion superimposed upon a con

Efficient Method for Generating Point Mutations in the Vaccinia Virus Genome Using CRISPR/Cas9.

The vaccinia virus (VACV) was previously used as a vaccine for smallpox eradication. Nowadays, recombinant VACVs are developed as vaccine platforms for infectious disease prevention and cancer treatment. The conventional method for genome editing of the VACV is based on homologous recombination, which is poorly efficient. Recently, the use of CRISPR/Cas9 technology was shown to greatly improve the speed and efficiency of the production of recombinant VACV expressing a heterologous gene. However, the ability to rapidly recover viruses bearing single nucleotide substitutions is still challenging

Recipient liver splitting to facilitate piggyback hepatectomy in adult living donor liver transplantation.

Recipient hepatectomy for an enlarged stony-hard liver is a demanding procedure, thus it is often accompanied by massive blood loss. Recipient liver splitting under prolonged hepatic inflow occlusion would facilitate the piggyback recipient hepatectomy. We herein present a case of recipient liver splitting, which was used for living donor liver transplantation (LDLT). A 48-year-old male patient diagnosed with acute-on-chronic liver failure underwent LDLT. During the recipient operation, the native liver was stony-hard and heavily adherent to the retrohepatic inferior vena cava (IVC). During li

A novel protein biochip screening serum anti-sperm antibody expression and natural pregnancy rate in a follow-up study in Chinese infertility.

Production of anti-sperm antibody (ASA) often suffers from autoimmune reaction against sperms in human infertility. The antibodies are measured in both blood and seminal plasma of males. Here, we reported a simple protein biochip methodology that takes advantage of a functionalized self-assembled monolayer modified by N-hydroxysuccinimide (NHS) and enables identification of anti-sperm antibody in Chinese male infertility. To validate this biochip platform, we immobilized purified sperm protein on the biochip surface and tested a variety of parameters in quality controls for the protein assay,

The quest to decipher HLA immunogenicity: Telling friend from foe.

Molecular mismatch load analysis was recently introduced as a means for performing risk stratification following organ transplantation. However, although good correlation was demonstrated between molecular mismatch load and generation of de novo donor-specific HLA antibody (DSA), quite a few exceptions exist, and the underlying factors that define HLA immunogenicity remain unclear. Herein, we present a new paradigm to interrogate differences between molecular mismatches that lead to the generation of de novo DSA and those that do not (the 2MM1DSA cohort). Specifically, patients transplanted ac

The Footprints of Poly-Autoimmunity: Evidence for Common Biological Factors Involved in Multiple Sclerosis and Hashimoto's Thyroiditis.

Autoimmune diseases are a diverse group of chronic disorders and affect a multitude of organs and systems. However, the existence of common pathophysiological mechanisms is hypothesized and reports of shared risk are emerging as well. In this regard, patients with multiple sclerosis (MS) have been shown to have an increased susceptibility to develop chronic autoimmune thyroid diseases, in particular Hashimoto's thyroiditis (HT), suggesting an autoimmune predisposition. However, studies comparing such different pathologies of autoimmune origin are still missing till date. In the present study,

Natural Killer Cells: Angels and Devils for Immunotherapy.

In recent years, the relevance of the immune system to fight cancer has led to the development of immunotherapy, including the adoptive cell transfer of immune cells, such as natural killer (NK) cells and chimeric antigen receptors (CAR)-modified T cells. The discovery of donor NK cells' anti-tumor activity in acute myeloid leukemia patients receiving allogeneic stem cell transplantation (allo-SCT) was the trigger to conduct many clinical trials infusing NK cells. Surprisingly, many of these studies did not obtain optimal results, suggesting that many different NK cell parameters combined with

Current status of hepatocyte xenotransplantation.

The treatment of acute liver failure, a condition with high mortality, comprises optimal clinical care, and in severe cases liver transplantation. However, there are limitations in availability of organ donors. Hepatocyte transplantation is a promising alternative that could fill the medical need, in particular as the bridge to liver transplantation. Encapsulated porcine hepatocytes represent an unlimited source that could function as a bioreactor requiring minimal immunosuppression. Besides patients with acute liver failure, patients with alcoholic hepatitis who are unresponsive to a short co

Darurah (Necessity) and Its Application in Islamic Ethical Assessment of Medical Applications: A Review on Malaysian Fatwa.

The discovery and invention of new medical applications may be considered blessings to humankind. However, some applications which might be the only remedy for certain diseases may contain ingredients or involve methods that are not in harmony with certain cultural and religious perspectives. These situations have raised important questions in medical ethics; are these applications completely prohibited according to these perspectives, and is there any room for mitigation? This paper explores the concept of darurah (necessity) and its deliberation in the formulation of fatwas on medicine issue

Blockade of fatty acid synthase triggers significant apoptosis in mantle cell lymphoma.

Fatty acid synthase (FASN), a key player in the de novo synthetic pathway of long-chain fatty acids, has been shown to contribute to the tumorigenesis in various types of solid tumors. We here report that FASN is highly and consistently expressed in mantle cell lymphoma (MCL), an aggressive form of B-cell lymphoid malignancy. Specifically, the expression of FASN was detectable in all four MCL cell lines and 15 tumors examined. In contrast, benign lymphoid tissues and peripheral blood mononuclear cells from normal donors were negative. Treatment of MCL cell lines with orlistat, a FASN inhibitor

Cell-based therapies for metabolic liver disease.

Liver transplantation is an important therapeutic option for many individuals with metabolic liver disease. Nevertheless, the invasive nature of surgery and limitations of donor organ availability have led to the search for alternatives to whole-organ transplantation. Cell-based therapies have been a particularly active area of investigation in recent years. Hepatocyte transplantations have been performed for a variety of indications, including acute liver failure, end-stage liver disease, and inborn errors of metabolism. Individuals with inborn errors of metabolism who have undergone hepatocy

Low nitric oxide: a key factor underlying copper-deficiency teratogenicity.

Copper (Cu)-deficiency-induced teratogenicity is characterized by major cardiac, brain, and vascular anomalies; however, the underlying mechanisms are poorly understood. Cu deficiency decreases superoxide dismutase activity and increases superoxide anions, which can interact with nitric oxide (NO), reducing the NO pool size. Given the role of NO as a developmental signaling molecule, we tested the hypothesis that low NO levels, secondary to Cu deficiency, represent a developmental challenge. Gestation day 8.5 embryos from Cu-adequate (Cu+) or Cu-deficient (Cu-) dams were cultured for 48 h in C
