Bladder necrosis presenting with hematuria in a patient with sickle-cell disease.

We present an interesting case of bladder necrosis in an 11-year-old boy with sickle-cell disease. The patient initially presented with sudden onset of abdominal pain and went on to have gross hematuria with clots and severe dysuria. Cysto-urethroscopy revealed global hemorrhagic cystitis and a suprapubic catheter was inserted percutaneously during cystoscopy. The symptoms spontaneously resolved over several weeks and the child voids normally at last follow-up. Multiple bladder biopsies were taken and all were completely necrotic.

Clinical management of urinary tract infection in women: a prospective cohort study.

BACKGROUND: It is unclear how symptoms of urinary tract infection (UTI) influence clinical management in terms of diagnostic testing and tre … f UTI determined. RESULTS: GPs were far more likely to treat empirically patients with symptoms of dysuria and frequency [odds ratio (OR) 6.50, 95% confidence interval (CI) 2.02-20.89] or dysuria alone (OR 5.24, 95% CI 1.62-16.95). They were far less likely to perform diagnostic tests in patients with dysuria and frequency (OR for near patient testing 0. …

Gastrocystoplasty: long-term followup.

PURPOSE: Gastrocystoplasty is no longer favored at many institutions due to complications, including the hematuria-dysuria syndrome and metabolic alkalosis. We reviewed our experience to determine the advantages and disadvantages of bladder augmentation using stomach body wall. MATERIALS AND METHODS: We retrospectively reviewed the medical records, urodynamic studies, x-rays and laboratory evaluations of 47 children who underwent gastrocystoplasty at our institution between 1986 and June 1997. Parents and patients were contacted by telephone for detailed interviews to validate the medical rec…

[Endometriose i klinisk praksis].

Endometriosis is a chronic, estrogen-dependent inflammatory disorder affecting ~10% of reproductive-age women. It is underdiagnosed, with delays of 5-12 years. Symptoms include dysmenorrhea, chronic pelvic pain, dyspareunia, dyschezia, dysuria, fatigue, and infertility with endometriosis present in up to 50% of infertile patients. Diagnosis relies on clinical assessment, imaging (TVUS/MRI), and occasionally laparoscopy. Pathogenesis involves retrograde menstruation, genetic and immunologic factors. Management includes analgesics, hormonal therapy, surgery, physiotherapy, and psychosocial suppo…

From contraception to calculus: copper-T migration resulting in vesical calculus formation.

A woman in her 40s presented with persistent lower urinary tract symptoms (LUTS) over 6 months, including suprapubic pain and dysuria. Initial investigations were inconclusive until non-contrast CT revealed a large bladder stone encapsulating an intrauterine contraceptive device (Copper-T). She underwent successful perurethral cystolithotripsy using Holmium:YAG laser, resulting in complete stone and device removal. This case underlines the importance of suspecting migrated intrauterine devices in unexplained LUTS and highlights minimally invasive endourological techniques as effective manageme…

Intermittent urethral infusion of dimethylsulfoxide for urethral amyloidosis: a case report and literature review.

Urethral amyloidosis (UA) is a very rare condition. We here report the case of a 63-year-old male patient who was admitted to our outpatient department due to aggravating dysuria, frequent urination, pain during intercourse, and a gradually enlarging mass at the ostium of his urethra, which he first notice one year earlier. Pathological tissue biopsy of urethral ostium mass confirmed UA. Intermittent urethra infusion of dimethyl sulfoxide was performed and the treatment effect is satisfactory.

MRI and FDG PET/CT findings of malignant fibrous histiocytoma of the prostate.

Malignant fibrous histiocytoma primarily arising from the prostate is extremely rare. A 43-year-old man presented with dysuria for 12 months. Serum prostate-specific antigen level was normal. T2-weighted MR images showed a well-circumscribed tumor with inhomogeneous intensity in the prostate. The patient underwent transurethral resection of the prostate. Prostate malignant fibrous histiocytoma was histopathologically confirmed. FDG PET/CT was performed showing extremely high FDG uptake of the tumor with SUVmax of 46.8.

[Ketamine-induced vesicopathy].

We report about a 25-year-old patient with transnasal ketamine abuse over years presenting with severe irritative urinary dysfunction (imperative urinary urgency, pollakisuria, dysuria) and severe alguria. Cystoscopia showed ketamine-induced vesicopathy with errosive cystitis; other etiologies could be excluded. Despite serious effort the patient was not motivated for abstinence from ketamine. After two ineffecient therapies with botulinum toxin A (200 and 400 I. E.) injected into the bladder, a prostate preserving cystectomia and ileum neobladder were mandatory.

Synthetic pubovaginal sling (TVT): failure in conservative treatment following vaginal exteriorization.

Female, 57 year-old patient, reported having undergone surgery for correction of urinary incontinence due to sphincteric insufficiency with the implantation of a synthetic pubovaginal sling 14 months earlier. Though she did not present urine loss any longer, approximately 60 days following the surgical procedure she started to report dysuria, pollakiuria and dyspareunia. Attempts of a conservative solution were ineffective. The appearance of a vaginal infra-urethral granuloma and the exteriorization of the synthetic material led to its removal.

Intraosseous epidermal cyst of the sacrum. A case report.

An unusual case of intraosseous epidermal cyst is reported. The patient, a 45-year-old Japanese female, had suffered from lumbago and dysuria for about 15 years. X-ray examinations and CT scan revealed an expanded osteolytic tumor without marginal sclerotic change within the sacrum, which anteriorly invaded the surrounding soft tissues at the S2/3 level. At this time, chordoma was suspected, but epidermal cyst with foreign body granuloma was finally diagnosed from biopsy and surgical specimens.

Presacral Keratinous and Dermoid Cyst Masquerading as Meningocele - A Rare Case Report.

Presacral tumours are rare developmental tumours associated with midline closure defects. A fifteen-month-old girl presented with pain in the lower abdomen and dysuria. After examination, the child underwent investigations and was diagnosed as a presacral mass with intergluteal extension and caudal mass in the subcutaneous tissue of the left gluteal region. Complete excision of mass along with coccygectomy was done. Histopathology showed presacral as a keratinous cyst and gluteal as a dermoid cyst. Later on, the child also developed a left facial dermoid cyst which was also excised.

A Sinister Gut Feeling.

Women are generally spared from abdominal aortic aneurysm (AAA) formation by the immunomodulating effects of estrogen. However, once they develop it, especially in the postmenopausal group, its behavior is more sinister with rapid expansion, a higher tendency to rupture, and higher mortality as compared to the male counterparts. Reported here is a case of AAA in a postmenopausal woman who came to the outpatient department with low backache, vague abdominal pain, and dysuria which initially gave a picture of pelvic inflammation with urinary tract infection but was later found out to be aortic a…

Urinary Tract Infection Antibiotic Resistance in the United States.

Urinary tract infection (UTI) is one of the most common entities in medicine and affected patients present daily in a typical family medicine practice. The patients often present with the "classic symptoms" of dysuria and increased frequency, but sometimes they are asymptomatic or have a mixed picture. In most cases, an antibiotic is prescribed, and this practice is likely contributing to increasing worldwide antibiotic resistance. To help combat this problem, it is important that clinicians seek out their local bacterial resistance patterns and antibiograms, properly diagnose and treat UTI if…

Carcinosarcoma of the Female Urethra.

Carcinosarcoma is a rare malignant tumor with a biphasic morphology characterized by the presence of a malignant epithelial and mesenchymal component. It has been reported in many organs, including the genitourinary tract. We describe a case of a 47-year-old woman admitted to our hospital for history of recurrent urinary tract infection, dysuria and discharge of bloody fluid from the urethra at the end of urination. A tender palpable mass under the anterior vaginal wall was found and pathological examination showed a urethral carcinosarcoma. The histopathogenetic hypothesis and clinical manage…

Retrocaval ureter: an innocent bystander.

Retrocaval ureter is a rare congenital anomaly, with the ureter passing posterior to the inferior vena cava. Though it is a congenital anomaly, patients do not normally present with symptoms until the 3(rd) and 4(th) decades of life, which result from a hydronephrosis. We are presenting a case of 45-year-old lady who had right flank pain and dysuria, which were associated with right hydroureteronephrosis. Diagnosis was confirmed by doing Intravenous Pyelogram (IVP) and patient underwent an open surgical repair of the anomaly.

Acute diarrhea and metabolic acidosis caused by tuberculous vesico-rectal fistula.

Acquired vesico-rectal fistula is an uncommon complication of pelvic malignant tumors, surgical injury, inflammatory disorders such as tuberculosis infection, radiotherapy and less commonly diverticulum of the urinary tract. The fistula is often identified by urinary tract abnormalities such as dysuria, recurrent urinary tract infection, pneumaturia, and fecaluria. Here, we report an unusual case of a patient with a vesico-rectal fistula of tuberculous origin, presenting with severe acute diarrhea, metabolic acidosis, hyperchloremia and hypokalemia while with only mild urinary tract symptoms. …

A case of emphysematous cystitis diagnosed by exploratory laparotomy.

Emphysematous cystitis is a rare form of infection. Well known symptoms are: dysuria, urinary frequency and lower abdominal pain. We experienced a case of emphysematous cystitis presented with atypical peritoneal sign and computed tomography findings of massive intra-peritoneal fluid collection and abnormal gas appearance in pelvic space. Due to its presentation as acute abdomen, patient underwent exploratory laparotomy and the diagnosis of emphysematous cystitis was established. As far as we know, our case is the first report of emphysematous cystitis with intra-peritoneal fluid collection.

[The frequencies of both urinary tract infection and asymptomatic bacteriuria increase with age].

Urinary tract infection is common in the elderly, and the prevalence increases with age due to pathological changes in the urinary tract and common use of bladder catheter. The increasing incidence of asymptomatic bacteriuria, together with fewer classical dysuria symptoms complicates the diagnosis in the elderly. There is also an increasing prevalence of complicated urinary tract infection including septicaemia. Due to an increase in antibiotic resistance urine culture should precede the choice of antibiotic treatment. Treatment duration is seven days; 14 days in patients with pyelonephritis.…
