Reconstruction of a large defect of the ear using a composite graft following a human bite injury.

Human bites to the ear are not an uncommon injury as a consequence of brawls or even contact sports like rugby. Untreated, the cosmetic outcome of such injury is unsightly. This case history documents how a patient, who had two-thirds of his external ear bitten off, was treated successfully by replacing the part as a composite graft. The segment replanted was minimally trimmed but inset to allow maximal soft tissue apposition at the skin edges. An episode of postoperative venous congestion was treated with leeches. Patience was exercised rather than early debridement of any struggling tissue.

[Effects of helium-neon laser energy on the tissues of the middle ear in the presence of biological fluids and drug solutions].

By means of biophotometry the coefficients of absorption and penetration depth of helium-neon laser radiation with a wavelength of 632.8 nm were measured. Measurements were made in the inflammatory mucous membrane of the tympanic cavity, mucous membrane of the tympanic-cavity infiltrated with the saline solution, cortical bone of the mastoid, cartilage of the auricle, blood, serum, exudate, 0.02% furacillin, 3% boric acid, 0.9% and 10% NaCl. These observations were used to develop an efficient method of laser therapy of the inflammatory and wound surface of the middle ear biological tissue, wh

Advances in the Development of Auricular Cartilage Bioimplants.

Conditions such as congenital abnormalities, cancer, infections, and trauma can severely impact the integrity of the auricular cartilage, resulting in the need for a replacement structure. Current implants, carved from the patient's rib, involve multiple surgeries and carry risks of adverse events such as contamination, rejection, and reabsorption. Tissue engineering aims to develop lifelong auricular bioimplants using different methods, different cell types, growth factors and maintenance media formulations, and scaffolding materials compatible with the host. This review aims to examine the p

Skin-derived epithelial lining facilitates orthotopic tracheal transplantation by protecting the tracheal cartilage and inhibiting granulation hyperplasia.

Long-segment tracheal defects caused by tumours, inflammation or trauma can cause serious damage to the quality of life of patients. Although many novel neotracheas have been constructed, the therapeutic effect of orthotopic transplantation was compromised mainly because of the lack of an epithelial lining in those neotracheas. In this study, we aimed to investigate the therapeutic function of skin-derived epithelial lining for orthotopic tracheal transplantation. Strips of auricular cartilage with fixed interval were interrupted sutured on a silicone tube to mimic the cartilage rings of the n

Hyaluronate production by the inner ear during otic capsule and perilymphatic space formation.

Otosclerosis has been hypothesized to result from a disorder of the extracellular matrix of the cartilaginous rests present in the adult temporal bone. Matrix relationship to bone formation and remodeling, as well as the fact that the pathogenesis of otosclerosis is expressed by the action of both of these processes, strongly suggests that more knowledge is needed about the process of otic capsule development. In pursuit of this goal, otic complexes were explanted from mouse embryos that ranged in age from 10.5 to 16 days old and were then exposed to 3H-glucosamine (50 microCi/ml) for 6 hours

Macrotia.

Techniques for treatment of severe and moderate macrotia and for hypertrophy of the earlobe are described. For macrotia, the excision of cartilage at the scapha, helical arch, and skin are performed at different levels to prevent a notching, mainly at the helix. For hypertrophies of the earlobe, through-and-through excisions of two triangular pieces are used with reinsertion of the remaining earlobe at the base.

[Auricular reconstruction for severe microtia. Schedule of treatment, operative strategy, and modifications].

The incidence of microtia in Germany is 100-150 per year. These cases require a specific and challenging therapy. All patients need audiologic consultation. If desired plastic reconstruction is performed, which is aiming at achieving a lifelike as possible appearance corresponding to the shape of the opposite ear including an excellent skin color. The present paper describes background information, the interdisciplinary schedule of treatment, and the results of our operative strategy in two to three steps using autologous rib cartilage. Furthermore we expand on anomalous cases of microtia whic

Chondrodermatitis nodularis nasi.

Chondrodermatitis nodularis helicis is a relatively common ulcerative, inflammatory condition affecting the skin, and cartilage of the ear. We present a case of chondrodermatitis affecting the skin and cartilage of the nose, designated chondrodermatitis nodularis nasi. Clinically, chondrodermatitis presents as a singular, painful, ulcerated papule or nodule that results from prolonged pressure, trauma, sun exposure, or vascular compromise. The clinical features resemble those seen in skin cancer, and a biopsy is often required to secure the diagnosis. Histopathologic analysis of chondrodermati

Laryngotracheal stenosis requiring emergency tracheostomy as the first manifestation of childhood-relapsing polychondritis.

Relapsing polychondritis is a rare childhood disorder of unknown etiology, characterized by inflammatory, recurrent and destructive cartilage lesions. The chondritis could be widespread and involves generally laryngeal and auricular hyaline cartilages. We described a 9 years and 4 months old girl, who presented recurrent acute laryngotracheitis and laryngotracheal stenosis, which were the first manifestations of relapsing polychondritis, and was submitted to emergency tracheostomy. She also had ear condritis and arthritis, being treated with prednisolone and methotrexate. In conclusion, we rep

[Three patients with complications following piercing of the auricular cartilage].

In three patients, a 21-year-old man and females aged 16 and 18 years, piercing through the auricular cartilage was followed by an infection. Treatment left behind a residual deformity, for which a reconstruction was carried out with a satisfactory result. The risk of infection following piercing through the avascular cartilage of the helix and tragus of the pinna is greater than following piercing of non-cartilaginous tissue such as the ear lobe. (Peri)chondritis leads to chondral necrosis and subsequent deformities. It is important to recognise the early features of perichondritis, which inc

[Otoplasty: results of a modified form of Mustardé's method].

In order to investigate the results obtained after surgical treatment of prominent ears with a modified method basically derived from the technique of Mustardé 340 patients had been interrogated and 150 had been examined postoperatively. In 91.4% of the cases the patients themselves and in 84.7% the surgeon was satisfied by the postoperative result. Recurrence was observed in 17% and was due partly to the intermittent use of a resorbable suture material. In some cases recurrence consisted in a slight asymmetry, that was not really disturbing and did not need a further correction. A fundamental

[Difficult tracheal intubation in paediatric: myth or reality?].

Difficult tracheal intubation is a rare event according to the airway feature in child. This situation is mostly predictable, occurring in identified malformations and in specific diseases. Pre-operative clinical evaluation must detect facial abnormalities (lip or palate cleft, microtia, facial asymmetry.), micro or retrognathia, limited mouth opening, reduced distance between thyroid cartilage and chin, macroglossia and external ear malformations. According to this clinical evaluation, a strategy for managing difficult tracheal intubation is planned.

Type II collagen-induced auricular chondritis.

Sensitization to type II collagen induced inflammatory destruction of elastic ear cartilage, as well as arthritis, in Sprague-Dawley rats. This auricular lesion morphologically resembled relapsing polychondritis in humans. Moreover, in the experimental model, as has been reported in human disease, extraarticular chondritis occurred in the context of immunologic sensitivity to native type II collagen. These similarities suggest that type II collagen-induced auricular chondritis may provide insights about the pathogenesis of relapsing polychondritis.

Reconstruction of congenital tragal malformations accompanied by dystopic cartilage growth (accessory tragus).

BACKGROUND: Patients with solitary-type congenital tragal malformations accompanied by dystopic cartilage growth (accessory tragus) show not only a disfigured or absent tragus but also unnecessary tragal or pretragal lumps. Furthermore, an absent tragus causes a wholly exposed external auditory canal. MATERIALS: Sixty-five ears (54 patients) showing congenital tragal malformations accompanied by dystopic cartilage growth were reconstructed from March of 1991 to February of 2014. Reconstructions were reviewed using medical records and photographs, surgical methods were analyzed, and postoperat

New concept in microtia repair.

Reconstruction of the severely deformed external ear has been a frustrating experience for many patients and surgeons alike. Until recently, children with microtia (1:10,000 births) have been subjected to multiple procedures with frequently ungratifying results. Because of the rarity of this deformity, most surgeons have had little personal experience with the common problems encountered in microtia reconstruction. The problems of (1) proper location of the reconstructed ear, (2) size and shape, and (3) durability of the implanted graft can be prevented by adherence to newer principles and tec

[Plastic surgical reconstruction in microtia. A follow-up study].

During the period 1967-1987, 26 ear reconstructions were carried out in the Department of Plastic Surgery, Rigshospitalet, Copenhagen. These 26 patients were followed up, partly from the hospital records and partly by means of a questionnaire. Twenty-two patients replied to the questionnaire. The average age was 8.8 years at the commencement of the reconstruction and the average duration of hospitalization was 41 days. The reconstructions employed autologous costal cartilage. Minor complications were observed in approximately half of the patients. One patient developed pneumothorax postoperati
