Comparing the impact of supine and leg elevation positions during coronary artery bypass graft on deep vein thrombosis occurrence: a randomized clinical trial study.

Deep vein thrombosis (DVT) is a common preoperative complication that occurs in patients who undergoing coronary artery bypass grafting surgery (CABG). Early ambulation, elastic stockings, intermittent pneumatic compression, and leg elevation, before and after surgery, are among preventative interventions. The goal of the study was to compare the effect of supine position with that of leg elevation on the occurrence of DVT during CABG and after, until ambulation. Between October, 2008, and May, 2011, a total of 185 eligible CABG patients admitted to the Cardiac Surgery Unit were randomly assig…

Intercostal muscle flap for decreasing pain after thoracotomy: a prospective randomized trial.

BACKGROUND: Intercostal nerve damage is often suggested to be the cause of pain after thoracotomy. METHODS: This was a prospective randomized study of 120 patients who had posterolateral thoracotomy. They were equally divided into two groups, the first in which intercostal muscle flap and intracostal sutures were used, and the other group in which the usual pericostal sutures were used. These two groups were compared regarding operative time, rib fracture, postoperative pulmonary functions, time to ambulation, pain score in the first week, doses of analgesics injected in the epidural catheter

Laparoscopy-assisted subtotal gastrectomy under thoracic epidural-general anesthesia leading to the effects on postoperative micturition.

BACKGROUND: Recently, the laparoscopic surgical technique has been widely applied to major surgery as it can minimize surgical incision, reduce blood loss, and shorten hospital stay. In this study, we evaluated the effectiveness of the laparoscopic surgery on postoperative micturitional function in comparison to open surgery. METHOD: Sixty patients undergoing subtotal gastrectomy were assigned to either laparoscopic (L group, n = 30) or open surgery (O group, n = 30) groups. The combined thoracic epidural-general anesthesia was performed on all patients, and epidural patient-controlled analge

Clinical and aesthetic outcomes of transumbilical single-port versus multi-port laparoscopic ovarian cystectomy.

INTRODUCTION: Ovarian cysts often require surgery. This study compared clinical outcomes and aesthetic satisfaction between transumbilical single-port (SP-LC)s and traditional multi-port laparoscopic cystectomy (MPL). METHODS: A retrospective analysis was conducted on patients undergoing laparoscopic ovarian cystectomy from October 2023 to September 2025. Patients were divided into MPL and SP-LC groups based on surgical technique. Perioperative parameters were recorded. Postoperative pain was assessed using the Visual Analog Scale (VAS) at 24 and 72 hours. Inflammatory markers were measured p

Nurses' Knowledge, Attitudes, Practices and Perceived Barriers to Early Mobilization of Patients in Intensive Care Units in Southeastern Iran.

BACKGROUND: Early mobilization (EM) in Intensive Care Units (ICUs) is a well-established strategy to mitigate complications of immobility, including ICU-acquired weakness, delirium and prolonged hospitalization. Despite strong clinical evidence and guideline recommendations supporting its safety and efficacy-even for mechanically ventilated patients-EM remains underutilized globally. Nurses, as frontline caregivers, play a pivotal role in EM implementation; however, gaps in their knowledge, attitudes and practices, along with perceived barriers, hinder its routine application, particularly in

A protocol for the rehabilitation of patients undergoing mechanical ventilation from the ICU to the post-hospital phase: the MOVE-ICUtoHOME, a multicentre randomised controlled trial.

INTRODUCTION: Intensive care unit-acquired weakness (ICU-AW) in patients with acute respiratory failure requiring invasive mechanical ventilation (IMV) is associated with reduced functional exercise capacity and increased mortality. Early mobilisation in the ICU may improve functional outcomes and reduce length of stay; however, evidence remains limited, protocols are heterogeneous and post-discharge rehabilitation is frequently lacking. This study aims to evaluate the effects of a continuous care programme combining early intensive ICU mobilisation with post-hospitalisation rehabilitation on

Promoting a Multidisciplinary Team for Early Mobilisation in Mechanically Ventilated Patients: An Innovation in Health Professional Education-A Quasi-Experimental Pre-Post Study.

BACKGROUND: Early mobilisation (EM) in mechanically ventilated (MV) patients is a safe and feasible, evidence-based practice with positive health outcomes. It decreases intensive care unit (ICU)-acquired weakness and delirium, hospital length of stay, duration of mechanical ventilation, readmissions and billings, morbidity and mortality. It improves the physical and mental health of critically ill patients. Healthcare professionals' education and readiness are key in establishing EM for practice. Although it is practised in many high-income countries, it has not yet been introduced as a standa

Conservative or operative therapy in patients with a fragility fracture of the pelvis: study protocol for a prospective, randomized controlled trial.

BACKGROUND: The incidence of fragility fractures of the pelvis is rising. Whereas the treatment for FFP type I, III, and IV is clear, the optimal treatment for FFP type II remains a topic of discussion. Traditionally these fractures have been treated conservatively. However, there is a shift toward early surgical stabilization with percutaneous screw fixation to reduce pain and promote mobility in an already frail patient population. High-quality evidence, however, is lacking. Therefore, a randomized clinical trial was designed to compare conservative management to early percutaneous screw fix

Effect of early mobilization on the development of pneumonia in patients with traumatic brain injury in the neurosurgical intensive care unit: A historical controls study.

BACKGROUND: Pneumonia has a high incidence in traumatic brain injury (TBI) patients and lacks effective treatments. Early mobilization (EM) may be a potentially effective treatment. AIM: To explore the impact of EM on TBI-related pneumonia in the neurosurgical intensive care unit (NICU). STUDY DESIGN: This study was a historical control study. 100 TBI patients who received EM intervention were prospectively included as the experimental group (EM cohort), and 250 TBI patients were retrospectively included as the control group. The propensity score matching (PSM) method was employed to balance

GET-UP Trial 1-year results: long-term impact of an early mobilization protocol on functional performance after surgery for chronic subdural hematoma.

OBJECTIVE: Evidence on timing for mobilization after chronic subdural hematoma (cSDH) surgery is heterogeneous, and practices differ considerably among neurosurgical centers. The Impact of an Early Out-of-Bed Paradigm in Postoperative Outcomes of Chronic Subdural Hematomas: GET-UP Randomized Prospective Trial (GET-UP Trial) is a randomized clinical trial comparing a postoperative early mobilization protocol to bed rest. Previously reported results at clinical discharge and 1 month after surgery indicated a decreased risk of medical complications in the early mobilization group. Herein, the aut

Slow Recovery of Weight Bearing After Stabilization of Long-Bone Fractures Using Elastic Stable Intramedullary Nails in Children.

Stabilization of diaphyseal long-bone fractures using elastic stable intramedullary nails (ESIN) in children promises early mobilization and rapid resumption of full weight bearing. We evaluated the duration of postoperative functional rehabilitation after ESIN, measured by the time from stabilization until first partial weight bearing, full weight bearing, and resumption of school sports. Fifty children with unstable, displaced fractures of the femur or lower leg treated with ESIN between 2002 and 2012 were included in this retrospective analysis. We classified fractures according to the pedi

Efficacy and safety of very early mobilisation within 24 h of stroke onset (AVERT): a randomised controlled trial.

BACKGROUND: Early mobilisation after stroke is thought to contribute to the effects of stroke-unit care; however, the intervention is poorly defined and not underpinned by strong evidence. We aimed to compare the effectiveness of frequent, higher dose, very early mobilisation with usual care after stroke. METHODS: We did this parallel-group, single-blind, randomised controlled trial at 56 acute stroke units in five countries. Patients (aged ≥18 years) with ischaemic or haemorrhagic stroke, first or recurrent, who met physiological criteria were randomly assigned (1:1), via a web-based compute

Peri-prosthetic hip infections: in favour of one-stage.

Although it does offer certain obvious advantages, the one-staged revision approach in infected THA remains rare in the field of orthopaedics. Besides the reduced number of surgical interventions for patients, it is associated with a decreased inpatient hospital stay and quicker mobilisation. Furthermore, it might be the more cost-effective approach and enables a reduced duration of post-operative systemic antibiotics, usually less than 14 days in our setting. Technically speaking, the presence of a positive culture of a pre-operative aspiration and respective antibiogram are mandatory require

Reevaluation of predictive factors for complete recovery in dogs with nonambulatory tetraparesis secondary to cervical disk herniation.

The vast majority of dogs with cervical disk herniation experience cervical pain and only mild motor deficits; therefore, not much is known about the factors that predict recovery in dogs with nonambulatory tetraparesis (NAT) secondary to cervical disk herniation. In this retrospective study, we tested the hypothesis that two previously reported prognostic factors, site of disk herniation and severity of neurological deficits, are useful predictors of complete recovery. Overall, 20 (62%) of 32 dogs with cervical disk herniation-associated NAT had complete recovery. Site of disk herniation was

Activity restrictions after posterior lumbar discectomy. A prospective study of outcomes in 152 cases with no postoperative restrictions.

STUDY DESIGN: A prospective clinical trial was conducted. OBJECTIVES: To determine the feasibility of removing activity restrictions after surgery and encouraging early return to work; to ascertain the clinical and behavioral response to such a strategy; and to identify factors predictive of early return to work, preparatory to possible randomized clinical trials. SUMMARY OF BACKGROUND INFORMATION: Current practice usually entails several weeks to several months of restricted activities after lumbar discectomy to avoid disc "reinjury." Earlier work has suggested these restrictions may not be

[Hip dislocation without bone injuries].

Dislocation of the hip is rare and so we regard every hip dislocation as an emergency until it has been reduced. From 1977 to 1996 a total of 82 patients with hip dislocation were treated at the Traumatic Surgery Clinic, University of Vienna, Austria. The average age at injury was 29 years. Some 77 patients had unilateral dislocation of the hip, but only in 5 cases was bilateral dislocation found. Iliac dislocation was most common (54 patients). Our treatment consisted in reduction within 6 h of the injury under general anaesthesia (Böhler) and 14 days bedrest. Then the patients were mobilized
