The radiographs and patient charts of 47 children treated with closed reduction and percutaneous pin fixation of displaced supracondylar humerus fractures were reviewed. Twenty-seven fractures were fixed with crossed medial and lateral pins. Twenty fractures were treated with two parallel laterally placed pins. Baumann's angle on the anteroposterior elbow film and the humerocapitellar angle on the lateral elbow film were independently measured by the three authors on initial postoperative films and on films taken at the time of pin removal. No statistically significant differences regarding ma

OBJECTIVE: To maintain concentric reduction of an unstable elbow and to allow active exercises after the treatment of complex elbow injuries or their sequelae.
INDICATIONS: Recurrent dislocation or subluxation of the elbow after repair or reconstruction of bony, capsuloligamentous, and/or musculotendinous stabilizers. Tenuous fixation of large coronoid fractures due to fragmentation or osteopenia. Stabilization of the joint after fascial arthroplasty or debridement for infection, if the debridement destabilizes the elbow. Relative indication: maintenance of range of motion after release of co

Perilunate fracture dislocation with an elbow dislocation, it is referred to as a floating forearm. Very few cases have been described worldwide. We report a case of a 37-year-old man diagnosed with a floating forearm (trans-scaphoid perilunate fracture dislocation combined with a terrible triad injury), after a fall of the top of an olive tree. Urgent closed reduction of the elbow and wrist was obtained. A double approach to the wrist was used: dorsal approach to fix the scaphoid and to reduce the carpus and a volar approach to release the median nerve and repair the volar capsule. A lateral

The Outerbridge-Kashiwagi (O-K) procedure has conventionally been used for the treatment of osteoarthritis of the elbow and to treat posttraumatic sequelae including posttraumatic arthritis, stiffness, contracture, and ulnar neuritis. The procedure involves exposure of the posterior elbow joint as well creating a window posteriorly through the olecranon fossa to target anterior aspects of the elbow. Several case series have shown the O-K procedure to have good functional outcomes with minimal complications. Used mostly for the surgical treatment of adult osteoarthritis, the O-K procedure has n

Transphyseal fractures of the distal humerus are usually seen in children younger than 3 years of age and are considered as Salter-Harris Type I epiphysiolysis. Neonatal transphyseal distal humerus injuries are extremely rare. It usually occurs due to trauma during difficult labour but can also be seen after child abuse. Since the distal humerus is composed of cartilaginous tissue in newborns, it is difficult to make a diagnosis with direct radiography. Patients are often diagnosed with elbow dislocation. However, elbow dislocation is almost never seen under the age of 3 years. Transphyseal fr

OBJECTIVES: The surgical reduction of intra-articular olecranon fractures is judged primarily on the lateral elbow radiograph, as orthogonal imaging of the articular surface is not obtainable. We sought to determine surgeon accuracy in identifying intra-articular olecranon malreductions on the lateral elbow radiograph.
METHODS: Six human fresh-frozen cadaveric elbow specimens were sagittally sectioned in 5-mm increments after olecranon dissection, preservation of soft tissue envelope, and rigid fixation of the elbow in an external fixator. Three patterns of central intra-articular olecranon m

RATIONALE: The nonunion of olecranon fractures is uncommon in simple fractures, and it is challenging to treat surgically due to the disruption of the anatomy of the elbow joint. There is limited literature on surgical options, and several factors to determine the treatment, including the amount and quality of bone stock, age, and degree of articular damage.
PATIENT CONCERNS: A 58-year-old man presented at the clinic with neglected olecranon fracture for 1 year (case 1). A 74-year-old man (case 2) presented with consistent pain and limited of motion after surgery for olecranon fracture.

We have reviewed 20 women and three men aged 22 to 73 years, who had sustained a Mason type-IIb fracture of the neck of the radius 14 to 25 years earlier. There were 19 patients with displacement of the fractures of 2 mm to 4 mm, of whom 13 had been subjected to early mobilisation and six had been treated in plaster for one to four weeks. Of four patients with displacement of 4 mm to 8 mm, three had undergone excision and one an open reduction of the head of radius. A total of 21 patients had no subjective complaints at follow-up, but two had slight impairment and occasional elbow pain. The me

PURPOSE: Comminuted coronal shear fractures of the distal humerus represent rare injuries and are difficult to treat, especially comminuted capitellum and trochlear fractures (Dubberley Type III). The on-table reconstruction technique of comminuted articular fractures may be an option, although it has not been reported in the coronal shear fracture of the distal humerus. The aim of the present case series is to determine the functional and radiological outcomes of on-table reconstructed Dubberley III fractures.
METHODS: A retrospective review was conducted of 10 patients with Dubberley type I

BACKGROUND Traumatic elbow varus posteromedial rotatory instability can involve an anteromedial coronoid fracture, proximal avulsion of the lateral collateral ligaments, and a tear of the ulnar collateral ligament posterior bundle, leading to chronic elbow instability, cartilage damage, and osteoarthritis. This retrospective study evaluated postoperative outcomes at 6 months in 9 patients with elbow varus posteromedial rotatory instability following an anteromedial surgical approach using a steel plate and high-strength suture. MATERIAL AND METHODS This retrospective study analyzed the data of

BACKGROUND: Aseptic loosening, polyethylene wear, and mechanical failure have limited the use of total elbow arthroplasty (TEA) in physically demanding patients. Newer implant designs have been introduced to improve mechanical performance. The purpose of this study was to report the results obtained after implantation of the Nexel TEA.
METHODS: Over a 3-year period, 2 surgeons implanted a total of 35 consecutive Nexel primary TEAs. The average patient age was 65 years, and standard TEA indications were utilized. Elbows were evaluated for pain, motion, the Mayo Elbow Performance Score, complic

OBJECTIVES: Total elbow arthroplasty (TEA) is becoming a popular alternative to arthrodesis for patients with end-stage elbow arthrosis and comminuted distal humeral fractures. Prior outcome studies have primarily focused on surgical findings. Our purpose is to determine the radiographic outcome of TEA and to correlate with clinical symptoms such as pain.
MATERIALS AND METHODS: This is an IRB-approved retrospective review from 2005 to 2015 of all patients with semiconstrained TEA. All available elbow radiographs and clinical data were reviewed. Data analysis included descriptive statistics an

BACKGROUND: The wrist/forearm complex is one of the most commonly fractured body regions, yet the impact tolerance of the wrist is poorly understood. This study sought to quantify the injury tolerance of the adult male forearm-wrist complex under loading simulating axial impact to an outstretched hand.
METHODS: Fifteen isolated cadaveric forearm/wrist specimens were tested. Loading was applied via an instrumented drop tower device designed to impact the palmar surface of the hand with the wrist extended to approximately 90 degrees. Impact severity was modulated by adjusting the boundary condi
