Preliminary evidence for enhanced auditory cortex activation and mental development after gene therapy in children with autosomal recessive deafness 9.

Individuals with congenital deafness that have received gene therapy represent a unique group who experience hearing recovery and speech development. However, it is unclear how hearing-related cortex changes because of gene therapy. Here we study neural processing in ten patients using functional near-infrared spectroscopy and electroencephalography during a six-month follow-up period. Patients showed an enhancement of activation in the auditory cortex, particularly in parts of the Sylvian parietotemporal area while listening to music. Activation in the right anterior temporal lobe and left Sy…

Laugh-induced seizure: a case report.

INTRODUCTION: A laugh-induced seizure is an unrecognized condition and to the best of our knowledge no case has been reported in the medical literature until now. We present an interesting and extremely rare case in which laughing generated the seizure activity that was recorded and confirmed by video electroencephalography. CASE PRESENTATION: A 43-year-old obese Caucasian man with history of bipolar disorder and chronic headache presented with multiple episodes of seizures, all induced by laughter while watching comedy shows. Each episode lasted approximately five seconds. In each instance, …

Subclinical seizures as a pitfall in 18F-FDG PET imaging of temporal lobe epilepsy.

A 61-year-old man with history of heroin abuse, hepatitis B, hepatitis C, and hypertension was evaluated for seizures. MRI findings were concerning for temporal epilepsy. A brain 18F-FDG PET study showed a hypermetabolic focus in the left temporal lobe, although the patient was asymptomatic during the scan. Later review of electroencephalography recordings revealed a subclinical seizure during imaging. A whole-body 18F-FDG PET scan performed 4 days later for cancer screening purposes, during which the electroencephalography tracings were normal, showed no abnormal metabolic activity in the bra…

Management of children with nonfebrile seizures in the emergency department.

BACKGROUND: Children with seizures are often referred to the emergency department where they are typically evaluated by a physician with lim … lood tests were performed in almost all visits; a minority also involved the use of brain scans and electroencephalography. A neurologist was consulted in about half the visits. Only electroencephalography and neurologic consultation contributed significantly to the final decision. Hospitalization was recommended in 71% of cases, but it was usually short term and not ac …

Paroxysmal tonic downgaze in two healthy infants.

A 5-month-old boy and 7-month-old girl had episodes of downward eye deviation starting at age 5 months, lasting seconds to minutes, and associated with stiffening of the extremities in one case and grasping, flailing upper extremity movements and retroflexion of the head in the other. There were no other clinical abnormalities. Electroencephalography and MRI were normal. The episodes stopped after 6 to 12 weeks and there have been no sequelae. This idiopathic condition resembles paroxysmal tonic upgaze in infancy. Until further documentation clearly establishes that this phenomenon is benign, …

Chronobiological Spatial Clusters of Cortical Regions in the Human Brain.

PURPOSE: We demonstrate that different regions of the cerebral cortex have different diurnal rhythms of spontaneously occurring high-frequency oscillations (HFOs). METHODS: High-frequency oscillations were assessed with standard-of-care stereotactic electroencephalography in patients with drug-resistant epilepsy. To ensure generalizability of our findings beyond patients with drug-resistant epilepsy, we excluded stereotactic electroencephalography electrode contacts lying within seizure-onset zones, epileptogenic lesions, having frequent epileptiform activity, and excessive artifact. For each…

Electroencephalographic Density Spectral Array monitoring in pediatric anesthesia: clinical background and practical applications.

Depth of hypnosis monitoring using numerical index based processed electroencephalography is part of the standard anesthesia equipment in many operating rooms. Unfortunately, this method has its limitations, especially in pediatric patients. Therefore, there is an increasing interest in raw electroencephalography parameters. Electroencephalographic density spectral array (DSA) monitoring is a real-time technique that can be used in all age groups and with all common anesthetics. We present a practice-oriented introduction to the principle of DSA monitoring in pediatric anesthesia for pediatric…

Quantitative Electroencephalographic Changes Associated With Brain Tissue Hypoxia After Pediatric Traumatic Brain Injury: A Retrospective Exploratory Analysis.

PURPOSE: Brain tissue hypoxia is associated with poor outcomes after pediatric traumatic brain injury. Although invasive brain oxygenation (PbtO 2 ) monitoring is available, noninvasive methods assessing correlates to brain tissue hypoxia are needed. We investigated EEG characteristics associated with brain tissue hypoxia. METHODS: We performed a retrospective analysis of 19 pediatric traumatic brain injury patients undergoing multimodality neuromonitoring that included PbtO 2 and quantitative electroencephalography(QEEG). Quantitative electroencephalography characteristics were analyzed over…

Roles of Testosterone and Estradiol in Mediation of Acute Neuroendocrine and Electroencephalographic Effects of Sevoflurane During the Sensitive Period in Rats.

Testosterone (T), predominantly acting through its derivative 17β-estradiol (E2), regulates the brain's sexual differentiation in rodents during the perinatal sensitive period, which mirrors the window of vulnerability to the adverse effects of general anesthetics. The mechanisms of anesthesia's adverse effects are poorly understood. We investigated whether sevoflurane alters T and E2 levels and whether they contribute to sevoflurane's acute adverse effects in postnatal day 5 Sprague-Dawley rats. The rats underwent electroencephalography recordings for 2 h of baseline activity or for 1 h befor…

Sleep neuroimaging: Review and future directions.

Sleep research has evolved considerably since the first sleep electroencephalography recordings in the 1930s and the discovery of well-distinguishable sleep stages in the 1950s. While electrophysiological recordings have been used to describe the sleeping brain in much detail, since the 1990s neuroimaging techniques have been applied to uncover the brain organization and functional connectivity of human sleep with greater spatial resolution. The combination of electroencephalography with different neuroimaging modalities such as positron emission tomography, structural magnetic resonance imagi…

Surgery for intractable epilepsy in a 14-year-old girl.

We report on a 14-year-old girl who presented with a 2-year history of simple and complex partial seizures with secondary generalisation. Monotherapy using carbamazepine and combination therapy using carbamazepine and gabapentin had been tried within the 2 years before presentation. Seizure control, however, was poor. Magnetic resonance imaging showed structural abnormalities over the right occipital and temporal lobes. Continuous scalp video electroencephalography was performed over 4 days, during which six clinical seizures were associated with electroencephalography changes at the right occ…

[Status Epilepticus: Convulsive and Nonconvulsive].

Status epilepticus(SE)is common, particularly among neurological emergency physicians. SE includes convulsive SE(CSE)and nonconvulsive SE(NCSE). Recently, the number of elderly patients with status epilepticus has increased. Most patients were diagnosed with NCSE. Prolonged SE often impairs neuronal function and is sometimes life threatening. Prompt diagnosis of SE and treatment initiation are essential for good outcomes. However, an accurate diagnosis of NCSE is difficult because the clinical symptoms of NCSE may vary, and electroencephalography may show various characteristics. Recent advanc…

[Characteristics of the effects of artificial alkalosis on electrical activity of the brain and ultrastructure of blood cells in oncologic patients].

The authors examined 40 patients with malignant tumors of various histogenesis, sites and extent, as well as 5 patients with benign tumors and other non-tumorous diseases. They also studied their electroencephalography and peripheral blood lymphocytic and erythrocytic ultrastructure in metabolic alkalosis temporarily induced by intravenous sodium hydrogen carbonate. In cancer patients without late metastases, alkalosis caused a transient normalization of previously altered electroencephalography, erythrocyte disaggregation and substantially reduced the count of killer cells in small and middle…

In vivo proton MRS to quantify anesthetic effects of pentobarbital on cerebral metabolism and brain activity in rat.

To quantitatively investigate the effects of pentobarbital anesthesia on brain activity, brain metabolite concentrations and cerebral metabolic rate of glucose, in vivo proton MR spectra, and electroencephalography were measured in the rat brain with various doses of pentobarbital. The results show that (1) the resonances attributed to propylene glycol, a solvent in pentobarbital injection solution, can be robustly detected and quantified in the brain; (2) the concentration of most brain metabolites remained constant under the isoelectric state (silent electroencephalography) with a high dose …

Brain targeted transcranial administration of diazepam and shortening of sleep latency in healthy human volunteers.

Application of medicated oils on scalp had been practiced for centuries in the Ayurvedic system of medicine in diseases associated with the central nervous system. It is possible that the effectiveness of the therapy may be a result of targeted delivery of active compounds to the brain transcranially. Evidence also comes from two previous studies with positive results on brain targeted transcranial delivery of methadone base and diazepam on rat models. Possibility of transcranial drug delivery was investigated in healthy human volunteers using electroencephalography techniques by assessing the…

CoolSim: using industrial modeling techniques to examine the impact of selective head cooling in a model of perinatal regionalization.

OBJECTIVE: A selective head-cooling device for the treatment of moderate to severe hypoxic-ischemic encephalopathy has been approved by the Food and Drug Administration for use in the United States. To reflect the complexity of health care delivery at the systems level, we used the industrial modeling technique of discrete event simulation to analyze the impact of various deployment strategies for selective head cooling and its partner technology, amplitude-integrated electroencephalography. METHODS: We modeled the course through the perinatal system of all births in Massachusetts over a 1-ye…

Changes in the Term Neonatal Electroencephalogram with General Anesthesia: A Systematic Review with Narrative Synthesis.

BACKGROUND: Although effects of general anesthesia on neuronal activity in the human neonatal brain are incompletely understood, electroencephalography provides some insight and may identify age-dependent differences. METHODS: A systematic search (MEDLINE, Embase, PubMed, and Cochrane Library to November 2023) retrieved English language publications reporting electroencephalography during general anesthesia for cardiac or noncardiac surgery in term neonates (37 to 44 weeks postmenstrual age). Data were extracted, and risk of bias (ROBINS-I Cochrane tool) and quality of evidence (Grading of Re…

Late-onset persistent visual hallucinations with epileptiform discharge.

We report the case of a 58-year-old female without a history of psychosis or epilepsy presenting with abrupt late-onset persistent visual hallucinations that had prominent epileptiform discharges with a diffuse sharp and slow wave complex on electroencephalography. Except for the visual hallucinations, other psychotic symptoms and epileptic seizures, such as disturbance of consciousness and convulsion, were not observed. The epileptiform discharge disappeared within 20 days after initiation of treatment with sodium valproate. Clinical improvement commenced after the electroencephalography impr…

Sporadic Creutzfeldt-Jakob disease in a patient with episodes of nonconvulsive status epilepticus: case report.

Creutzfeldt-Jakob disease is the most common form of human prion diseases. A 57-year-old woman was transferred to our Department from a local hospital, where she had been treated for two weeks due to consciousness disorders and convulsive epileptic attacks that progressed to refractory status epilepticus. Electroencephalography showed diffuse spike-wave complex discharges and development of nonconvulsive status epilepticus. The causes of metabolic encephalopathy and paraneoplastic syndrome were ruled out. A combination of clinical features and findings of diagnostic procedures including electr…

Improving Multiscale Fuzzy Entropy Robustness in EEG-Based Alzheimer's Disease Detection via Amplitude Transformation.

This study investigates the effectiveness of amplitude transformation in enhancing the performance and robustness of Multiscale Fuzzy Entropy for Alzheimer's disease detection using electroencephalography signals. Multiscale Fuzzy Entropy is a complexity measure particularly sensitive to intra- and inter-subject variations in signal amplitude, as well as the selection of key parameters such as embedding dimension (m) and similarity criterion (r), which often result in inconsistent outcomes when applied to multivariate data, such as electroencephalography signals. To address these challenges an…
