After reviewing a typical example of functional and morphologic correlations in a cochlear and vestibular comparative study of three aminoglycosidic antibiotics and discussing clinical and experimental observations in Menière's disease and experimental endolymphatic hydrops, some difficulties in studying morphofunctional correlations are discussed. A particular electrophysiologic method, based on electrical stimulation of the ear and acoustic masking, notably with continuous pure tones, is illustrated in normal and pathologic experimental conditions in the guinea pig. This method, which allows…

Several clinical reports suggest allergy as a causative or associated factor in the etiology of Ménière's Disease or endolymphatic hydrops. The A.A.O.O. subcommittee reported in 1972 its criteria for a therapeutic improvement in Ménière's Disease. This important report allows for clinical studies to proceed in a scientific manner using the subcommittee's criteria to evaluate the results. In an ongoing study utilizing a double-blind crossover technique, fourteen Ménière's patients have been evaluated for allergies utilizing the Rinkle and Lee techniques for inhalent and food allergies. Several …

OBJECTIVES: This study was aimed to elucidate the diagnostic significance of the summating potential (SP)-action potential (AP) ratio and th … stage 3 (p <0.05).
CONCLUSIONS: An enhanced SP/AP ratio might not always indicate the presence of endolymphatic hydrops associated with an increase in endolymphatic pressure. An increased AP con-rar difference might reflect the presence of a biased basilar membrane resulting from an increased endolymphatic pressure, and hence it is diagnostically essential to simulta …

The pathophysiology of hearing deterioration in Ménière's disease (MD) is unclear. Hair cell loss has been proposed to be the cause of severe hearing loss in Ménière's disease. The cochlear microphonic (CM) is known to be the receptor potential of the outer hair cells in the cochlea. This study measured the CM in Ménière's disease and investigated its relationship with the degree of hearing impairment and endolymphatic hydrops. Transtympanic electrocochleography (ECoG) using rarefaction (RAR) and condensation (CON) tonebursts at 1 kHz was performed on 130 ears of 119 patients. Ninety six ears …

Meniere's disease is a disease of the inner ear characterized by a triad of symptoms: vestibular symptoms, auditory symptoms, and pressure. The pathologic correlate of Meniere's disease is endolymphatic hydrops and the etiopathogenesis involves a deficiency in the absorption of endolymph. The pathophysiology of the symptoms is still disputed: membranous ruptures, pressure and mechanical displacement of the end organs, or obstruction followed by an abrupt clearance of the endolymphatic duct. The course of the disease may be progressive or nonprogressive and, in addition to the typical presentat…

38 patients with proven acoustic neuroma were examined. Intratympanic electrocochleography was performed with alternating clicks of 90 dB (n … he changes such as increased SP are found in patients with Meniere's disease, supposingly caused by endolymphatic hydrops. For tumours of the acoustic nerve the hydrops theory does not seem likely; the loss of amplitude due to adaptation within the first neuron seems to point to mechanical reasons due to the pressure of the tumour itself.

INTRODUCTION: Menière's disease is a clinical entity with no definitive objective testing. It has been hypothesized that underlying endolymphatic hydrops stiffens the basilar membrane leading to increased speed of the acoustic stimulus, therefore traveling wave velocity has been proposed as an objective test to aid in the diagnosis. The objective of this study is to compare electrocochleography frequency-specific action potential latency, basilar membrane traveling wave time, and summation to action potential (SP/AP) ratio in Menière's and non-Menière's patients.
METHODS: Tympanic electrococh…

This work was based upon the need to look at the role of surgical intervention in cases of presumed hydrops encountered in usual medical practice. A retrospective cohort study of 292 consecutive cases diagnosed as Meniere's disease was used to identify subjects severely enough affected to elect surgical relief. Two nested case-control design studies were subsequently conducted: one to compare surgical failures (4 cases) with successes (15 controls); and the other to compare the surgical group (19 cases) with a uniform random sample of all presenting Meniere's disease cases (10 controls). These

Spontaneous intracranial hypotension (SIH) is a rare condition characterized by orthostatic headache, pulsatile tinnitus, vertigo, nausea, and fluctuating hearing loss; this latter seems to be due to the development of cochlear endolymphatic hydrops following negative cerebrospinal fluid (CSF) pressure transmitted to the perilymphatic space through a patent cochlear aqueduct. We here describe a case of bilateral progressive sensorineural hearing loss (SNHL) due to intracranial hypotension caused by an undiagnosed spontaneous CSF leak from a skull base defect in a middle-aged woman. To the best…

It has been known for some time that musical hallucinations occur in deaf patients. This has been ignored, as it has been believed that neurological and psychiatric causes predominate. However, despite specific appeals, no one with musical hallucinations and a lesion in the brain but not in the ear has been produced. The postulated otological basis is a hyperactive state of the ear, a slight endolymphatic hydrops or pre-Meniere's disease. The hallucinations probably develop out of rhythmic tinnitus. A review of all the very disparate states and diseases supposedly causing musical hallucination…

The early results of hyperosmolar perfusion of the scala tympani for advanced Meniere's disease were reported in 1987. Thirty patients were treated in this way between March 1981 and March 1986; follow-up is now 2 to 7 years. The rationale of cochlear dialysis is to reduce the endolymphatic hydrops of the pars inferior by filing the scala tympani with a sterile solution of mannitol in 5% dextrose that has an osmolality of 350 m.osm per liter, which compares with a scala media osmolality of circa 300 m.osm per liter. The technique is a qualitative and quantitative extension of the "salt in the …

The endolymphatic sac (ES) has been suggested to serve an important immunological role within the inner ear. Meanwhile, immunological mechan … logical derangement of the ES function could be the first step in a cascade of reactions leading to endolymphatic hydrops and presenting as Meniere's disease.

Serial electrochochleogram (ECochG) recordings were obtained over a long time span from 24 patients with Ménière's disease showing dominant negative SP. The long-term fluctuation of responses (especially SP) was evaluated. Dominant -SP or abnormally increased SP/AP ratio has been unchanged over long periods in most operated or nonoperated patients. It is assumed that the generation of -SP dominance may chiefly result from the malfunction of hair cells, which may be caused by abnormal changes in inner ear fluids associated with endolymphatic hydrops or by other unknown disturbances in the cochl…

This paper provides a new hypothetical explanation for the etiopathology and pathophysiology of Menière's Disease (MD), which to date remain unexplained, or incompletely understood. The suggested hypothesis will explain the close connection of MD and Migraine, the coexistence of endolymphatic hydrops (ELH) and Menière attacks and the signs of inflammation detected in the inner ears of MD patients. Although as yet unproven, the explanations provided appear highly plausible and could pave the way for the generation of the first animal model of MD - an invaluable asset for developing new treatmen…

Thirty guinea pig ears were studied to investigate the effect of endolymphatic hydrops on the cochlea. High K(+) solution was injected into the scala media, and cochlear microphonics (CM) and endocochlear potential (EP) were observed before, during and after the injection. The CM amplitude decreased rapidly after injection, ending in a depressed plateau value. By contrast, EP remained almost unchanged. By changing the composition of the solution it was suggested that the effect of the injection was mechanical one, rather than biochemical. In three ears, spontaneous recovery of CM was observed …
