We studied 50 hips in 46 patients with slipped capital femoral epiphysis (SCFE). Seven patients with acute cases were treated by closed reduction with excellent results and without any known complication. The cases in the remaining 39 patients were of the chronic type. The method of treatment was determined according to the posterior tilting angle: in situ pinning for angles less than 30 degrees, intertrochanteric osteotomy for angles between 30 and 60 degrees, and subcapital wedge osteotomy for angles greater than 60 degrees. Chondrolysis was observed in one case treated with intertrochanteri

Three children with azotaemic renal osteodystrophy were treated with 1,25-dihydroxycholecalciferol (1,25(OH)2D3). All showed clinical, biochemical, and radiological improvement within 6 months of starting treatment. There were no complications. The dose of 1,25(OH)2D3 required was 0-5 microgram per day for 2 children aged 22 and 30 months, and 2 microgram per day for a 15-year-old boy. 2 of the patients were receiving phenobarbitone and phenytoin and in one of them prior treatment with dihydrotachysterol 0-5 mg daily and 6 microgram 1alpha-hydroxycholecalciferol (1alphaOHD3) daily had failed t

In view of the lifelong persistence of the physis, the femoral head of rats may serve to model Perthes disease and slipped capital femoral epiphysis. To produce osteonecrosis, the blood supply of one femoral head of 133, 6-month-old animals was severed by circumferentially incising the periosteum of the neck and cutting the ligamentum teres. The rats were killed 7 days to 90 days postoperatively. Associated with resorption of the necrotic bone and marrow, remodeling of the epiphysis was characterized by an ingrowth of vascularized fibrous tissue, formation of new bone and some cartilage, archi

BACKGROUND: Evaluating postoperative patients with hardware is challenging following surgical intervention for hip maladies such as femoral neck fractures and slipped capital femoral epiphysis (SCFE). These children are at increased risk of developing avascular necrosis, and imaging may be requested to confirm or exclude this diagnosis. Children with Legg-Calvé-Perthes disease can be monitored for restoration of blood flow to the capital femoral epiphysis to guide management and help with prognosis. Although MRI is sensitive for detecting early avascular necrosis, the presence of hardware degr

CONTEXT: Genetic defects in many biological pathways, including activation of the Ras-MAPK pathway, cause short stature. Vosoritide, a C-type natriuretic peptide analog that inhibits this pathway, has been approved for use in achondroplasia.
OBJECTIVE: To determine whether vosoritide improves growth in children with disorders of the Ras-MAPK pathway, including RASopathies, ACAN, and NPR2 deficiency.
DESIGN: Prospective Phase 2 basket trial.
SETTING: Academic medical center.
PARTICIPANTS: Thirty prepubertal children aged 3 to 11 years with a RASopathy, ACAN or NPR2 deficiency and height ≤ -

Growth hormone prepared by recombinant DNA technology (somatropin) has been commercially available for over 11 years. More than 38,000 children have been treated with different growth hormone products. While the best response to treatment occurs in children with severe growth hormone deficiency, therapy with growth hormone will increase the rate of statural growth in children with short stature of many different aetiologies. There are few studies of the effect of growth hormone treatment of final adult height, and the magnitude of this effect is harder to gauge, particularly in children with i

UNLABELLED: In-situ screw fixation (ISSF) is the gold standard for treatment of slipped capital femoral epiphysis (SCFE). While central screw placement within the epiphysis is known to reduce the risk of intra-articular penetration, the impact of the screw starting point on the proximal femur and the resulting screw trajectory relative to the proximal femoral physis on radiographic or clinical outcomes remains unclear. This study evaluated whether screw starting point or inclination affects outcomes following ISSF. A retrospective review was performed of SCFE patients treated with ISSF at a si

We present a familial case of valgus slipped capital femoral epiphysis (SCFE). Charts of members of the same family having the condition were retrieved. Clinical and radiological examinations were conducted after 10 years of the initial presentation and treatment. Two siblings, brother and sister, were initially diagnosed with valgus SCFE and treated surgically. Normal clinical examination was found for both siblings and both, treated and untreated hips. A radiological examination for the parents revealed signs of valgus SCFE in both hips of the mother. No association with metabolic, hormonal

BACKGROUND: Children with slipped capital femoral epiphysis (SCFE) are often seen by an array of medical professionals prior to diagnosis. Patients with mild slips, slips with knee pain, or bilateral slips can occasionally present a diagnostic challenge that increases the risk of a delay in diagnosis and associated complications.
OBJECTIVES: This study introduces a new radiographic parameter, which we refer to as the S-sign, and analyzes its diagnostic utility on a frog-leg lateral radiograph.
METHODS: Twenty observers reviewed the radiographs from 35 patients with SCFE using Klein's line on

AIMS: Several studies have reported the safety and efficacy of subcapital re-alignment for patients with slipped capital femoral epiphysis (SCFE) using surgical dislocation of the hip and an extended retinacular flap. Instability of the hip and dislocation as a consequence of this surgery has only recently gained attention. We discuss this problem with some illustrative cases.
MATERIALS AND METHODS: We explored the literature on the possible pathophysiological causes and surgical steps associated with the risk of post-operative instability and articular damage. In addition, we describe supple

The number of skeletally immature athletes participating in organized sport is near an all-time high. For nearly half a century, the medical community has investigated the link between shoulder and elbow injuries with repetitive throwing. Despite substantial literature and research, several controversies still exist, including when to begin throwing breaking pitches. Furthermore, despite introduction of pitch recommendations for youth baseball, misconceptions, poor understanding of, and adherence to these guidelines persist. High-velocity and high-volume throwing and throwing while fatigued ar

INTRODUCTION: Osteotomy performed below the femoral neck plays a leading role in the treatment of slipped capital femoral epiphysis (SCFE). It results in anatomical reduction. Several modifications have been made to Dunn's original osteotomy technique. We have developed another modification to this technique that uses an anterior surgical approach on a traction table with fluoroscopy control.
HYPOTHESES: Will this technique help to reduce the number of complications? Will its results be superior to those achieved with the standard Dunn osteotomy procedure?
MATERIAL AND METHODS: This was a re

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2014eng
The variety of hip pathology that can be addressed in a minimally invasive fashion in the young, pre-arthritic patient has rapidly grown in parallel with technical advances in hip arthroscopy. However, the indications and limits of arthroscopy must be carefully defined and indications must evolve correspondingly to avoid an increase in failure rates and unsatisfactory clinical outcomes. Some diagnoses may be better and more comprehensively addressed with open procedures or combined surgical approaches. The purpose of this article is to provide an unbiased and evidence-based review of condition

Slipped capital femoral epiphysis (SCFE) is a relatively rare growth disturbance in the proximal femoral growth plate. It results in posterior displacement of the femoral head. Symptoms are an intermittent hip, knee or groin pain, which is exacerbated by sports, giving restricted hip motion, and sometimes a limping gait with out-toeing. SCFE is typically seen in overweight boys, but may also occur in normal-weight teenage girls. Early image diagnostic assessment of both hips is crucial, since slip severity and consequently the risk of disabling complications are strongly correlated to late dia
